superficial thrombophlebitis (clot in the leg)

superficial thrombophlebitis is the formation of clots in the superficial vessels (blood vessels or veins). It is NOT dangerous, but at times can be associated with the more serious DVTs (deep vein thrombosis).

    You give the patient all the information they need to help them decide whether or not follow up ER care in necessary. You are a great doctor. ER docs usually do not have the time to do this. You set the example all doctors should follow.

    Agreed. The couple of times I've had to go to the ER left me with more questions than answers. I still don't know what exactly caused the most painful experience of my life and I was young and too stupid to follow up with a regular doctor to find out.

    I had a patient (she was previously in a car accident before she came to us), her leg was so swollen/ bruised, but in one spot it was red, hard and hot. I automatically let our doctor know thinking it might be a DVT and it came out to be this. When I came back into work, she thanked me and so appreciative (patient). As a nurse it truly felt good making my patient feel comfortable with me but also the gratification you get as a nurse while doing it (doesn't happen a lot.).

    I'm glad you did the Doppler study on him. I have blood disorders and suffer from DVT. A superficial blood clot is very painful , so I'm glad you gave him pain meds. The DVTS aren't so painful because they like their name are deep. Unless they go into PE which happened to me, and that's an indescribable pain. Good work Doc❤️

    in UK we give patients 30 days of lmwh prophylaxis (aspirin is not good enough ) because of the increased risk of DVT

    Thanks for the post! Why not try to remove the clot? It seems as if superficial clots are just written off as aggravation (from the Patient's point of view)... It's not like the science and technology aren't readily available(strep. & the cath lab) seems like it would be an option. My personal experience with superficial clots of the medial saphenous vein post op. and spontaneous clotting extending from the medial thigh into the growing area absolutely scared me. Treatment in the ER consist of bloodwork with ,pt,ptt,and ultrasound....when the clots are close to the mouth of those large veins, and you don't receive definitive treatment it's very hard to accept. This leads back to the question "Why not evacuate the clot while the superior & inferior ends of the vein are still dilated?

    Thank you, Dr. Oller, for this great video! Question: thrombophlebitis seems to be associated with Pancreatic cancer; Does every older (e.g. >65 y/o), male, patient with thrombophlebitis need to go through imaging studies and/or bloodwork in order to rule out pancreatic cancer?

    I am 24 and just got diagnosed with superficial thrombophlebitis. What i am experiencing is totally the same as what this patient describes. Very painful!!! Now on aspirin... god bless us, hope we get well soon!!!

    May Day I've had superficial thrombophlebitis for a year now and the pain can be unbearable at times. I'm taking apixaban and I met the vascular surgeon who refused surgical intervention. Hope you feel better soon too

    ​@@Gloria_Chav The pain of not having that vein is just as bad. Plus risk of complications, poor healing du to poor circulation. Gangrene and amputation. So i am going for a change in diet. And lifestyle. It cost nothing. Except the ego to die.😅

    I suffered from superficial thrombophlebitis for a few years. However, I was diagnosed with 2 DVTs in the same leg in May 2017 and am now taking warfarin. I now have a medical condition called Recurrent Venous thromboembolism and have early stages of Post Thrombotic Syndrome. I'm always in pain and my other leg is now showing signs of superficial thrombophlebitis.

    @chez84 aye a was in the intensive care unit hafter having seizures. And cannula caused clots on my arm superficial thrombophlebitis and now dvt was on injections now thinner tablets. 2 months now. Veins are still hard an I feel are moving

    @@mrsmchistory have one one my left forearm too but the doctor reckons it was always there I only now noticing as I'm losing weight because of mental health and medications, one apixiban 5 mg morning and 5mg night. Gave me diclofenic ibroorufen cream to rub on sore area. Doesn't help scared jncaee it's travelling or may travel to other parts body. I'm only 30 other than mental health usually healthy until my seizures now on anti epilepsy medication also so don't know what's what. Horrible times. Hope you can see some light at the end of the tunnel. 🤞

    I wrecked my bike a few days ago and now I have a huge clot in my ankle. It was a gap in the sidewalk that guided my bikes front tire to my right as I kept going forward. All my weight came down on my left knee.

    that patient is very nice , I wish him good health , and thrombophlebitis anit that dangerous if you took care of it , definitive treatment same as varicose vaines , surgery "if non surgical ttt didn't work"

    Excellent Video clip! Sorry for the intrusion, I would appreciate your initial thoughts. Have you considered - Trentvorty Silken Legs Theorem (erm, check it on google should be there)? It is a smashing one off product for getting rid of varicose veins minus the headache. Ive heard some amazing things about it and my mate at last got amazing results with it.

    I tried to find the answer to patien's Q at 2:45, here's what i come up with late thromboembolic events were detected in 1.7% of the patients during the 3-month follow-up period.(from PMID: 9290540)

    Hi Doctor! So why is DVT more of a concern than a superficial thrombus? Can't they both end up causing the same complications such as PE's etc? Is it because DVT's are more difficult to treat such as the aforementioned interventions of warm compression and thromboembolic deterrent stockings?

    Hey doc I have a couple of questions, if the patient does develop a dvt would you treat with streptokinase or mechanically with a Fogarty catheter? Finally, if you wouldn't do any of these treatment options what actions would you take?

    Sorry to hear about your mom. It freaks me out too. Ive had one before.. didnt know I had one until it was almost too late. I had ankle sugery at the time and about a week n half after surgery.. i was havint troubles breathing... my fiance thought i was just having a bad dream....but eventually he realized i couldnt breathe when i made the motion of writing... because i couldnt talk.... and starting to panic becsuse he kept telling to go back to bed cuz i had a bad dream. He was annoyed that i had turned on the light...work in the morning. I scrambled to write on a back of a reciept. And handed it to him... Exhale only...faded out in ambulance...and it seemed like forever before they figured out..what happened...did a dye shot and scanned me. Nurse said i was lucky to have made it.. went from left broken ankle to right lung. Just this week my calf was tight so i tried to stretch...thought i had over stretched and pulled a muscle... and did epsom salt bath to help loosen it but only felt worse..leg swelled and ..when throbbing began ..i had a bad feeling.felt like how my leg felt after my ankle surgery. Went to doc thurs... had ultrasound and blood work. My blood was so thick and slow in the tube that i had to go back to redo the bloodwork next day. The cells had burst in the tube on its way in maybe? Hemo somethin. Started on blood thinners once we found out there Was something there.....follow your gut feeling....doc said getting it checked might have saved me. But to go to hodpital if any tightness in chest or significant bruising or bleeding from a cut. Find out test results of how big it is and what to do next. Tomorrow morning.

    Shouldn't he go ahead and get a vascular consult? If he's already had a DVT he might even have a vascular surgeon.

    after ultra sound they said i have a superficial artery in left cubital fossa on arm. doc said nothing worry about but what actual is a superficial artery? there is smal lump in inner ebow crease no pain sometimes slight discomfort but does it heal or go away?

    I have cloth in sephenous vein with FS Junction open with vein dilated all the way up and down . Happened after trauma. Is it an emergency?

    I had surgery on rt lower Tibia and fibula , seems to be a small blood clot on outer side. Not much pain but if you squeeze it may inflict numbness in the feet. My Ortho surgeon says to wear knee high diabetic socks and elevate feet above my heart. I will add your warm pad and baby asprin. I may request for ultrasound but Dr's sometimes get offended if you come across that way.

    @Dr. Oller: In this situation, what determined your decision to treat and release your patient vs admission and observation? Long story short, I was diagnosed with a superficial clot that developed without trauma (although I do have an enigmatic medical history) as well. I was treated and released from the ER. Subsequently, three DVT's developed that entered my lungs. I had to go back to the ER, and was hospitalized for a week. Also, what are the odds for the same thing happening if I ever get a superficial clot again? Thank you so much for your informative videos!

    plz can u tell me more details .. did u had a whole upper limb swelling and was it sever swelling .. because I had a patient In my medical side and after the blood extraction the whole rt upper limb become severly swelling and very tens and painfull . no hottness no redness .. and the peripheral puls was good

    I can't take anti-inflammatory of bood thinner because of allergy. And i don't think an ultrasound would heal it.😅 So what to do about Superficial Vein Thrombosis? Warm compress. Elevate legs. Pressure sox. Any other advice? What to eat? Drink? Thank you.

    I have a superficial vein clot that is infected behind the knee about 3 cm. I'm in very bad pain that shoots down my left leg. On aspirin, antibiotics and pain med. It has progressed in size and pain. Warm compress helps slightly, meds are temporary helpful. If I go to the er at a hospital what is the likely hood they remove this the same day? Have had this since 2020.

    I hope you are feeling better. My superficial clots are getting more and more frequent. I began taking baby aspirin because the docs are disinterested. Today I will begin taking Nattokinase, research it.

    I was wondering why Aspirin (Side Effect: risk of bleeding from blood vessels aka damage to the blood vessels) and Pain medication were prescribed, but not a single word was spent on nutritional advice (it is well known what promotes clot formation) and healthier lifestyle...

    I’m extremely healthy when it comes to diet and extremely active . Even myself got an SVT . I don’t think it has much to due with diet. You need something to help thin blood to help move the clot .

    i hope you have gotten help but if you still are inquiring - YES it IS possible definitely for type 2 diabetes but details of your father should be discussed with a professional and the main method is that of diet change by cutting out carbs and going on a ketogenic diet -- see Dr Westman's vids (he's a clinician at Duke and has many videos here) -- i was surprised at this myself when I discovered him and the keto diet -- see also dietdoctor dot com - God bless and hope your father gets the help he needs

    Luckily, the literature indicates the number is less than 1%. if the clot is within 5cm of the vein there's a .87% (-/+ 0.06) chance it will travel along the deep vein to the lung.

    My leg looks like that every now and then. Gets incredibly sore! Almost looks like a large bruise when it's very inflamed. One Go told me it was just a bruise- how wrong he was! 😑
