Stephen Cunnane - Can Ketones Slow Down Alzheimer’s?

A diagnosis of Alzheimer’s disease (AD) strikes fear into everyone. But can AD be slowed down; can we do something to delay when it starts and how hard it strikes? There is growing evidence that a conscientious prevention approach can help one resist AD. The prevention approach to AD is the same as for what are called the ‘chronic Western diseases’ - hypertension, type 2 diabetes and obesity. The hormone insulin appears to play a key role in AD. Insulin tends to be less effective at controlling blood sugar (glucose) in older people, especially if they are sedentary. This in turn decreases the amount of fuel getting to the brain making it harder for brain cells to talk to each other so memories gradually slip away. Our research shows that it is difficult to increase glucose supply to the brain once it has declined in older people, even with exercise. Ketones are the brain’s back-up fuel and the brain of someone with AD can use ketones as well as the brain of a young adult. Moderate exercise helps get more ketones into the brain. Some older people may not be able to exercise but one can make a ketone drink made from medium chain triglyceride (MCT) in one’s kitchen. We are in the middle of a trial with an MCT drink in people at high risk of AD. The early results show a significant benefit for memory. There will be no miracle cures for AD but a prevention approach may delay and/or slow it down.
A major focus of Dr. Cunnane’s research over the past 30 years has been to develop a better understanding of the role of omega-3 fatty acids and ketones in human brain development and function. His team uses PET and MR imaging to study the impact of aging on brain structure and fuel metabolism. This information is applied to the development of keto-neurotherapeutic strategies for reducing the risk and progression of aging-associated cognitive decline and Alzheimer’s disease. He has published over 300 peer-reviewed research papers and five books. Two of his books highlight the concept that the key role of ketones for normal human brain development was of importance to overcoming the nutritional, metabolic and structural constraints on human brain evolution: Survival of the Fattest: The Key to Human Brain Evolution (World Scientific 2005), and Human Brain Evolution: Influence of Fresh and Coastal Food Resources (Wiley, 2010). Dr. Cunnane was elected to the French National Academy of Medicine in 2009. In Oct 2017, he received the Chevreul Medal from the French Society for the Study of Lipids for outstanding contributions in the area of omega-3 fatty acid metabolism and the role of ketones in optimal brain function during aging.

    Thank you. At seventy, I have been low carb or Keto with intermittent fasting for three years. My A1C went down to 5.2. I am fighting memory loss also with exercise and painting classes. Finding joy in life is key.

    No one can find "joy in lifw...when the 95%...need to work 60-70 hours weekly in crappy jobs that bring no joy. Period....ie: the 95%.

    On the coconut oil and MCT oil issue, here's my experience with our mother in law. She doesn't have mild cognitive impairment, rather full blown Alzheimer's, having deteriorating to the point around a year and a half ago to no longer speaking but making whooping noises instead. Having read about ketogenic possibly helping I tried a quick test by giving her 3 teaspoons of coconut oil per day, thinking to do that for a month and see if there was any effect. If there was, then move on to a ketogenic diet for her. But in just a couple of days she had stopped whooping, and not long after was talking again. So I put her on a ketogenic diet and after a while dropped the coconut oil, since the ketogenic diet is more effective in producing ketones. Except that she started whooping again. So I went back to coconut oil - plus ketogenic diet. After a few weeks she was able to follow the TV and read (even though somewhat laboriously). She has been stable at this level for 14 months now with no further deterioration. I give her 4 teaspoons of coconut oil per day, every 3-4 waking hours, along with the ketogenic diet. This indicates to me that something else in coconut oil is going on as well as ketone production, which Dr Cunnane hinted at. It would be great if someone could do a study on this, as I don't think I'm the only one to have noticed the effect.

    +Ekondig G, WOW, this is excellent. Thank you for your comment, as I needed from anecdotal proof on giving coconut oil. I had my Dad (85) taking just a tsp. at each meal and his mild cognitive impairment never got any worse in a yrs' time. BUT the evil media and sinister family members sabatoged things. BTW - Never believe the media or the American Heart Assoc. FAKE news. The AMA gets paid for endorsement and will let anyone (even those with ties, as they too have ties to the big processed food industry and Soybean Oil industry). That report from June 2017 as well as another duplicate, (nothing new), headline that "Coconut Oil is pure poison", had to go back 48 YEARS, to 1970 to find a study or paper to agree with what they say about saturated fats. DEVIL IS IN THE DETAILS; the study was about HYDROGENTATED Coconut oil, v. Saturated Fatty Acids of Virgin Raw or unrefined Organic Coconut oil!!! Ha, ha! Also just the word saturated is misconceived by the American pubic. Saturated = SATIFIED that this fatty acid has All it's Carbon Atoms to it's Hydrogen Atoms. Good example, how a school bus has all the seats maxed out in the total area of the bus itself. What does this mean Saturated = IT DOES NOT OXIDIZE, like all other oils to include MONO-Unsaturated, with MONO meaning it is missing 1 [ONE] of it's Hydrogen or Carbon atoms and Air gets inside, creating unstable molecules called FREE RADICALS, (cancer tumors anyone)!!! -- Poly-Unsaturated, even worse, missing POLY, more than 1. This oil goes Rancid almost immediately after openin up a bottle or package of processed foods, or oil. So back to your experience; Yes and No that there something more to Coconut Oil and the ketones that the liver creates out of this miraculous Medium Chain Fatty Acid. 1st: According to Neuroscientist Dominic D'Agnostino, MCFA burst through the blood-brain barrier to feed the brain cells (neurons), and INCREASE BLOOD FLOW TO THE BRAIN BY 30%-40%. - In quells inflammation - It cuts down on B-Amyloid plaque - Shuts out virus, which also trigger an inflammatory response. You need to read Dr. Bruce Fife's book, Stop Alzheimer's now. Or any of his books, such as The Coconut Oil Miracle. - MCFA, stabilize blood sugar, blood pressure - MCFA used for over 90 yrs. for drug-resistant seizures in children Just endless and amazing. Spread the word, especially about the Fake & Corrupt American Heart Ass.

    @Catherine Murphy Yes, the MCT content was also my understanding for coconut oil. That was why I only considered it as a test before moving on to a ketogenic diet, and is why I was surprised that removing the coconut oil made things worse after her being on the ketogenic diet for a few weeks. The fairly low amount of MCT in coconut oil should allow the liver to produce a lot less ketones than a ketogenic diet, hence my question of whether there's something else involved as well.

    @@ekondigg6751 Mary T Newport used both MCT oils and Coconut oil on her husband. She states MCT (C8 and C10) oil is 60% of coconut oil. Lauric C12 is also in the coconut oil. She suspects it may have an as yet unknown benefit.

    I started a ketogenic diet and 18 hour intermittent fasting diet with MCT oil. Found out that MCT oil works for dementia. Wasn't sure but I started sprinkling MCT on my parents' food. My parents who both have dementia so I can do experiments. Turns out my dad is showing some slight improvements. My mom can still function very well but she cannot remember anything after one or 2 minutes. Today, my mom showed 2 signs that MCT is working. I showed her an unusual plant which started growing in the garden. It was my 2nd time telling her. Her response was "Yes, you told me several days ago." and she was not surprised or fascinated with the plant because she remembered. I am very pleased. My parents also take plasmalogen for their brain and this also works. Now they are taking MCT and plasmalogen.

    @@kylina3432 Hi Kylina, Thank you for your comment. My conclusion is that it works. My mom and dad are both in a care home since October because my mom wanted to be near my dad during the pandemic. They both take plasmalogen and MCT oil tablets now. I insisted that they take both of them instead of the medication dementia patients usually takes. (They both used to take donepezil but I made them stop) On March 15th, 2021, my mom could not remember me and that she was at her house having supper. I cried so hard because I knew her dementia got worse because of the pandemic and because I was not giving her plasmalogen DAILY due to the high price (about 80 dollars per person per month). I quickly started giving my mom daily and her condition improved. Then, when I wrote the above comment, I was testing MCT and I saw some clear improvements in my mom. She has never forgotten me since per episode in March. I think her condition is even better because she is having more interaction at the care facility. She can join a conversation and ask questions now. Before she went to the care facility, she mostly listened. Now, when I ask, "Do you know who I am?" she says, "Oh don't be silly. Of course I do." I hope you spread the word out. Doctors don't know anything about these "supplements". My mom started showing signs of dementia 10 years ago.

My husband's Mom has dementia and he has been hurting because his Mom has started to forget details about him and he is afraid she will forget him soon.     @@kylina3432 Here is the TV program I found 4 years ago on plasmalogen. kzread.info/dash/bejne/q4qfo5Z6fdi8pJs.html They say at the end 50% of the people show improvement and 50% no change. KZread translation is improving but not good. But I think you will get the idea.

    Maria, this is so interesting and I know how this can work. My daughter-in-law’s 2 1/2 year old niece was flown all over the country for having more then a 100 seizures a day. Finally they found a doctor in Delaware that took her off carbs using a Keto diet and within a few short weeks she was fine. She is now in 1st grade and doing just fine. I truly believe diet makes a big difference in all of us. To much processed food and a decline in our soil that is affecting all of us. The obesity is out of control. Too many fast food places and way to many preservatives.

    I can attest low carb-high good fat works. I had a few episodes of memory issues and transient amnesia. Within the first week of Keto my cognition improved. Watched improvement over 1.5yrs with testing with brain neurologist and psych eval to ensure I was ok and not dementia. I past, and so grateful...walked this path and researched alone. My doc told me there wasn't anything he could do...this attitude in allopathic medicine is so wrong! it was a scary journey alone.

    My Mother has AD and everytime she eats baanas, blueberries, or grapes, even oranges her symptomology gets exacerbated . She can no longer eat sweet fruits. This makes sense if we consider that there is insulin resistance in the brain

    My sister's husband was FINALLY diagnosed with Frontotemporal dementia (FTD), an early onset type of dementia, a behavioral, not so much a memory problem disorder. The frontal lobe and anterior portions of the temporal lobes begin to shrink, with entanglements of Tau protein seen at autopsy; everything at the back of the brain works fine. (Is anyone thinking a metabolic disorder of the brain?) He's had this for 5 years now and he's just now getting the Dx because doctors always misdiagnose this as a psychiatric problem or as Alzheimers, even though it's the second most frequent cause of dementia. For a year now I've been encouraging my sister to feed him MCT oil and stop giving him carbs but she didn't believe it. Nobody ever believes me even though I have a strong medical and biochem background. Sigh... I sure wish researchers would look into Diabetes Type 3 for Alzheimer's, which my mom died of, and for FTD which my brother in law is now dying of. Note - my sister has dished out seed oils and margarine in her house for years and I suspect that plays a major role! But mostly...it's insulin resistance, carbohydrate resistance.

    It is super frustrating when you have a scientific background and research this stuff and no one in the family believes you. It’s hard to watch family members deteriorate because they are too stubborn to try something you suggest.

    Cunnane is talking about buying MCT oil from health food store etc, or waiting to get approval. One researcher at NIH said he was working on isolating the active ingredient, presumably so he can patent it and make loads of money. Why? This is food, approval is not needed, buying it at a specialized shop is unnecessary. It is coconut oil. You do not need to do anything special to obtain. And thank God for that, even if I am not religious.

    it is MCT oil derived from coconut oil which is the active ingredient to facilitate ketone uptake, NOT coconut oil. MCT has 40%caprylic acid (C8) coconut 5%, This said coconut oil has a bioavailability effect on C8 which is not well understood, so you need to use both.

    @@bernardmauge8613 I have never heard that you need to manufacture coconut oil, concentrate it for it to work. No. Coconut oil quickly goes to the liver and then just as quickly is available to the brain to produce ketones. No fancy manufacturing needed. Nature took care of that process. Plus I suspect if humans concentrate it, put a label on it, it would be faulty. Money corrupts even nature.

    Great presentation and wish more Health care practitioners encouraged ketogenic diet. I live in Ocala and would have loved attending your presentation but was unaware. Sure sounds like my beloved veterinarian ( now retired) was in attendance and asked a question toward the end.

    I just walked my big rottweiler 1 1/2 miles and after listening to this I just took another tablespoon of mct oil. Alzheimers is a horrible disorder. At the pet food store as we were walking out I turned towards the back of the store instead of the front to leave. Something I never would have done ten years ago. So exercising and taking a tablespoon of mct oil seems I am still dropping in performance. I still understand correctly where I need to drive to, and my driving has improved 100% since taking this supplement. I have recently, (past month), also started k2 m7 supplement dropping atorvastatin to try to keep my heart arteries clear. Statins seem to not extend life at all and have bad effects on our brain tissue, never mentioned this to my primary physician since he does not seem to understand insulin resistance in the brain, but seems tied to the standard American Care philosophy. I no longer even believe in the standard American philosophy of cancer genesis, but the mitochondrial failure that causes cancer and can be reduced by giving the mitochondria enough keytones, especially in the brain.

    The keto diet is exploding. There is now so much on the internet about it. A low-carb high-fat diet solved my diabetes problem. No more meds. When people get the idea that medicine has sold us a lie with the pyramid diet, people will start to realize that diet is something to be experimented with. What doctors tell us is no longer valid. As folks say below, a low-carb high-fat diet can be really easy to do. More veg, a bit more fat, just no carbs. It is not a horrible greasy diet at all. Olive oil, sesame oil, coconut oil, MCT oil for bulletproof coffee, no supermarket oils. When you get into a low-carb high-fat diet, hunger pangs become much milder. Then you can easily do intermittent fasting, which is the ultimate way to get into dietary ketosis and get the body into repair mode (and autophagy).

    When I first started keto I wanted to share it with my family and friends but I was met with so much suspicion and resistance that I’ve stopped talking about it. They keep quoting their doctors or nutritionists who are also resistant of, or under-informed about keto.

    The Blind Faith people have in medicine and their doctors is based on really the fear of death. Everyone believes that the worst possible thing that could happen to them would be told to them and warned about by their doctor. So they have to believe that that doctor is all knowing. To actually doubt that would be to confront the idea that you can be dying and no one would tell you. And since almost everybody has a great fear of death even if it's not very conscious, it is climbing a huge mountain to come out of that fear and actually try Alternatives and give up your Blind Faith. We may think it's a slow change and you can slowly try different things but for people to entertain that idea is a mountain of difficulty

    What is really terrifying is that some very important people have serious issues with the mind. And they are not eating properly. I work for some people who need sharp minds and they don't have it. Is this why our country is so messed up? What happens when we start getting people to understand what their mind can be like on ketones. I have heard that silicon valley fasts for this purpose, and it seems, they own the world.

    What is so difficult about a ketogenic diet? I have been eating ketogenically for 2 years now with no difficulty at all. Why MCT oil? My sister's partner diagnosed with AD, has stabilized the progress of AD with coconut oil, a natural source of MCT , although she gives it to him by stealth, as he does not like the taste!

    Therapeutically yes, it may be difficult to fast, but eating ketogenically means that the "hunger hormone" (grehlin) is not stimulated and the "satiation hormone" (leptin) is not suppressed in the case of insulin resistance, so one can go a long time without feeling hunger. Having said that, it is all about adaptation, which can take up to 6 months. As a rule, I stop eating at 5 pm and start again around 9 the next morning, but I have sometimes swum 2500 metres crawl before eating at midday and felt fantastic for it!

    The fasting is not imperative, though. It's helpful, for sure, in increasing ketone levels (and for some folks, limiting your feeding window to 8 hours lets you get away with eating more carbs and still staying in ketosis). 18 hours a day is a much longer fast than is necessary to get the benefits -- 14-16 is perfectly sufficient and pretty easy to do if you consider that completing your last meal at 7 PM and starting your first meal at 9 AM gets you there. There's also some research that suggests that specifically for women, intermittent fasting can put stress on the body that counteracts the benefits. I was doing IF for awhile, and it worked OK for me, but if I get hungry, I eat, regardless of what time it is. Ultimately, every individual has to listen to their own body, understand its signals and respond appropriately.

    I agree! It is a very easy diet. I am never hungry. Been on it for a year and I look forward to trying new keto recipes. I make coconut oil fat bombs with natural peanut butter and cacao and add it to my coffee in the morning.

    Ketogenic is actually very easy to stick to. All the advantages is the thing that keeps you to sorting out all carbohydrates.BUT always start with sorting out the carbs from you house; all pasta, rice, cookies, bread, flour, fruit (YES, fruit too), cereals, potatoes and of course the most important: sugar. All kind of sweetener; white, brown, honey, etc etc... if it tastes sweet => Into the junk with it. 4,5 years now.

    I've been in ketosis, for 3 months ongoing (2 daily ketone blood level tests) and have been including modest levels of certain fruits in one meal per day. That is, to a mix of ground nuts and seeds, soaked for a couple of hours in water and coconut milk, add raspberries, max 10, or blueberries max 20, and 1/3 of an apple (not a sweet one). I've tested an hour or so after and always am in ketosis. Watch some of the different keto advisory videos - such as Dr. Berg - for more details,

    One interesting thought about giving Metformin to someone with dementia is that it does seem to lend credence to the idea that Alzheimer's should more properly be called Type 3 Diabetes.

    Yeah but Metformin nullifies some of the effects of exercise, so we're all just better off exercising and using time restricted eating on a low carb diet

    I used to work at an Auto Auction. There was many retired truck drivers, retired City employees and retired mailman . I noticed that the elderly retired mailman had very sharp memory regardless of diet

    Anyone have success using MCTs to help heal people going through withdrawal from psych meds? My friend, who is suffering still from getting off Klonipin has had her symptoms ameliorated by a low carb diet, but she could use extra brain help. I'm wondering if anyone has any thoughts on this.

    Ordinary coconut oil works fine and way cheaper than MCT. Not all people can take MCT which makes my skin and throat itchy, histamine hay fever type reaction.

    The main reason I started dieting was to get rid of cellulite, however the keto diet made it unbearable. That's why I tried the *Agoge diet* and it worked great

    Coconut oil is good but MCT #8 and #10 are part of Coconut oil & more effective at creating Ketones. I take it in a powdered form & use it as a coffee creamer.

    Reading the comments is really helpful for me. I really want to cut some of my food intake, especially sugar. I have diabetes family and I seriously consider cutting my sugar intake. Craving for foods is just a big challenge for me, especially when I work late at night. But I slowly change the coffee into more water and fruit. It is hard, but I should get used to it.

    Fruit unfortunately is also sugar ,Ay Mate. Having worked nights all my working life I know how hard it is to cut back on food at night, but Nadogrl is right get your kitchen organised, if you have low carb foods available(Nuts, Cheese and jerky or biltong) and cut out the high carb stuff out of your eating(Breads, cakes anything with flour etc) you will be getting used to something worth getting used to!

Btw, I just tried to find the ages of both, could only find personal information on Mosely, who has a wiki page. Mosely was born in 1957. I agree Cunnane looks a little older, maybe 5 to 7 years?

    There is a type of rodent known as Octodon Degus in Chile that develops "Alzheimer" naturally. This rodent can live up to 13 years in the laboratory, and develop plaques and phosphorylated tau starting from 18-36 months of age. Oxidative damage, cognitive problems and decline in acetylcholine are evident with very old normal mice (2.5-3 years of age). There are transgenics, and much more, when looking to answer your question.

    @@tfoxen7518 In the late 70s, I fed my rats Purina Rat Chow (carbs) - they would always last about 1000 days, dying about 200 days after they developed a big cancerous lump on their backs. I saved them from my old job at a Skinner "Learning Lab." They had "Kill" signs on their cages after the experiment ended.

    microorganisms make b vitamins, b vitamins are required for carb breakdown, your foods are often depleted of Bs, and other ingredients reduce Bs further and also kill microorganisms ha make the Bs that you need for carb digestion, (1 by not giving the fiber they need by starving them 2 killing them by byproduct like alcohol smoking and 3 by digesting things made to deliberately kill them like antibiotics and traces of pesticides) then making you further dependent on dietary Bs while you digest further carb for which you have no b storage anyway. of course you use ketones better, you dont need Bs for that and alzheimer is called t3diabetes a lifetime of bacteria depletion. others will say Bs and omegas are good for AD but wont figure out or mention that one is the product of microorganisms and the other feeds the microorganisms.

    Ketogenic can even be done as vegetarian. Could probably, with some work, be done vegan. MCT oil really helps, especially the sicker one is to start with. MCT oil doesn’t place a load on liver function, the way whole coconut oil or other fats do. Most with Diabetes, also have fatty livers or otherwise impaired liver function, even when lab tests fail to show it.

    I have been vegetarian for 50 years and i regret the eating all the carbs which caused quite a few issues for me. I have been doing keto for a couple years and have no problems doing keto but i will admit eating meat (fish, chicken and beef) would make it far easier (you cannot eat alot or the protein will convert to glucose).

    You can't "take a walk" and fix a bad diet. You have to stop eating cake with flour and sugar in it. You have to eat a truly keto-friendly dessert, rarely, instead. Diet is far, far more important than exercise. This guy is great, but his explanation of keto is ... well, other people explain it much better, and seem to grasp it much better.

    Definitely human breast milk is ketone and cholesterol rich. Babies are born in ketosis and remain in ketosis as their brains develop. Don’t know about cows’ milk.

    Coconut oil is a hype. It has some C8, C10 but the major components are long chain fatty acids. Bad saturated oil. Go for MCT instead

    Yes, we do need veggies. Those on keto diets need certain nutrients added in, or they develop ills, too. Same for vegans...those need to augment certain nutrient(s), or develop malnutrition, too. Hunters always also gathered plant foods they knew they needed.. Even Inuit gathered berries and other plants from the tundra.

    No argument about that. Yet, How healthy they look, often has something to do with how long they’ve been using a particular diet, their Genetics, as well as Epigenetics, which dictate how well a particular diet works for someone. I kinda favor keto, but on a more cyclic pattern, for myself; that can differ drastically for others. I do NOT have any Inuit or similar, DNA. AND, what passes for Ketogenic now, is .Not. what those who spent thousands of years living above the Arctic Circle followed. And, there are various versions of Keto! It’s not “one size fits all” for any diet. There are people I’ve helped, who follow vegan, and those who follow Keto, and numerous in between. ALL of them have issues following well, Unless they are also well-informed what foods provide what nutrients, and, how to recognize finely-tuned hunger signals, which can differ, depending on what the body needs most...and if they can afford to keep following it.~C

    I mean...any lay-person who studies human physiology at all...shld be able to deduce that these findings are "true anecdotally". Why? Our food is now "sh÷t" worldwide for the Capitalistic gains/money. Less nutritional density, equates to mass food production. .which becomes more and more - unaffordable. So...I think k most people shld just learn about "Intermittent Fasting", to reduce one's glucose intolerance...without living "in hell...on a non-sustainable ketogenic diet".

    Yes, I do like the occasional beer, but just cutting out wheat products, pasta, rice and potatoes gets you almost there. If you eat the same in grams of fat as you eat carbs+protein, therefore deriving 70% of your energy from fat, most people are in ketosis. So, 50g max carbs, around 75g protein and then you can have salads drenched in olive oil, oily fish, avocados, eggs, dairy from grass-fed cows, the same goes for meat with the fat on and the all-important vigin organic coconut oil. Easy-peasy!

    It IS easy to follow, in part because you don't HAVE to say no to the occasional beer or slice of cake. The key is OCCASIONAL. If you think "occasional" means only once a day, that's probably not going to fly. But if you're not trying to lose weight and just wanting the health benefits of Keto, a piece of cake or a beer once a week is not going to be a deal-breaker.
