I have a model 18-3, with the 3 t's. I see some signs that your revolver may need some servicing. The wear on the barrel, by the forcing cone, caused by the cylinder hitting it. Mine was doing the same thing, it was the yoke screw and adjustment. It caused the cylinder to not lock up as tight as it should. That might be why the gun was locking up on those last shots too. Beautiful revolver!
I have a model 18 with a 6 inch model 17 barrel. A 'frankenstein' indeed.

I shoot tighter groups than that with my model 17-3 at 25yds and I'm 75yrs old!!!!
