Any consideration for scapular stabilization performing special tests in supine?? For instance performing neers in supine might hold the scapula in neutral that would have been previously in anterior tilt. This could affect subacromial space and impingement symptoms being looked for
What about O'Brien's test, or is that not part of the standard examination and only when suspicious for labral tear?
I'm sorry but to me, looking at this seems like his left side is a bit higher than his right, so it's definitely unleveled or is it just me?

Not always uncommon to have slight asymmetrical shoulders. Take it into consideration with other symptoms.
