Shockwave therapy for knee pain is a very effective and efficient way to help manage painful knee symptoms. Shockwave therapy delivers high energy sounds waves into the injured tissues which causes an inflammatory response from the body. This inflammation produced by the body via the shockwaves helps the injured area regenerate, and increases the formation of blood vessels (creates new capillaries). This improvement in blood flow to the area allows for more oxygen, nutrients, etc. to be delivered to the injured area.
Shockwave therapy also has an analgesic effect. The acoustic waves disperse pain mediating neurotransmitters in the area. This means that the pain signals in the nerves are not able to send the signal (Pain!) to our spinal cord/brain. With less pain, we can then challenge the injured area more effectively with rehab exercises which will ultimately strengthen and bolster the injured area.
With all shockwave therapy, we utilize loading exercises which result in further strengthening of the injured area which results in further improvement in the injured structures. For knee pain, it is important to load the quadriceps tendon, patellar tendon, and other muscles/tendons that surround the knee.
Knee pain can be due to many different causes. Shockwave therapy for patellar tendinopathy or shockwave therapy for patellar tendinitis is one of the most effective treatment options. Patellar tendon injuries are often long standing, have poor blood supply and the tendon is usually weakened to a degree. Shockwave for patellar tendinitis helps promote further blood flow, and beneficial inflammation to the area while the loading exercises strengthen and support the tendon to make the pain go away permanently.

    VMO, distal Vastus lateralis, distal rectus femoris, proximal gastrocnemius. That is the general approach but getting feedback from the patient while performing helps identify specific areas/tigger points that need to be released.

    Can this help heal a small muscle tear in the front of the shoulder? I've done prp before and it helps but I just don't want to spend a grand on a single treatment again 🙄 would love to hear your input on how shockwave can or if it's possible to heal a grade 1 muscle tear.

    Yes, It will help stimulate blood flow to the affected area. It helps loosen up the tight muscle fibers surrounding the strain, helps break down trigger points, and allows you to train the muscle without any down time.

    Hi and thanks for the questions. The number of treatments depends on how functional a patient is with the knee. If it is severe and activity is limited/motion is limited, it could take several rounds of 4 visits each round before noticeable effects are seen. If a patient can exercise/load the connective tissues around the knee while receiving shockwave, the treatment need will be fewer. The first round is always recommended to be 4 sessions at 1 session every 3 to 7 days, so 1x per week. Then reassess 5 weeks after the last session.

    Hallo, I've had quad tendon pain for months (powerlifting), should I go with the prp or with shock wave therapy or Hyaluronic acid.....which one has more benefits?

    I wish I had an easy answer for you. Of the three, shockwave is the least invasive and research is coming around to support its use; we have seen excellent results with shockwave for quadriceps/patellar tendinopathies in athletic populations. Part of shockwave is that you want to load the tendon while getting the shockwave treatments for maximal effect so you can essentially stay in the gym while getting the treatments. PRP can be very effective, but the patients I have had require down time for a couple weeks (one case up to about 6 weeks) because the injection causes quite a bit of pain. Which is a good thing, means it is working. I don't know much about the Hyaluronic Acid injections. Just a quick search showed decent relief in patients at a 6 month check in but not sure how it stacks up with power lifting. If it were me, I would try shockwave first as its the cheapest and less invasive.

    Hard to say online... We take out patients through a movement exam, see what they can, can't do and if we think shockwave will help them bridge the gap to more functional movements. Shockwave alone won't make a substantial increase in flexibility, but shockwave can allow for more pain free motion to utilize during exercise which will the create permanent restoration of range of motion.

    Not in a physiological way. The metal just acts as a conductor so patients who have a knee replacement feel more vibrations from the shockwaves than other people. Same thing happens when we go directly over the bone, the shockwaves feel more intense and travel down the bone.

    It can help with pain and superficial tightness around the knee/scar but not deep down in the joint if that is what you are asking about.

    We have used it on 2 patients with knee replacements with good results. For one of the patients she had significant pain on the back of her knee after the replacement. The shockwave treatments reduced her pain significantly which allowed for greater improvements in her exercise routine (squat, lunge). One patient we did not do shockwave on was a patient who had a knee replacement and she has so much scar tissue deep in the joint that the knee wouldn't move past about 60 degrees. Shockwave does not help significantly with that type of scar tissue; that patient had to undergo manipulation under anesthesia to improve the range of motion.

    No real evidence to support specifically. But shockwave offers an analgesic effect which can be helpful to get a patient back to exercises/mobility more pain free which will allow for further improvement for those conditions specifically.

    Hi! They are the same thing referring to low energy shockwave. High energy ESWT is reserved for in patient shockwave treatments requiring anesthesia.

    Hi Tammy, we accept almost all major insurances. Depending on the type of insurance, it may not cover all services that we perform for the treatments. Shockwave is one procedure that is not covered by any insurance. If you call our office at 619-818-4306, our office manager Daysha can look into insurance coverage for you.

    Yes, in Mission Valley. It is only painful during treatment. Afterwards most patients report a decrease in symptoms and feel good. A package of 4 is $600, $100 savings; single sessions are $175.

    If the knee pain is cause because there is no cushion in the joints where your bone on bone does this procedure cause those tissue to grow back or numb the pain temporarily

    Will not grow back cartilage with any significance, but can substantially reduce pain which allows for more exercise and thus preservation/conditioning of the tissues around the knee.

    Most patients tolerate it just fine. Not every point is painful. The machine is adjustable so we keep it in the therapeutic range but dial it to the lower end of that range if it is too painful. It feels like a deep ache, every now and then depending on the spot it can increase significantly but we keep moving so it’s more tolerable at the painful site. Usually as the treatment goes the area becomes “desensitized” and it’s much more tolerable to patients.

    It can help manage the pain in conjunction with a proper strengthening plan. Doing shockwave alone may help temporarily with some pain but being able to strengthen (exercises that do not stress the knee joint too much) will see more substantial results and rebuilding of the surrounding tissues.
