I do not think that omen was viper's failed experiment. I think that omen was a scientist along with viper, theres no evidence that viper experimented on him, he probably failed an experiment himself, and died because of it. Viper was probably really close to him

I think he is a scientist because of the line where he says something like Sabine, (viper) Look at the monster's we've become. at least you have flesh to hide it. He wouldnt have said that if he wasnt a scientist as well.

Okay hear me out. Omen and Viper were coworkers and/or a couple working for Kingdom Corp. Hypothetically speaking, the little girl in the Viper // Reyna player card is Reyna’s younger sister. In the player card, she’s shown wearing something similar to a hospital gown, holding Viper’s hand. In one of Omen’s voiced lines he also mentions how Viper used to use her abilities for healing, and who else uses healing abilities? Sage. Viper and Sage could have worked together on this little girl who was suffering from some unidentified illness that was sapping away at her life force, later revealed to be her older sister’s powers, and the two were trying to heal her or find a way to make her better. Viper attempted to experiment on the young girl, wanting to find a way to cure her of her illness, but her experiment failed, and the girl died. Reyna learned about this and flew into a rampage, attempting to kill Viper by draining her life force/soul or smth but Omen, or Fred, blocks her and instead he loses his soul. This makes Viper break down completely. After Reyna is restrained she begs Sage to attempt and revive Omen, but Sage wasn’t yet strong enough then; as shown by how she needs 8 ultimate points to revive in game. The rest follow your theory. Sage was unable to fully revive Omen, leading to him receiving partial memory loss, forgetting his own identity, etc. After being revived into the soulless shadow agent he is, Omen was experimented on by Kingdom Corp, thus explaining how he’s been ripped apart and his mind has been split so many times.

Or maybe sage failed at reviving reyna's hermanita

Yes that is one theory, excellent. But there is no voicelines or interactions that prove reyna and sage knew each other, if to say nor reyna expresses any anger towards Sage.

There is a theory too that Omen is an octopus. I think the experiment was to convert Omen into octopus and after sometime he could not be contained in an octopus body and died and sage failed at reviving him.

He was revived partially, neither human nor ghost. And assumption was made based on the voicelines. As riot has said that the lore is based on the ingame interaction and voicelines

That would actually make sense, but there are no such interactions which can show love between them
