What is risk information management?
Thinking about risk, what do we mean by absolute risk and relative risk
Sounds a bit like the level of risk can be presented in a way that gives a particular impression of a greater or a lessor risk or benefit?
Are there occasions when a vested interest might want to give an impression of an increased benefit, or reduced risk from an intervention?
How could choosing RRR versus ARR influence the impression given for adverse reactions is medical journals?
Conversely
Are there occasions when a vested interest might want to give an impression of a decreased risk from an intervention?
So will RRR always be higher than ARR?
Why can the RRR vary between populations and vary over time?
So using one of the two measures to estimate the risk/benefit ratio would lead to different conclusions?
How could risk / benefit reporting be optimised in the medical literature?
Do you think the editors of prestigious international journals are aware of the differences between RRR and ARR?
In the case of this paper from the Journal of Expert Reviews of Vaccines
Efficacy and effectiveness of covid-19 vaccine - absolute vs. relative risk reduction
www.ncbi.nlm.nih.gov/pmc/arti...
The original phase three data gave an efficacy for Pfizer vaccine, at preventing infection with SARS-CoV-2 of 95%, but it turns out the ARR was 0.84%
Why are these figures so different?
So is it fair to say that the ‘real world translation’ of the ARR of the Pfizer vaccine over 5 to 6 months of use increased to 3.7%?
What do we mean by number needed to treat?

After 30 years in a hospital pharmacy it’s always telling how drug reps squirm when you note this about their studies. This is how they roll.
I read Pfizers paper on the trials when it came out in the New England Journal of Medicine almost 2 years ago. This was all obvious then just upon reading it. The problem is most people (unfortunately including doctors) don't actually read the papers and are therefore easily manipulated. Great and important discussion!

it is also amazing to see that during the pandemic the term "expert" has lost credibility and authority; instead it arouses suspicion and mistrust
A brilliant discussion. I remember in the early stages of vaccination, I was questioning every week with my sister who was very pro-vax and worked in the NHS. I studied the data supplied daily on deaths in NHS hospitals (therefore people needing treatment), yet even this was "with" covid, not "from" But my main question was, almost every day, 52% of the "deaths with covid" were in the over 80 age group. This was BEFORE any vaccination, during the early stages, and in the mid stages. When over 80% of over 80s had been vaccinated but only a very small number under 80, I asked: WHY was the percentage of those dying "with covid" STILL at 52%. Surely if the vaccine "worked" then the margin should have dropped? Surely very few over-80s should have been dying, resulting in the % below 80 rocketing? She couldn't understand the maths, blindly believed "it works", and just kept saying "give it time". Yet anyone who could read data could tell they weren't performing like we had been led to believe.

Worth also looking at Dr Seheult MD on Absolute and Relative Risk reduction. Puts it all into perspective. Crucially, need to know the prevalence in the population to get the Absolute risk. If there is not much of ‘it’ whatever the contagion may be, the risk will be low. And conversely risk increases if there is more of ‘it’ about. So much common sense. Risk reduction of the intervention is against that population background. And in so much of medical reporting, it,s the relative risk reduction that gets broadcast. ( It,s the normal way of writing up medical research papers. Not lying. Critical analysis is key. ) And be aware, not all medics are good at understanding the difference and not all science journalists ‘get it’.

There's literally a book called, "how to lie with statistics." You can definitely mislead or even outright lie with statistics, especially by omission. It's even in how you word it and the way you explain the data. Pharmaceutical companies for example will say, "x new drug reduced symptoms by 30% while placebo only reduced symptoms by .29" 30% sounds like a lot more than .29 but.. .29 is 29%. So a 1% difference. They have all kinds of tricks they use to confuse people and outright push misinformation. You quite literally can't believe words written on the page anymore, even if they're peer reviewed studies. Hell there was the big statin scandal. They gave all possible participants statins for 6 months before the study. Anyone who had side effects was pulled from the study. Then after 6 months on the statins they conducted the study and deemed minimal to no side effects. Well no shit, everyone who had side effects were removed before the study even started.

The easiest way to understand this is absolute risk applies to the individual and the relative risk to the group. The absolute risk reduction for the P vaccine ( from the trial) was 0.84%., the M vaccine 1.2% Which is why ( as a practicing doctor) I was set against these interventions from day 1. I’ve followed all of Norman work and my patients benefitted. For that reason I’m eternally grateful to Norman.
Thank you for this "private" lesson on epidemiology and statistics! Highly appreciated.
Just read a very quick and short news article relating to the mRNA cancer vaccine completing its trial. It stated there’s a 44% relative risk reduction in the death of those who took the drug. I noticed there was no ‘absolute’ risk reduction in the article anywhere. (I believe the trial was also based on just 157 people). I’ll be noticing this language use from now on. I spent years in the role of Quality & Risk manager in a design & manufacturing business. And I understand very well how statistics can be 'massaged' to suit the situation one desires. Plus years ago I had done a course on demystifying statistics at Royal Holloway. That had helped my work, with a discerning eye on how various people: marketing & sales, product engineering, manufacturing, etc presenting data. When I audit such data, often I see how people used statistics to advance personal agenda. It is disappointing to learn how the medical & health profession would have allowed some of the 'tricks' to propagate. I guess setting good standards in reporting is urgently required. Such standards shall prevent people comparing apples with oranges just to mislead.

The key to this analysis is to make absolutely sure the base numbers being used haven’t been manipulated before running the analysis. For example (hypothetically) when a paper lists half the participants as being un-injected because it’s only been 13 days since they’ve been injected then it’s “Garbage in, Garbage out” regarding a potential harm evaluation between this and another relative group.

It can be even worse than what Norman is demonstrating here, because the seedeesee in the US, when it's director said about 15 months ago that this is a pandemic of the unv'd, was including people who were hospitalized or died from C infections that occurred a couple of months before "the intervention", and in a large infection wave, to boot, so most of the deaths that occurred in their 6-month survey window were from pre-rollout infections in December 2020 and January 2021.
That was mind blowing: 1. I understood it 2. There is clearly a new category for the old saying, "Lies, damn lies and statistics". That would be "Lies, damn lies, statistics and risk reduction publicity".

Everyone - from the expert to the uninitiated - must beware all the available tools to analysts, pundits, etc to spin the numbers in their favor. Your best protection is thinking critically about the author and where their interests lie relative to the truth / public good. If you’re reading this - I doubt you need the reminder.

Great work Norman! Thanks for another brilliant and informative video of the FACTS!
I learned many years ago that pharma LOVE to use RRR to push drugs. My GP was constantly trying to get me to take statins. I went away, did my research, and learned the ARR was pathetic, but the RRR was fantastic! Alongside that were the horrible outcomes for women my age taking statins. It was shown to cause type II diabetes--from which I had already been trying to heal myself! I refused statins every time I saw my GP, went on a keto diet and put my type II diabetes into remission, and refused these jabs when they came around. I've been extremely healthy for 5 years (zero meds of any kind), and I've never had Covid.
Thanks for explaining and going over the data and statistics. This is so important. I think many people don't understand how much statistics can be misrepresented.
This was a brilliant presentation and discussion. Thank you so much for communicating honestly about the limitations of statistical inference and how easily some apparently small changes (e.g., in large-scale population studies with a high risk of bias or sources of error) could lead to extremely big differences in how one views e.g. mortality rates or other measures, like discussed in slides 22-23 (44:25 ; 45:22). And the theoretical example of how a dangerous placebo being classified and effective due to a statistical trick (38:20) really blew my mind, realising now with a technical understanding of this concept how deeply propagandous the Big Pharma companies most likely have been advocating for their products.

I had a lot of questions regarding the vaccines and a top allergist here in Fort Worth kept saying, " Just get the vaccine. I've been involved in many clinical trials. The risks outweigh the benefits. And if you needed to see a doctor, not many will see you unless you are vaccinated." Since I had delved into what the ingredients were - lipid nanoparticles with PEG, etc, - I was not convinced. I still haven't had a vaccine...preferred to hibernate.....haven't had covid yet, but I am sure in time I will. I am 72, a retired political science professor.
