(Ride Along) LACo.FD Squad 171

Los Angeles County Fire Dept. Squad 171 following up with McCormick Ambulance to Kaiser West LA from a possible stroke patient in 172's area.
Units due: Engine 38, Engine 171 (canceled), Squad 161 (canceled), Squad 171.
Thanks to the crew of Station 171-A for the hospitality and allowing me to ride out.

    That's partly because one of the paramedics is in the ambulance and will need a ride back from the hospital, and they will need to restock their medical supplies. It is also in case additional assistance is needed enrout.

    FB1 Response Videos based upon the actions of those two vehicles @6:17 can they actually get a citation or fine? They clearly saw you and felt it was a more of a inconvenience to move out of the way. But thats just my take.

    Have you seen Erie, PA drivers with first responders going to calls? Worse than CA. Not kidding either. Watched one day as Emergycare drove down the middle of the busy street with sirens blaring. People just don't care. Unreal.

    I know it must be annoying to see people like those at the traffic lights not being able to get out of the way even though with a little more thought they could have, I still really love and feel good when I see people trying, I don't know its just nice to see people move and panic and do all they can to make room for these guys to get through. I guess I'm used to seeing to many vids from other parts of the world where people just don't seem to give a shit.

    LA Engine 51 is different than the one from Emergency!, because LAFD 51 is located at LAX, and LACo.FD is located in Universal Studios, and doesn't communicate via the primary radio often.

    Pretty neat. Didn't know that rescue squads like in Emergency still existed. Haven't seen one here in years. Assumed that the ambulance crew did all the work, now. I also didn't know the rescue squad really does follow the ambulance to the hospital. Figured that Johnny and Roy only did for the sake of getting screen time.

    No that's not why. That rig is BLS and the truck of the POV was ALS, ALS was in the back of the rig one of the EMTS from the rig was driving the medic truck both always have to go to the hospital together. happens all the time

    Actually, alot of departments have rescue squads. Its just they usually operate medium or heavy rescue vehicles, not light rescues like this one. The difference is size and equipment carrying capacity mainly, that and most light rigs are built on pickup chassis' while the medium and heavy rigs are built on fire engine chassis'. In the case of LAFD, their light rescues double as EMS units for calls not requiring an ambulance, or in support of one as in this case. Most rescue units run with an EMT and EMS gear on hand as most respond to not just fires but also car accidents and such.

    They're not suppose to pass people on the right, especially as the second vehicle. There's the risk of the people thinking it's just the ambulance passing and sideswipe the squad as he's going along aswell.

    If you are sitting at a red light in traffic and you get dispatched to a call are you allowed to respond code 3 before the light turns green or do you have to wait until it turns green? The reason I ask is out here in Las Vegas we have to wait for the light to change to prevent people from making bad decisions and causing a accident.

    if there’s cars in front of the unit before the intersection, they’ll usually wait it out. if they’re at the front of the intersection, they’ll usually take it; of course using proper code 3 skills at that point.

    Actually you not quite correct, until the paramedic turns care over to someone who's training is equal to or higher than the paramedic; the paramedic cannot leave the patient as that would constitute abandonment.

    Where I live, our ambulances are equipped with train horns, not air horns but train horns. people don't want to move out of the way with our air horns, lights, and rumbler sirens. that's sad

    always code. the point of having squads instead of ambulances is to be able to drop off the patient quickly and come available for the next run, especially for busy squads like this one.

    Kevin Douglas so als runs they will transport the patient to the hospital with the squad paramedic in the ambulance and then once they get to the hospital the squad paramedic will get back on the squad go back available now if it's a BLS run then the ambulance will just transport and the squad will go back available from scene. Hopefully that answers your question.

    Chris Maize partially but what about turnover to the RN in the ER? I understand that California can have extremely long waits to transfer a patient to the ER( problem we don't necessarily have here in Michigan). So say if the Squad and Ambulance bring in a Chest Pain who stays with the PT until the RN takes report?

    ALS stands for advanced life support; more critical calls like heart attacks, strokes, cardiac arrests, etc. BLS stands for basic life support; minor calls. broken bone, fall, non-life threatening injuries.

    A 10 minute response time to get to the hospital? That's INSANE!! The poor patient could have died if she or he were having an M.I. Traffic in Los Angeles kills in many ways other than just vehicle accidents. Delays in getting to emergency care are a killer in themselves.

    I've been a paramedic for 25 years (in EMS for 37 years) and I still don't understand why a non-transport squad would run "hot" to follow an ambulance to the hospital. It's a dangerous practice and a crazy liability.

    usually the medics will compromise but certain hospitals do certain things. for example, Kaiser West LA is a stroke center so we transported the patient there, Little Company of Mary in Torrance is also a stroke center. Harbor General and UCLA Ronald Regan are two popular trauma centers.

    FB1 Response Videos Jay , except in a rural area where I work as a medic because our district has only 2 hospitals that we can transport to and we can't pick and choose like LA . The hospital here can do most things , it's a regional medical facility that has a Level 2 NICU , Level 4 trauma center , regional Cancer center , and cardiac catheterization lab . Any patient beyond the hospital's capabilities are quickly transferred by helicopter air ambulance to specialized medical centers in Boise , Pocatello , or Salt Lake City Utah .     not every hospital does the same thing. yes, centinela is the closest but we had a stroke patient, Kaiser West LA is a stroke center, so is Little Company of Mary. being north of Manchester, Kaiser West LA was probably the better hospital to go to.

    well only kaiser patients get dropped at kaiser. since this was a stroke call, if she wasn't a kaiser patient, we more than likely would have went to harbor or lcm.

    Perhaps Kaiser is not as good in LA but here in San Diego they are great. I am a retired RN and have seen their level of care first hand as a patient and have had no complaints. I once presented via POV to the ER with very tachy afib (130 BPM) and chest pressure. I was on a gurney in an ER room within 2 minutes with 2 doctors and 3 nurses around me. Lab draws, EKG, and chest x-ray done within 7 minutes of arrival at the hospital. I'll match those numbers against any other hospital around.

    @@vgk7028 The squad follows the ambulance so if the ambulance has to stop due to a problem with the patient the medic driving the squad will know it and be available right away. When I was working the paramedic unit always followed us to the hospital.

    @@vgk7028 There is only one medic on the squad...the man driving. The other is the person doing the ride along and making the video. The other medic is riding with the patient.

    This video was shot from an official LA County Fire Rescue Squad vehicle. It is a paramedic unit and has the same priority and emergency vehicle status as the ambulance does.

    it was good for EMS to go around. they have the patient. time is of the essence with the ambulance. however, on the squad, there's a little less on their shoulders, as long as we get to the hospital by the time the 2nd paramedic is ready to go. and on another note, we taught those two a lesson that you need to pull to the right for emergency vehicles, not just come to a stop or pull to the left.
