RESIDENCY IS HARD - Watch This First!

Ойын-сауық

Reflecting on my time during residency and I would have to say that it was one of THE HARDEST and challenging experiences of my life. I spent my 20's going to into debt, working 80 hours a week, weekends, and holidays. Missing birthdays, weddings, baby showers, and funerals.
In spite of all that - It was a very humbling experience. It has made the person I am today.
Keep in Touch!

@@drjennale If you thought another resident made a mistake would you speak up? This is when you're allowed to make mistakes? Is that because you aren't held accountable and the doctors that oversee the residents will cover up your mistakes? I'll never trust a doctor again because of what a resident was allowed to do to me and get away with. She said she was an advocate for women. I think of commiting suicide everyday because of what she did to me.

I appreciate hearing about Dr. Le’s journey through residency. Her video clearly shows that residency is a busy time which can take you away from other aspects of your life like spending time with your friends and family. I cannot imagine how difficult it must have been raising two young children during residency! For me, though, I think the greatest concerns I have during residency are my relationship with my attendings and being a competent doctor. I have had the opportunity to discuss these concerns during my ethics class. Power dynamics between residents and attendings can lead to ethical concerns. Residents may be hesitant to bring up concerns, especially when they contradict an attending’s actions or opinions, in fear that that it could be perceived as an insult towards the attending. This can result in maleficence, or harm, to the patient when the most optimal care is not provided. We hear about Dr. Le’s own experience when she asked her attending for feedback regarding the delivery of difficult news to a family about their child’s diagnosis of osteosarcoma. I think it is clear that Dr. Le wanted to do her best to support the family, but the attending gave her feedback that seemed to convey that there was nothing she could do but “live with the fact that you’ll be part of the worst day of their life.” On the contrary, the attending could have suggested giving the family resources and ensuring that the family feels that the medical team will be there for the family every step of the way. Upon starting residency, the heavy responsibility of caring for patients can also feel overwhelming. Dr. Le, herself, mentioned that she had doubts and fears when she started residency as her “poor decisions could lead to poor patient outcomes.” I love that at the end of the video, Dr. Le reassures us that residency is “the time to make mistakes.” I am still worried though that certain institutions or attendings may create an environment that is less understanding or accepting of mistakes. This may result in the lack of disclosure when a mistake is made, which is unethical. For a patient to fully exercise their autonomy, they must be fully informed. Also, in an environment where residents are not able to learn from their mistakes, there is an increased risk of harm to future patients. Wu et al. published a study in JAMA where they found that residents “who accepted responsibility for the mistake and discussed it were more likely to report constructive changes in practice.” In other words, learning from mistakes during residency can prevent mistakes in the future. Despite these concerns, I remain optimistic of the positive opportunities ahead!

Here I am, deep into dedicated with COMLEX level 1 in nine days. It is SO good to watch your videos and get a glimpse of the light at the end of the long (LONG) tunnel. I'm a mom of two kids myself, and I have really appreciated your videos chronicling your journey through med school, residency, and now life as an attending. They have reinforced that YES, you can be a mom in medical school. YES, you can achieve a life balance that works for you. YES, you can be a good partner to your spouse and a good mom to your kids. And YES, it is okay to struggle sometimes. Speaking of the struggle, I should probably stop procrastinating and get back to board prep... I appreciate you. Keep doing what you do.

Hi Dr. Le, First of all, thank you for the video. It's very inspiring! I’m a premed. I’m just wondering if I choose to go into Family Medicine, is residency for Family Medicine easier comparing to other specialties? I heard the schedule is easier, comparing to other specialties, particularly surgical specialties. I am not sure if that’s true, so I just want to ask you. How was your schedule like? How many hours do you typically work as a resident of Family Medicine? Also, do you have to be on call? Thank you!

I am really struggling with mom guilt. My plan is to apply to medical school next spring (2023), but I am strongly considering the PA profession because residency is not required. The 60 to 80 hour weeks scares me a bit. I fear that I will not be a significant part of my son’s life during residency. My son is 5 and will start school and sports/school activities this year and I know I will miss many events, do you think you would have missed out on any or all of these events if Wyatt was of school age during your residency? My worst fear would be that my son grows up thinking I wasn’t a part of his life during residency. Thank you for your KZread channel and for being an inspiration to all parents.

I’ve talked to a lot of people through the years. Many who have had both physician parents - they have all said that they always felt loved and remember the amazing trips they took together. It’s the quality of time rather than the quantity of time that matters. That being said. Wyatt is currently 5 and London is turning 3 and I am personally choosing to work part time (2.5 days a week) even with 350k in school loan dept 🫠 because I feel like I’ve missed so much already. They will only be young for so long and need me for so long. I personally don’t want to miss it. They said that your kids will have spent 80% of the time they will ever spend with you by their teenage years. After that, they rather be with friends, focus on their career, have their own life/family etc. If I had to do it all again right now with the kids at this age. To be honest, I’m not sure that I would. The time and financial burden so very high. Hope that helps. This answer is very personalized to me. You need to see what is best for you and your family.

I'll never trust doctors ever again because of a bad resident doctor. Imagine a patient that wants to commit suicide because of the mistake the resident made. This resident says she's an advocate for women and hides behind the male doctors that oversee her. I hadn't even finished my last PET scan after having throat cancer. She has caused me so much pain mentally and physically. This is your time to make mistakes because you'll be protected and never made to admit to those mistakes. No matter how much you've hurt a patient. Shame on the medical community that is allowed to police itself.
