Josh had slurred speech etc, because he "self-medicated". And whether you're mentally unstable for psychological or drug-induced reasons, it comes across fairly similarly (and often goes hand-in-hand)

"Self-medicating" means getting drunk and/or drugged - which may include getting high. Josh went off his meds, but he was still using/abusing substances. And yeah, obviously Rami Malek is just a talented actor with an interesting "way" about him.
