3:40 Ideal Patients for Active Surveillance
Microcarcinomas, low risk, older than 20 years of age and has no indication of immediate surgery.
Invasion of the thyroid capsule and metastasis of the lymph nodes should be carefully evaluated prior to ablation by a thyroid ultrasound expert.
5:28 Indications for Ablative Therapy
Not yet clearly established, patients with surgical contraindication, no desire for active surveillance due to anxiety or a desire to have treatment.
Ideal patient with an ideal tumor has at least 2 mm of healthy tissue around the tumor not in close contact with the trachea and well-defined micropapillary thyroid carcinoma.
History of the literature of systematic reviews outlining the success of RFA.
8:40 Can RFA replace surgery?
A myriad of studies highlight the effectiveness of RFA on large cohorts of individuals with minimal complications in comparison to surgery.
It is considered almost as effective as surgery and potential better quality of life compared to surgery.
Explained further clinical cases studies conducted by Dr. Dueñas showing evidence of successful treatment 9 months post treatment.

Great potential in the near future, with low rates of complications, acting as an alternative for active surveillance and no difference in recurrence rate in comparison to surgery. Patients need to understand risk and benefits and future work needs to include larger cohorts with greater follow up time to make stronger recommendations. RFA requires an ideal patient, with an ideal tumor and ideal multi-disciplinary team.
