It is already unlock from the start. Just need to get lucky. You can check where it can be found. I think it's only a secret room item (also can be found in greed mode)

​@@renuteronpi484Yeah, it's in the secret room item pool, so if you have a restock machine or some way to reroll item pedestals in there, you should be good to go
