    I had prostatitis for over 2 1/2 years but a standard urine test did not indicate a bacterial infection. My urologist blamed all of my symptoms on my radiation treatments. Once hospitalized for 3 days and receiving antibiotics intravenously my symptoms disappeared. It is wonderful to be normal again.

    Every single prostatitis is caused by infection. These stupid urologists only ask for simple urine and semen tests where nothing is found. You need to go to hospital to get antibiotics via intravenous. I believe the best one is Zosyn. These cultures invariably will not show anything. Chronic non bacterial prostatitis and pelvic pain syndrome are bullshit names. They need to put you on intravenous medication

    Did you have a surgery. I'm having a pain after peeing and i have frequency (if i sip a water,i ll have to go to toilet very urgently within 10 to 20min) and I get a very bad pain in my clit after peeing and my bladder pains too. I get shivering sometimes if i get a bad pain in my clit and sometimes i get a itching in my vulva as well Did you have these symptoms? I gave this more thn 1 1/2years for now I have got enough UTI reports and drugs for urinary infection but still not cured. Could you please help me out here

    Prostatitis is often a neural muscular disorder. The pelvic floor muscles are very sensitive to stress. If one is tense, anxious, or stressed chronically, the pelvic floor muscles can become reversibly dysfunctional causing pain, sexual dysfunction, and urinary dysfunction. Infection is rarely the cause and when it is found to be the cause antibiotics clear it up quite quickly. I suffered greatly from prostatitis for two years. Took all kinds of antibiotics none of which fixed the issue. It wasnt until i addressed the stressors in my life and learned to quiet my over active mind that I was able to cure myself. A Headache in the Pelvis is a great book, as well as Dr. John Sarnos books. Prostatitis is poorly understood by the medical community who generally throw a pill or a knife at the problem.

    I just ordered that book. Hopefully it is of some help. I have had a non-bacterial prostatitis since September last year. I have been prescribed NSAIDs, antibiotics, tamsulosine hydrochloride and I take another muscle relaxant for the night. I have been sitting on a heating pad for hours a day for months now, which is about the only thing that seems to help a little. PSA was elevated at first (6.82), but then it dropped to a normal level.

    @@xjyrki Once infection or other differential diagnosis have been ruled out via testing the only thing left is an overactive mind and a fight fight freeze reaction that is on a hair trigger. As the pain drags on, neural pathways in the brain are created and reinforced.

    @@Moonless6491 I'm not physician so keep that in mind. If you are experiencing pain always get examined by at least one physician. A second opionion wouldn't hurt either, but I would stop after the 2nd opionion if nothing remarkable is found. I went down the rabbit hole of Drs. I saw 8 different Drs (2 GPs, 3 urologists, 3 orthopedic, even went to the ER once). None helped. One wanted to do surgery which I'm glad I declined. If you've been examined by a doctor and the doctor didn't find anything amiss; then you need to move past the idea that something physical is causing your pain/dysfunction. Caffeine is a stimulant. If your autonomic nervous system is already on a hair trigger, caffeine will certainly exacerbate that state. I temporarily cut caffeine out of my diet until I got my mind back into a calm state. If you like coffee, drink decaf. Your mind may view spicy foods as a "threat" and kick off an autonomic response (fight/flight/freeze). Once your brain is trained to view something as harmful it will respond accordingly. The good news is that your brain can be retrained, but it takes time. The mind is very powerful. Control your mind and your control your pain. Pain is a contruct of the mind. Your pain is trying to tell you something about your life that needs to be addressed so listen carefully. 😀

    Thanks for the information. My advice for everyone out there don't have the prostate massage especially with blood work showing an infection in the body. Two years now dealing with hydroceles bilaterally and most likely septis after it. Also make sure your urologist actually listens to what you have to say.

    U need to be Ur own ADVOCATE, list Ur questions B4 Ur next appt and if Ur doctor doesn't address Ur concerns time 2 find a different doctor. I find that when U give them an organized list of concerns they're more apt to spend time needed with U. Remember THEIR time is as Valuable as YOURS, organizing Ur concerns lets them know that and prevents U from rambling or forgetting what U really are concerned with. Just what works for me So Far. Good Luck

    Unfortunately, I have been to two urologist and been prepared with my information and questions. Both doctors were more about getting me in and out so they could collect their fee. The second doctor was actually rude. I agree that Dr. Malik is very informative but she cannot treat over KZread. :-(

    I've been suffering this for a long time. I never took antibiotics. I was diagnosed with H Pylori last year. I took a heavy dose of two antibiotics for 2 weeks. Towards the end I was having blood and brownish fluid when I would ejaculate. Could this be the antibiotics clearing out the infection in the prostate?

    For all who suffers I solved this issue by: 1. Zero alcohol, cut down on sugar/white flour 2. Try to reduce your ejaculations, it’s very challenging for a man but it gets worse by over using the system 3. Warm baths and EXTREMELY important get massages often. I talked to my masseur about the issues and he helps with massage of lower back, glutes, thighs. You can also by yourself massage your own private parts a little. The problem is very tense pelvic muscles (from stress, sitting all the time etc) and inflammation.

    Thank you for the insight. Having non bacterial prostatitis for 30+ years is extremely difficult. Oddly in learning to deal with it, I'm more in tune with my body and its now a benefit with sex..and my gf loves it. (Last for how long she needs, multiple orgasms, dry orgasms, ect) Relaxing the pelvic muscles and controlling contractions helped me out tremendously.

    ​​​@@stevewilliams7659You may try, sure, but you may also consider the user dreamer6737 îs crystal clear to me îs not a medical doctor, there's growing evidence that increasing the frequency of ejaculations actually helps in reducing the incidency of prostate cancer (excluding the most serious and dangerous ones which are believed having a crucial hereditary co-factor) for the stagnation of the fluids may degrade them and make them harmful, and have great care with the warm bath, the warmest it can get is 3⁰ celsius below body temperature which(=the body temperature )îs 37.5-38⁰ celsius, warmer temperatures may easily compromise the testicles functions (production of sperm can only occur at some 35⁰ or Below that). There's people who've lost one or both testicles for wearing tight pants in hot summer days out there, and many more suffering from similar accidents.

    I had chronic non-bacterial prostatitis maybe 20 years ago for more than a year. Couldn't sit down at all and had to sleep on my stomach. I'm a lifetime vegetarian and had eaten tofu nearly everyday for more than 30 years. In the midst of my suffering I traveled abroad fo 6 weeks. During these travels I ate no tofu or soy of any kind. And by the end of my 6 weeks my prostate pain was completely gone. When I returned home and returned to my regular diet my pain returned within days. It took some experimentation to narrow down what in my home environment was causing the problem but through trail and error I finally narrowed it down to soy. Now I've totally avoided soy for twenty years and the problem hasn't returned.... unless I cheat, (eg. tofu stir fries at Chinese restaurant when lunching with colleages) which immediately re-inflames my prostate. I'm surprised MDs (I saw many) seem to be totally unaware of the possible link between prostatitis and diet....

    I experienced back pain, pain in my testicles, fever and vomiting. A uouroligist told me: no smoking, no caffeine or alcohol. Also to sit in the bath in warm water twice a day. This advice has really helped.

    I think this was the best most complete video on the topic I have seen (and I have seen a lot). Most doctors tell you "take this antibiotic for 28 days" "oh, it was not fixed, take it for a few MONTHS and some antiinflamatories" "oh, it was not solved (or came back), take more antibiotics and antiinflamatories" ; "but doctor, what is the underlying cause?" ; "oh, who knows, no idea" . They never tell you about posible connection with chronic bowel inflamation, anxiety, constant muscle tension in the zone, testosterone levels, dht levels, etc.

    @@el-cheneke no idea, I have not fixed it. Symptoms got better with time but it is still pretty annoying. Just now I decided to try to stop eating bread done with a specific flour I`ve been eating the last 4 years or so because I suspect it makes my symptoms worst. My depression, anxiety, insomnia and angst make it worst in general. Ejaculating more than once a weak seems to make it worst. No idea what to do with it

    Dr Malik, you're fantastic. I think actually many cases are non- infectious, non-bacterial, which is from temporary pelvic dysfunction, due to nerve/s. Alpha blockers for a while, and therapy, also for men with no enlarged prostate. Even if you have a small prostate, things happen.

    Was diagnosed with prostatitis some years ago. Frequent need, discomfort and difficulty emptying bladder. And a 103 degree temp. Off to urgent care. Got ibuprophen for the temp and had near immediate relief of all other symptoms. Urinalysis (yea! I can pee again) did not detect infection, but still did a course of antibiotics.

    Alastair, I agree. My GP gave me absolutely no info about it . Just prescribed antibiotics, which after 8 weeks is not working. No advice on food, drinks, exercise, etc. Luckily i found some videos such as this one.

    @Rena Malik, M.D. Non bacterial prostatitis was a miserably perplexing experience. Incredulously, I was unable to get a clear diagnosis nor efficacious treatment from several urologists at renown teaching hospitals. The pelvic floor dysfunction & related PT may help some patients but is not the panacea for all. Your adept thoughts on a NBP segment would be appreciated.

    I had a bad case last November. It was pretty brutal for a while! It felt like I was trying to pee a golf ball through a garden house!! You talk about “PAIN”!! But my PA Sarah came to the rescue! Since then I have been hitting the saw palmetto and things are working great!! I am 71 and do not take a single pharmaceutical! I am a blessed man!

    I was having problems urinating, went and had a PSA test taken, it showed a high number , and I found out I was almost a stage 4 prostate cancer. I had radiation for 5 days a week for 9 weeks. Fortunately, I'm in remission for 5 yrs now .

    GOOD NEWS for you. I got treated. I was sufferring the prostatitis pain for 2 years. The all test results were fine while I had too much pain. Finally I tried 200 milligrams of doxycycline antibiotic for twi weeks same time me and my partner. Both us took same medicine and finally cured.

    Had once, years ago. Col. Potter told me to lay of the night shift coffee. She said something about be cute. It was not cute, it hurt to pee. The doctor was retired military, just part timing to not get bored. Reminded me of Col Potter. He squeezed my prostate like a pimple and wrote some heavy-duty antibiotics.

    I've had problems with my prostate for forty years. Paralyzed at 23 in 79 left an incomplete quad its been time aft time of infections. Dilation helped for awhile but it still has me in hospital many times. Blocked kidney was fun in 2009 and again a few yrs back which had a stent that caused infection. I've been through hell . Foley now helps being bed bound and not able to move from constant spine pain. Spasms that rock me without NY precaution. Any suggestions???

    What a thorough, articulate presentation! I know also that prostatitis is sometimes mistaken for PCa and vice versa. I know both conditions can raise the PSA, but I believe prostatitis presents usually with higher fPSA % than PCa. I believe also that a tPSA between 2.5 and 4 but with a fPSA of 16% or less is extremely suspicious as indicative of PCa rather than prostatitis. Would you care to comment on any of this? Thank you.

    I was diagnosed with prostatitus in 2015. 5 months. 4 Drs every imaginable test. PSA 0.7. Then i found Howard Schubiner s : Unlearn Your Pain. Book. Gone in 5-7 days. Zero burning horrible pain. Gone! Prostatitus is almost always emotional. 90% Read a Sarno book read a book on mind body.

    I had persistent pain that brought me to ER a couple of times. Urine and blood work normal. Eventually a urologist said prostatitis. 2 months on doxycycline and pelvic stretches helped, but after having a hernia repair my symptoms went away. I guess my hernia was causing muscular issues. It seems the causes for prostatitis ranges widely.

    @@UserNamez It was inguinal, same as yours. Of course each body is different, but I am back to normal now. The urologist and the surgeon never put 2 and 2 together but my primary did. I think primary was correct in my case

    Great video Doctor!!! Wish you were my doctor when I lived in the U.S. I'm not going to slam the clowns I had there my surgeon put me through hell. When I moved to the Philippines kept having the same issues in the video and through a lot of prayers and a great doctor he prescribed the alpha blockers described in the video and I been uti free for over two years now!!! I feel this a good choice of surgeries for me because my prostate is about 5 times the normal size. I have some urinary problems but not so severe and so have decided for the surgery. My fear is that my ED will continue or even get worse. Can you help?

    Very informative video. I’ve been suffering from testicular and lower back pain for about a month now. I’ve had a ct scan and an ultrasound. The ct scan did not show any abnormalities but the ultrasound showed a small cyst on the right testicle and a small hydrocele on both. Thank you for putting up this video. I’m seeing a urologist on June 1st and now I know what to expect. I like the way you make your videos. They’re like a virtual office visit with you.

    I was suffering with excruciating pain in right testicle. Went through all kinds of tests and exams and about $3k later in bils I mentioned to chiropractor. Turned out to be a twist in the vasdefferens between testicle and prostate. Gentle massage and loose fit clothing to 'let it hang' and straighten itself out. Later found a co worker who's doc suggested same plus jumping/ bouncing allowing free fall to help correct twist. Doesn't cost anything and worth a try!

    I was given Cipro for "prostatistis" when the only symptom was trouble urinating. I had a history of benign enlargement but not prostatitis. When I ended up in the hospital because of side effects from Cipro I was treated as having benign enlargement and the prostate issue cleared immediately. I'm now disabled because of the side effects from Cipro. My GP handled the issue rather than sending me to a specialist.

    CIPRO is a black box drug and is considered a drug of last resort by the FDA. I was given CIPRO before and after a biopsy as I have an enlarged prostate. 2 days after the bio I was in ER with a severe reaction to CIPRO, I thought I was going to die. If you look at the side effects of this drug and accounts of what people have suffered as a result if it's use its pretty terrifying. I was not given any warning or advice about CIPRO. So my advice to anyone who is prescribed this drug to do the research and discuss with the physician before taking it. As the URO says it hard to get antibiotics into the prostate and CIPRO is good at it and why it's used as a first choice. You have to know what it is you are taking and it's side effects and I suggest you do. Talk it over with your PCP and then decide.

    Thank you Dr. Malik. In my mid twenties I got Prostatitis once and had to be treated with antibiotics now at 59 y/o. My Urologist said my issue today has nothing to do in my twenties. I don't believe him because in my young days I could hold back 3 & 4 ejaculates daily maybe this over time caused my BPH? BPH with elevated PSA (4.7) 2023,(5.5) 2022 & (5.3) 2021 had prostate biopsy no cancer. I tried flow max and didn't like the side of effect not taking any meds.
