This video will provide a demonstration of perineal care using Prometric's guidelines. The perineal skill is always performed on a mannequin.

    Dr. Lee! Let me tell you something! I watched this skill most of all, I studied this skill most of all. I don’t know why but something told me to. This was one of the 3 skills I had to perform on test day and I truly believe I wouldn’t have passed without the grace of God and constantly watching you during my waiting period. Thank you so much!     @@txcnaskills2599 I passssssed yesssss thanks to your videos I was caught up on everything I needed to know. Man I was nervous up until testing time I smashed that test. I told myself I was taking that test one time that’s it lol and I suggested your videos to the other testing candidates and they loved your videos too.     Hello! I absolutely love your videos and they are extremely helpful. Is it okay to put the supplies away then remove gloves/wash hands and then go back and give the resident their call light and open the curtain without gloves?

    Yes you can! Be sure to wash your hand again on the end. Most skills require handwashing three times. Once in the beginning, after the opening Second, after putting up supplies and removing gloves Third, at the end just before you say I have completed my skills

Another question is what about if I decide to remove the linen protector and at the same time roll in another just being careful not let them get in touch each other?     Hello Tere, Welcome to my channel. (1) If your patient is asking you to adjust the pillow, I would remove my gloves and wash my hands if my gloved hands are dirty. If my hands are not dirty, I would go ahead and adjust the client's pillow. Now, remember you must wash your hands after ALL patient care. (2) When removing a dirty linen protector, your suggestion is good. The behavior keeps the patient from lifting up and down twice. I hope this answered your questions. If not let know and I will make a video demonstrating it.

    You are correct. I poured the water after cleaning the client into the sink. Prometric does not care if the water is poured into the toilet or the sink. Both ways are acceptable. Take care Elizabeth.

    Hello Rock-An-A-Rollin95, When you take the state exam, you won't have this problem. All students are screened before the test begins. Now in a real-life situation, you will want to get with the charge nurse to have directions. Different things may be happening with the patient to keep them from raising their knees and hip. (hip replacement, blood clots, arthritis, etc...)

    Hello Serena, Peri Care 1. Once the resident has been cleaned does it matter if we place an underpad underneath them instead of a towel? Prometric requires in step 15: Leave resident on dry underpad at completion of procedure? Prometric does not care what is used as an underpad. 2. 2. Also, should we wait to put soiled linens in the hamper until we are cleaning up, or can we place them in the hamper as soon as we are finished with them? Prometric does not care if you put the dirty linen up when cleaning or at the end. To receive credit you need to make sure the linen is put up before you complete the skills. See Step 16: Complete skill having rinsed, dried and stored basin, placing soiled linens in hamper, disposing of trash, and leaving overbed table dry? 3. .Lastly, do we close the curtain as we leave to gather our supplies and to clean the basin towards the end, but leave the curtain open after the skill is complete and we leave the residents room for the final time? I always get confused when it comes to the small details! I teach students to open the curtain in the beginning and to close the curtain with the closing.

    Hello Dr Lee I love your videos you try and help out on teaching younger people and older people.. on how to clean the vulva/vaginal because some of us wasn’t taught properly.. I’m a young teenager and I’m actually kinda of confused on how to keep my vulva/vaginal healthy when it comes to washing.. I wanted to explain to you how I do it to make sure I’m doing it right.. if not then can you fully explain it to me.. I wash from my Clitoris down to the perineal line.. or do I suppose to wash from my perineal line up to my Clitoris.. I was kinda embarrassed on asking but you’re a nurse so I know you wouldn’t tell me wrong thank you!

    @@txcnaskills2599 thank you Dr Lee you’re a amazing Dr to teach these things and to have patience to explain.. they should have Heath classes for these type of things in school.. i have a few more questions then I’ll for sure know that I’m on the right track.. when washing the butt area what’s the proper way wash from the part where the poop come from to the crack.. or from the crack to the rectum.. and is it a proper way to wash your cheeks.. like to start at the end of the lower back down i apologize for all the questions.. I just prefer to asks a dr to get the full understanding.. can you tell me everything I need to know about how to properly wash the buttock area.. from how to wash the butt crevices and etc.. thank you for your time and for your good advice!!

    Hello Obianuju, Prometric does this skill a little different from Pearson Vue. If you go back to your notes and look at the Prometric Nurse Aide Clinical Skills Checklist, you will not find a reference for using the side rails in this procedure. Prometric wants the client to STAY away from the edge of the bed at ALL times. If the client rolls and grab the side rail, he or she will be near the edge of the bed and this will be a reason to not pass the exam under indirect care.

    Hello Cynthia, You would remove your gloves AFTER putting up ALL of your supplies. Prometric does not require changing gloves during this skill.

    Dr. Lee, I am getting ready to take my test in a couple of weeks. I just have one question, after you washed the basin and use a paper towel to grab and put away, you grabbed the soap bottle with a gloved hand and put it in the basin, I am observing everything to a T, is this okay since gloves are dirty?

This is acceptable because you HAVE NOT started to wash the patient. Prometric requires glove to be on when BEFORE you wash the patient. 2. and on when removing the privacy blanket at the end without gloves? this is after supplies and basin etc. have been removed then at this point remove gloves and "washed hands" . after that remove privacy blanket , put in linen and do closing steps . Is this ok to do? hope you understand my questions. thank you for all your help Prometric requires GLOVES to be on when removing dirty supplies. The privacy blanket has been on the patient; therefore it is dirty. You must have your gloves on to remove the privacy blanket.

    Hello Claudia, When performing pericare, all of the water can go down the sink. Think about it. Some people take a wash-off in their private homes at a bathroom sink. We don't take the water and put it in the toilet. This is different for the bedpan skill and measuring urine output. For these two skills, the rinse water will go into the toilet because we don't put waste products in a sink.

    Prometric does NOT have a requirement to put on gloves before you gather your supplies. Let's think for a moment. Gloves are considered a part of PPE. Healthcare workers do not wear PPE in the halls. The clean supplies are located in the halls. Therefore, we Do NOT put on gloves and go get our supplies. Prometric guidelines REQUIRE gloves to be on BEFORE you start washing the client. You will SAY that you are washing your hands BEFORE going to get your clean supplies with your bare clean hands. I hope this answers your question.

    Hello Shay, Prometric does not grade for everything we consider as contamination moves. To answer your question, Prometric will not count against the student is the soiled bed protector is placed on the sheet. Yes, the soiled bed protector will contaminate your gloves which is why we wear gloves instead of cleaning a patient with our bare hands. Prometric will use the skills checklist to grade the student during the test. I hope this is clear. Take care and thanks for the questions.

    Hello Nerm, The state Requires the gloves be on BEFORE washing the patient. So, it does not matter is they are on when testing the water or placing a pad under the client's buttocks. Both steps are before washing.

    Hello Caroline, I teach according to Prometric's grading style. Raising the bed for body mechanics is NOT a step for any of the Prometric skills. Remember not all patients have an electric bed, especially when doing home health. Prometric requires the gloves to stay on until all of the dirty items have been cleaned and put up.

    Hello Felecia, Prometric can only grade the students on the indirect care steps or the clinical skills checklist. Changing gloves after removing the dirty pad is not graded by Prometric.

    Hi Dr.Lee! Is it necessary to place a bed pad underneath the patient even if they aren’t incontinent? I was told that if they have incontinence, that is when you leave one underneath otherwise it is ok without. Also, is using one towel ok for drying the front and back?

    Hello Shea, I hope I understand your questions. 1. For the Prometric state exam, you MUST remove the pad/towel under the client when you start the skill AND replace it with a clean dry pad/towel. 2. For the Prometric state exam, you may use the same towel to dry the front and the back of the client. Think about it. Most of us who are not in a healthcare setting, take a shower/bath and we dry the front and back of our bodies with the same towel.

I meant once you’re completely finished with bathing should you leave them with a clean bed pad underneath, of course after firstly removing the wet one? My training instructor said that only if the care plan says they’re incontinent, to leave a pad for accidents. But if they’re continent, you don’t have to leave a pad underneath them. And yes I figured that as far as using one drying towel! I thought about it that way but wanted to be sure.

    Hello Tatyanna, The proper way to wipe the buttocks to prevent infections is to start at the bottom near the rectum and wipe upwards.

    Hello Lisa, I think you are asking 2 questions. 1. When I clean the vaginal area, I do clean between the labias. 2. You can raise the bed to your waist level. I don't encourage my students to raise the bed up because the student might get nervous and forget to put the bed back down which will make the person fail that skill.

    Hello Daryanna, The guideline doesn't require us to put a linen protector/pull sheet under the client for the exam. You will not get points deducted if you do or don't. I DON'T teach my students to use a pull sheet. I teach my students to move the manikin over as needed for the Peri care skill.

    Hello, You don't have to describe your actions to the evaluator. Prometric requires that we explain to the patient what we are planning to do before actually performing the skill. Most of my students find it helpful to talk through the skills while doing the skill because it helps the student to remember what to do. I hope this makes sense. A part of indirect care with each skill except handwashing is talking to the client.

    I worked in a nursing home for years and we always emptied in the sink. Unless, the patient had a bowel movement then you would have to empty it in the toilet.

    Hello Sarah, Raising and lowering the bed is NOT required for this skill. I teach my students to leave the bed as it is. The reason for this, it that some students get nervous and forget to put the bed back down. Not putting the bed back down is a flunk. Therefore, for the test, just leave the bed as it is and pass.

    Hello Ari, I live in Texas. I am not familiar with Massachusetts guidelines. My understanding is that Prometric uses 5 skills on their state exam. Handwashing and Indirect skills are not written on a card that is given to the student at the test site. Therefore only three skills will be written down on paper.

    Hello, Please pay attention to the skill directions. For this skill (Peri Care) the water can go in the sink or the toilet. The state does NOT tell the student where to place the dirty water. There are different instructions when dealing with the BedPan or emptying the urinary drainage bag. The STATE gives specific instructions as to where the rinse water goes. So are you confused about this skill or another skill?

    Hello, I just thought about something that may help you know when to pour the rinse water in the sink. Some people take a sponge bath at a sink. Therefore the person wash and rinse at the sink. So, we can pour BATH water in the sink. All fluids that we eliminate (urine and BM) MUST go into the toilet. This includes the rinse water. I hope this makes sense. If not let me know.

    Hello Kimberly, I totally understand what you are saying. Please keep in mind that linen in the workplace may be a treasure. Therefore, we need to use only enough to get the job done safely and save linen for other clients. I will try to make another video about using the washcloths in 4-5- sections. You will be just fine taking your exam using 4-5 extra wash cloths. Do what you need to do to pass the exam. Good Luck.

    Hello Christer, That is a god question. Prometric does not care if the patient's knees are bent or not. What is important is that we minimize exposure of resident. This will be graded under patient's rights for indirect care. while bathing

    Hello Baraket, Prometric does not tell us where to put the bath water. It is acceptable to place the water in the sink or toilet. In real life situations, many patients take a wash off at the sink and clean their peri area. These patient don't sit over a toilet and wash themselves. Therefore, during testing this is reasonable.

    Hello Tayyaba, Prometric will not count it against you if you give the call light to the patient while you are at the bedside with gloves on. Some people wait until the end after washing their hands. Bottom line it does not matter. What you will be graded on is giving the resident the call light before the end of the skill. Take care.

    In the real world, you're going to want your gloves ON when you touch a call light. People get all kinds of nasty stuff on their gloves then grab that light. You don't want that mess of germs

    @@txcnaskills2599 Never mind the question but I was asking why did you roll over the patient and then lift her up again just to put the linen protector, why didn't you roll it under her while removing the dirty one but I passed my HHCA on the 2nd of November.

    Hello Dr. Lee. I thought the center of the vagina was the last part to wash instead of the first since it’s the most contaminated area. Am I mistaken? Thank you!

    Hello Dyan, Prometric does not indicate the order of cleaning the per area. You must include certain areas in the procedure.

    Also, i noticed you gave Ms Mary her call light with your gloves. Is that ok? I thought our hands had to be clean?

    Hello Nerm210, The state will not count it against you if you give the client the call light with your gloves on. I do try to wash my hands and then give the patient the call light.
