Progressive supranuclear palsy (PSP) is a rare brain disorder that causes problems with movement, walking and balance, and eye movement. It results from damage to nerve cells in the brain that control thinking and body movement. The disorder’s long name indicates that the disease worsens (progressive) and causes weakness (palsy) by damaging certain parts of the brain above nerve cell clusters called nuclei (supranuclear) that control eye movements.
PSP is different than Parkinson’s disease-another movement disorder-although they share some symptoms (see section, “How is PSP different from Parkinson’s Disease?”). Currently there is no effective treatment for PSP, but some symptoms can be managed with medication or other interventions.

What are the symptoms?
PSP affects movement, control of walking (gait) and balance, speech, swallowing, eye movements and vision, mood and behavior, and thinking. The pattern of signs and symptoms can be quite different from person to person. The most frequent first symptom of PSP is a loss of balance while walking. Individuals may have abrupt and unexplained falls without loss of consciousness, a stiff and awkward gait, or slow movement.
As the disease progresses, most people will begin to develop a blurring of vision and problems controlling eye movement. These symptoms may include:
Slow eye movements.
Trouble voluntarily shifting gaze vertically (i.e., downward and/or upward).
Trouble controlling eyelids.
Tendency to move the head to look in different directions.
Involuntary closing of the eyes.
Prolonged or infrequent blinking.
Difficulty in opening the eyes.
Inability to maintain eye contact during a conversation.
People with PSP often show alterations of mood and behavior. These symptoms may include:
Depression.
Apathy.
Changes in judgment, insight, and problem solving.
Difficulty finding words.
Loss of interest in ordinary pleasurable activities.
Increased irritability and forgetfulness.
Sudden laughing or crying or displaying angry outbursts for no apparent reason.
Personality changes.
Other symptoms may include:
Slowness of thought.
Memory problems.
Slowed, slurred, or monotone speech.
Difficulty swallowing solid foods or liquids.
Mask-like facial expressions.
What causes PSP?
The exact cause of PSP is unknown, but research suggests that it involves a gradual deterioration of brain cells in a few specific areas in the brain, mainly in brain stem. The death of brain cells in one of these areas, the substantia nigra, accounts in part for the motor symptoms that PSP and Parkinson's have in common.
The hallmark of PSP is the accumulation of abnormal deposits of the protein tau in nerve cells in the brain. These deposits cause the cells to malfunction and die, which stops the flow of information to other nerve cells. The accumulation of tau puts PSP in the group of disorders called the tauopathies, which includes Alzheimer’s disease, corticobasal degeneration, and some forms of frontotemporal degeneration.
PSP is usually sporadic, meaning that it occurs infrequently and without a known cause. In very few cases, the disease results from mutations in the MAPT gene. This mutation provides faulty instructions for making tau to the nerve cell. Genetic factors have not been implicated in most individuals.
How is PSP diagnosed?
Currently there are no tests or brain imaging techniques to definitively diagnose PSP. An initial diagnosis is based on the person’s medical history and a physical and neurological exam. Identifying early gait problems, problems moving the eyes, speech and swallowing abnormalities, as well as ruling out other similar disorders is important. Diagnostic imaging may show shrinkage at the top of the brain stem and look at brain activity in known areas of degeneration.
Is there any treatment?
There is currently no effective treatment for PSP and symptoms usually do not respond to medications.
Parkinson’s disease medications, such as ropinirole, rarely provide additional benefit. In some individuals, other antiparkinsonian medications, such as levodopa, can treat the slowness, stiffness, and balance problems associated with PSP, but the effect is usually minimal and short-lasting.
Botulinum toxin, which can be injected into muscles around the eyes, can treat excessive eye closing.
Some antidepressant drugs may offer some benefits beyond treating depression, such as pain relief and decreasing drooling.

    My father is buried at Arlington National Cemetery had PSP. It is a gut wrenching process to a watch the human body shutdown! It was far worse for my father…he was trapped with no way of communicating with us. He would also laugh at dirty jokes! FYI-Dudley Moore passed from PSP! God bless anyone with this and those around them!

    My dad passed two weeks ago. It was and still is rough, but I am glad he is no longer suffering. Also, whoever edited this video using upbeat jazz was a bad choice. lol

    My mum was diagnosed with PSP about 5 1/2 months ago, but we knew something was wrong for nearly 2 years and to see how fast she's getting worse is so heart breaking and really upsetting for us all. We'd never even heard of it before. I wish there was more research and awarness about it.

    I'm sorry. That is fast. My mother was a very active 64 yr old when she started falling constantly. She broke 7 bones with 11 falls in a year and a half. She is now bedridden and a few weeks ago she had a big aspiration event and has been hospitalized since for aspiration pneumonia. She can no longer even swallow liquids amd barely speaks. I can hear she still has her voice but she barely moves her mouth and tongue. She will have to be on a feeding tube from now on. I can't believe how quickly it has come in 5 years.

    PSP is a truly evil disease. I watched my partner fall victim to PSP over a few years until his death in 2017. I pray for more research into this rare disease.

    My heart goes to all who either have PSP or someone close to them has it. My father passed away 2 years ago due to a tragic accident that only occurred because of his PSP. We like to think God didn’t want him to suffer from PSP any longer. He suffered for 3 years and it mainly affected his mobility and speech, both of which rapidly got worse as time progressed. We were able to determine it was exactly what the doctors diagnosed after UCLA studied his brain after his passing. We were glad UCLA was able to study his brain and encourage all those with PSP to be donors and donate their brains to science after their passing.

    For those concerned about death dont be, I've had PSP for thirteen years, yes I'm really Fked, and have feeding tube fitted ( a peg), But i fight daily against this bugger of a decease and you must do the same. God Ble4ss all those who suffer such illnesses.

    You have use of your hands or voice? I'm glad.y mom can't type, wrote, talk, swallow, walk, sit up on her own, is incontinent, and now in the hospital for aspiration pneumonia. She will be getting a peg tube tomorrow.

    My grandpa got diagnosed with this in 2020 and at the time he was walking fine but would have occasional falls. Now, he needs a walker and someone to walk with him wherever he goes (he lives at home with us) and every time he eats, he coughs even if the food is almost puréed. It truly is a fast progressing disease but just know, on the outside they may look and act different but inside they’re still there. I get anxious as well but be there for her as much as possible and try to make her laugh as well. ❤

    Same here. My mum was diagnosed last year, but the first symptoms showed themselves 5 years ago and in the last year it's progressed massively. She now struggles to see things, can't walk far even with a walker, her speech has gone very slow and she gets confused. It's awful. It has taken us months to get any care in place and we only got that because she was admitted to hospital and they wouldn't let her out until it was in place 😪

    If he’s choking, come on, you may consider a swallowing test regularly to see if you would need additional assistance, overtime, and what exact consistency is right for him. With my mom, we need to feed her a spoon by spoon and remind her to swallow in between spoonfuls of nectar smooth. Puréed Food only. Sending blessings to you and your family.

    ​@@2212GirlI know your comment was a year ago so maybe you have already experienced this but if not, please be aware.....I just found out the hard way...my mother used to cough every time she drank ( thin and thick liquids) and then she had one big choking event 2 weeks ago. She has been in the hospital since with aspiration pneumonia. I was told that it didn't likely just come from the one big event but that her choking was aspiration each time. And then I find out that 1. Choking is the leading cause of death for people over 65 and 2. Aspiration pneumonia is the leading cause of death for people with Parkinsons and abnormal Parkinsons diseases.

    My grandfather lost intrest in his boat 2 years ago he is a avid hunter fisher very unusualy fast forward to now hes in the end stages of psp

    This is very dangerous disease and has no treatment. My father fought with this disease almost 5 years and ultimately lost. Patient and his/her care taker suffered lot in this disease.

    My significant other was diagnosed 2021, and the disease seems to be progressing rapidly. He recently started having epileptic spasms going into complete collapse! Scary to me, I just want to know if this is yet another symptom or more serious.

    Doctors said my dad may have this from MRI results. This started maybe 3-4 years ago and we noticed his speech was impaired and he isnt as sharp as he used to maybe 2 years ago. Is this considered catching it early on? Is he life expectancy really only 5-10 years?

    Yes dear...only 6 to 9years. As my father also died on March 2024. Psp detected in 2014 15. It's very cruel disease please support your father as dar as possible after few year he will be no more with you.i miss my dad.he suffered a lot with this disease but hus will power n our support system make him to fight like a warrior. Please do care n spend maximum time with your father as within few years he will lost his speech.

    9 years for some. I think the range is about 6-9 from what I learned. That said it all depends on how they take to treatment and overall health.

    Very fast...with proper care one can survive atleast 6 to 8 years...as my father died within 8.5 years.as per doc he might die in his 6 years of psp but my dad's will power n our support system he fought like a Warrior n survived for 8.5 years.

    I think air embolism (when one or more air bubbles enter a vein or artery and block it) in brain is reason of progressive supernuclear palsy
