    They are using Australian accents, but the words work okay for American English as well.

    I didn't know that an answer is expected for "how do you do?". I thought the "answer" is always the same question again - so "how do you do?".

    Correct. For me, it sounds weird to answer "I am fine".

    "how do you do?" will be replied with "how do you do" also if they have never met or acquaintances before. but "how do you do?" can be replied with answers like "I'm fine", "I'm very well thankyou", and etc if they were introduced before. In the video, Mr. Tang and Mrs. Lin Chen have been introduced by Mr. Sam. so it's only natural that Mrs. Lin asked Mr. Tang "how do you do?", Mr. Tang will answer "I'm very well thankyou" instead of answering the same question.
