This song makes me feel like I'm floating over a burning city as a ghost, watching the terror, and destruction of mankind. I want to help the people who are hurt, the families who are grieving, but I'm quickly carried away by the wind.

I thought I'd throw in my two cents into what this song is saying. Honestly... it's about being an Artist. For him it's specifically about being a singer/song writer. This is going to be a bit of a long explanation, so please bear with me. "There are twenty years to go" is actually about how long successful bands will stick together, before retiring or death of some sort, that's not even including if they have a massive breakdown and split up. It's the beginning, a countdown towards a successful career. 20 years. "Who'll wear the hat" is likely referring to who's going to be successful, meaning that once you've been doing this job long enough you can see the talent and understand who has the potential to make it big. Personally, these first lines sounds like the singer is speaking directly to a person newly introduced into the world of Music, being in a Band, and quite possibly hitting it big unexpectedly. "The best of all I hope" is him wishing the new guy luck and happiness. "Enjoy the ride" basically is 'Enjoy being famous and successful for as long as it lasts, however long that is'. "The Medicine show" is him talking about the possibility of becoming hooked on any substance just to keep yourself afloat. Basically telling him that he could be drunk or high on stage and performing, and how he's making it sound normal the singer is actually introducing him to the darker possibility his fame's pressure. "Them's the breaks" And "For we designer fakes"... Living up to your own fame can give you the sense of loss of self worth. Like somehow you're life, your art, has been turned into a brand that you have to constantly live up to, leaving you feeling like you're living a fake version of yourself. "We need to concentrate on more than meets the eye" is the singer telling the person that they can't just 'Be' people anymore. They can't hang out with long-time friends at the theaters going to see a bad movie because of the image it projects onto them and their brand. They need to constantly be aware that everything they say and do and believe can create or destroy their careers, they lose the right to just live normally. "The faithful and the low" is talking about the ebb and flow of fandom in general. You can release one song and it hits top charts for months and your fans remain loyal. Other times you can release entire albums and your fans don't know you're even still a band. It's the singer, once again, telling the person that no matter how hard you try, your success depends on the fickleness of your fans. "The best of starts, the broken hearts, the stone" is the singer recounting what he's seen. Bands that come swinging onto the scene and are beloved for a short time, forgotten, then the bitterness of irrelevance. "The punch drunk and the blow" is referring to the artists that have scraped, clawed, and fought like hell for their success, then they take that last dip into irrelevance and they never return. "The worst of starts, the mercy part, the phone" is talking about bands that did horribly right out of the gate. They try hard, and they're even acknowledged by possibly tens of thousands as trying their best, and no one mocks or blames them but instead feel bad for them and keep trying to encourage them on. The phone is very literal, it's a call. A band that does poorly for too long, even if the crew is best friends, it's just time to call it. "But it's you I take, cause you're the truth, not I" is the destroyed sense of self worth. If the whole world thinks you're great, and no matter how much like a jerk you think you are, the world won't accept it. Same goes for the opposite. Popular opinion will override the truth. You're not allowed to have a differing opinion of yourself, because you're either being too modest or delusional. "A golden age I know" being famous and relevant leaves you feeling important. You can do things and get away with others purely because you're famous. Basically just being able to do things you normally couldn't is fun. "But all will pass, end too fast, you know" the singer reminding the person that this 'Golden Age' is fleeting, so the person should enjoy it but to understand that the person shouldn't get used to it because it'll be gone just as soon as it came. This includes not just the perks of fame, but fame itself, and the idolization and adoration with it. "Though some may hold the rose" And "Some hold the rope"... not all the friends you'll make will be worth having. Some will make your life great, colorful and vibrant... regardless of your fame. Others will help you destroy yourself and your career, and once you're not famous anymore they'll leave you. (Really long section of verses) is the singer hammering the point home. He's wished him well, and warned him of what may be to come. "It's the long and short of it". The last section where it's repeating "You're the truth, no I" I believe is the singer letting the person know that he knows that the person's own idea of his burgeoning fame will be like and how it'll be different. Basically saying: "I know you think you're right, because everyone says fame is awesome. Because Everyone Else says fame is awesome." All in all... after doing a pretty lengthy dissection of this gorgeous song, I believe it's a lament. It's a congratulations to anyone who's just starting to get famous or officially became famous, letting them know the singer wishes them nothing but the best and true success. But... there's 20 years of success... and bad road ahead. Your fame is going to be awesome, for now, eventually... you won't be you to the people who don't know you personally. You'll be the idol they've created, the brand-safe version of yourself you can never break, and eventually that freedom that fame brings you becomes a solitary prison. "Congrats kid, you made it to the big time. The only way out, is through." But that's my 2 cents. Regardless how wrong I might be, I still love the song.

In a interview, Molko said that "20 years" was an idea a friend of him from another band had. This friend was always playing the riff along with the phrase, however he never made the song, so Placebo start to make this song as a way to remember him. I don´t know if this works as an answer but I think it could be useful to understand the song.
