Hello- my father (80) has had recurring headache/vertigo (he had ear issues) and became worse last couple of months which promoted MRI scan- report just details enlarged pituitary is 14x14mm w few necrotic foci but no supersellar extension. , no invasion etc. He had recent blood tests- normal, he has had no vision problems either. Will be taking him to specialist soon but curious about your take.

➤ Superior margin of the pituitary gland is convex / elevated (2mm) with well defined rounded enhancing lesion (~9x7mm) in the central aspect of pituitary gland and the lesion is extending into the suprasellar region, not abutting the optic chiasm. The infundibulum is centrally located. Findings are suggestive of - Pituitary macroadenoma....sir av surgery ki jarurat hai kya...hormone profile normal hai

Had macro pituitary adenoma and had very successful excision of the tumor. Everything is back to normal, my sight and hormones are normal.

Is the tumor completely removed this time or is there residual tumor ? In case if there is residual tumor better get gamma knife therapy done at your place. If the tumor is completely removed then follow up with regular MRI and so that if there is any sign of tumor regrowth it can immediately be subjected to gamma knife therapy

Prolactin level of 2000 is highly suggestive of prolactinoma. It should come down with cabergolin therapy don't worry

A female patient 17 years old complaining of amenohrria since 3-4 years as well as polyuria and polydepsia and sometimes headaches. If all her symptoms also resolved along with normalisation of prolactin, then u can for now ignore about the stalk thickness and do a repeat MRI after 6 months

It's a small tumor (microadenoma) and if your hormones are normal there is nothing to be worried about. You may just need regular follow up MRI to look for any significant increase in the size of tumor and vision charting

May I know your prolactin levels ? If its a prolactinoma it shrinks with cabergolin however if the raise in prolactin is because of the stalk effect as explained in the video then it may not shrink

Sir, I took an MRI 6 months ago and found a microadenoma 3mm my prolactin was high & my vitamin D was at 10. My endrocrinologist did lots of blood work to see if tumor was affecting other hormones besides my prolactin, and it wasn’t. I recently started to have irregular period cycles. Last week MRI results the tumor doubles in size it is now 6mm. Endrocrinologist is doing more tests to see if hormone levels are being affected. She recommends starting the Bromokriptin medication. What are your thoughts on that medication?

It's high but not in the range of prolactinoma. But it's possible because of stalk effect which I discussed in the video. Since you are having irregular menses, it is better to take bromocriptine regularly

Like the doctor said Cabergoline 0.5 should resolve the level after 6 to 7 months, from my own experience and the right dosage for me was 2 times a week Monday and Friday before bedtime. My level went from 174 to 5.7 and a regular mri checkup every 9 o 12 months for any increase or decrease in size before doc recommends me to be off cabergoline. Hope this help and I’m hoping to stay away from surgery.

My prolactin levels are normal but I have microadenoma.....last year i had been on a contrasptive pill for irregualar periods and suddenly my prolactin levels increased and was diagnosed with microadenoma. But my prolactin levels are normal as soon as I stopped the conrasaptive pills. Will the adenoma shrink on its own sir? Because I don't take any tablet for prolactin as they are normal in range. Hello Dr Moorthy I was diagnosed with a 2 mm pituitary microadenoma on 1/29/23 MRI . My prolactin levels were 27. 8 in 12/2022 then 23.1 on 1/04/23 and back to 16.9 on 1/10/23. I originally asked my doctor to check my hormones as I noticed lack of sex drive and for a few weeks I was sleeping 14-16 hrs . My sleep regulated it self in late December and the low libido I think it's from anxiety over previous gynecological procedures in 2022 My question is , what is your opinion on treating this with medicine vs surgery vs monitoring it ? My doctor is referring me to Endocrinologist to discuss but my doctor said they could start me on Cabergoline .05 half tablet twice a week . Good morning Dr. Skanda My husband was diagnosed last year with a benign pituitary adenoma his only symptom is vision loss. He has partial vision on both eyes. His initial scan measured 3.4 centimeters. He has had two surgeries both of them you mentioned in the video. The surgeons have not been able to remove the whole tumor since they said it was hard like rubber and they are waiting to see if the tumor can fall on its own because removing it can be dangerous. Would the medications you mentioned work on my husband's tumor? Also, we have bee. Trying to convince for about 8 months now but no luck. Could the tumor be affecting his reproductive hormones? Hi...medication would work if it's a prolactin secreting pituitary adenoma (prolactinoma) ... according to what you have mentioned I guess it's not a prolactinoma because in that case your doctors would have already prescribed to him

So high prolactin is definitely indicative of prolactinoma...first line of management will definitely be starting with medication (cabergolin) however management will also depend on multiple other factors important one being MRI brain finding

Surgery would not be recommended for 2 mm cyst anyway. It's very unlikely that such a small cyst causes any vision problems . Pls get it evaluated it maybe because of some other problem in the eye. Hormonal replacement can be done for hormones which are less in quantity

The very fact that you were operated twice means it was not a prolactinoma. However treatment now will depend on multiple factors like its size , hormonal levels, your vision etc. If the tumor is not large and depending on its relation to the optic nerves gamma knife therapy can be one option to decrease it's chances of further growth

Empty sella can happen because of multiple reasons . And about estrogen replacement it can have complications ..may or may not too...hence you should follow your regular doctor's advice

Pls Get a complete hormone profile ...if it's a prolactinoma it can be managed with medications only...but in your case it's a borderline case as she is pregnant too and already has blurring of vision ...operation though comes with more than usual risk, can be considered in second trimester pregnancy

You mentioned you can feel a small change in vision. By change, do you mean improvement? If so it's a good sign. If you are having some improvement then with time you may have more. It's generally not possible to predict the exact time but roughly in about 3 months we will get a good picture of the improvement

Get a baseline vision test including visual field charting. Prolactin is slightly elevated due to stalk effect. If you develop symptoms , you may need cabergolin

Value of 150 is not really indicative of Prolactinoma. It could just be because of stalk effect as explained in the video

Sir my father 80 his CT scan report shows " Homogenously enhancing hyperdense lesion with no calcification of size 35 by 30mm with no perifical edema in Sella region ,MRI with MRS suggested." Pt has no symptoms, it was incidentally detected, MRI & other harmones relating tests performed, MRI shows it as macroadenoma size 3cm,generalized cerebral atrophy, Na 131, K 4.2, chloride serum 94,Fsh 14.76,Tsh 3.11, ACTH 17.30 & Prolactin 27.66, will be highly obliged if you guide & suggest, Thanks.

Rest of the hormones are normal? If so then this is a non hormone secreting pituitary microadenoma. If you are absolutely asymptomatic then there is no need to undergo surgical excision for a microadenoma. But you need to be under regular follow up and may require surgical excision of there is significant increase in its size in the future

Yes u ll require replacement for that ...but ur adenoma I wouldn't advice to get operated but still my suggestion is still to follow ur treating doctor s advice as he / she knows the complete picture

Basically, they like to make money off of "watching" it. I stopped getting MRI's of it 8 years ago. I get regular field vision test, my adrenals are awful, my testosterone is too high, I have pseudo cushings and they refuse to do anything about it. I am getting my first MRI in 8 years. They do not care. They can not even tell us where it comes from. If you are wealthy, they will remove it, or if they catch it secreting something, otherwise be prepared to spend a bunch of money on labs and test, with your hormones getting worse and worse.

I just saw through a youtube video how it is possible to remove a non-surgical pituitary tumor near you then how is it done and if my wife has been suffering from this problem for six to seven months then can we get this treatment and how can we go if we get it I am from Bangladesh

Did you have any symptoms at a prolactin level of 62.5 ? It's not really too high anyway. You need to undergo follow up MRI after an year to look for any increase in the size of the adenoma . Headache may or may not be related to this

If the patient has symptoms because of elevated prolactin, then cabergolin is given to bring it back to normal

As my prolactin level has reduced to 77 but macroadenoma is there with some bleeding so doctor has suggested for surgery and to stop cabergoline as it would solidify the lesion. So is there anyway to avoid surgery or that's the only treatment?

If it's a macroadenoma and has bleeding inside (called as pituitary apoplexy) then it's an absolute indication for surgery. Better to go ahead with the surgery as rightly advised by your doctor

It could be because of multiple reasons, like either inadequate dosage , cystic changes in the tumor or sometimes it might just be that it's not responding to cabergolin.

It would not be right on my part to comment much without examining you or seeing the MRI. But if your prolactinoma is not responding to medications, and if the size is quite big it would be better to operate and remove it. However I would still advice you to follow your treating doctors advice.

2 mm sized microadenoma is very unlikely to cause blurring of vision. Pls get your exact Visual acuity and Visual field tested. N as rightly advised take the tablet regularly for a prolactin level of 760

Hello dr i have been taken dostinex and after cabergoline for 7 years with no decrease in my prolactin 1500 now it has been 5 months i am taking bromocriptin (parlodel) started with one pill each day and my prolactin remain the same but after increasing the dose to 2 pills a day my prolactin level start decreasing till now 359 .should i keep taking bromorcriptin or is it ok to stop .i still have pain in my head and most of the time stress and sometime Loss of consciousnes.any advice

This may or may not be a prolactinoma , it maybe a stock effect which I explained in the video ..just continue for one more month and you can repeat your prolactin values

Sir my prolactin level 193 then doctor give me cabgolin 0.5mg.After consuming this medicine 3 months doctor provide cabgolin 0.25mg.But problem is that when I eat cabgolin 0.5 my headache and eye irritation symptom is completely absent this symptom present before eating 0.5.Now when I eat 0.25 my headache and eye irritation came back. My brain MRI is normal,TSH also normal.what I do?

Hi.. Normally in Brain MRI, the prolactinoma does not get detected. If possible, please get a Dynamic MRI of Pituitary done. It will give you a clear picture.

Everyman should keep prolactin below 7 and never let it exceed 10! For woman, upmost limit is 15-20, any value above that is harming the body. Many out there tell below 30 is in no problem zone, which is a false opinion

There are more than 100 brain tumors pituitary adenoma (prolactinoma) is one of them

Can the same treatment work on spindle cell oncocytoma,even after two surgeries the doctors were unable to remove atleast a 50% of tumor beacuse of excess bleeding, they said so, we are unable to decide our further steps, if anyone of u can help us we will be grateful 🙏

Hello Doctor, My mother had benign pituitary macroadenoma which is 2 cm in size. Our doctor undergone a surgery through nasal passage and removed the tumor. Post-operation after 2 days they have undergone a CT scan, report follows: 1. A Poorly defined soft tissue dense foci with minimal enhancement is noted in right para cavernous region. The same measures 15*11 mm. Consider possible residual foci . Needs follow up 2. Both cavernous ICA flow otherwise appears normal Sir, here my actual doubt is does it regrow again after several years? If so, what is the treatment for that

Yes it does regrow...the rate of regrowth is variable. The residual tumor can be subjected to Gamma knife therapy which will decrease the chances of regrowth. However in case of significant regrowth a repeat surgery maybe required
