Dr. Forman is the medical and lab director at Columbia University Fertility Center where he oversees the medical practice as well as IVF embryology and andrology labs, while also actively seeing patients. He set the standard for eSETs (elective single embryo transfers) across the globe. And, he revolutionized how we freeze eggs, with his research proving that the rapid vitrification technology does not increase the risk of genetic abnormalities in embryos.
Dr. Forman is joining me today to discuss a hot topic in fertility medicine: PGT-A. We are going to discuss preimplantation genetic testing (PGT), the science behind this testing, and a recent paper, published in the New England Journal of Medicine (Live Birth with or Without Preimplantation Genetic Testing for Aneuploidy).
Welcome to the show, Dr. Forman!

I started my IVF journey to do PGT-m, as we have a daughter with autosomal recessive disease. my first egg retrieval was yasterday and they have collected 10 eggs, I decided to freeze the embryos from this cycle and have another stimulation and retrieval before doing PGT-m to save some money. Hoping to have good number of healthy embryos 🙏🏻

This is such a great and concise explanation, exactly what other doctors have been telling me

Thank you for the clarity. It limits women's chances that is not fair. 6K and they only test up to 8 eggs that is unreal

So glad to hear there is more research about this topic. I got 16 eggs but ended up with just 4 embryos. Without genetic testing, I never would have known that half of these - which were all “high quality embryos” - were abnormal and would certainly have resulted in miscarriages. I’m only 34.

Isn’t it the case that competency and precision varies greatly from lab to lab? I have 14 frozen untested embryos, one failed transfer. I’m considering thawing and testing them (or some) but I don’t know how to tell if my lab is one of the good ones. I’m wondering if it’s worth switching to a renowned one like where this doctor works, just for the testing. Is that a lot of stress on my embryos?

Do you think a couple with a balanced translocation (male half) would be better off when doing IVF to do pgt? We already had one failed cycled with a 5AA normal tested embryo… we miscarried at 8 weeks. It’s our 4th miscarriage but first IVF cycle

that's so hard and I'm so sorry. Talk to your doc about other causes of miscarriage before your next cycle. I would recommend IVF with PGT-SR again.

7 day blastocyst, 4CC embryo PGT-A tested normal...what are the chances of pregnancy?

It's so hard to answer because your lab could have a different answer. 4CC in my lab= not viable and no chance for pregnancy. But talk to YOUR clinic and ask "what is my pregnancy rate using a 4CC"

On this transfer I had 3 pgt-a embryos transferred, (had 4 chemicals all with tested eSET embryos) it was against my better judgment as I would be high enough risk for twins as I am 41and I really believed if I transferrd 3 then it would definitely be twins -well one gorgeous cute 😍 embryo took, its still early days-9 weeks but everything looks perfect-I am so so unbelievably grateful but it is scary to think had I transferred 1 or even 2 this time (which is what I would have preferred) I wouldn’t be pregnant now....

12:00 I'm not sure about this....b/c in the real world most people can't afford to PGT more than 3 embryos. It's expensive. The limitations in the study maybe due to most people that go through this can't afford the extra expense of testing ALL of their embryos. I know we only tested 3 embryos and it was over $600 usd
