It says that douma didn't want to kill kotaha, he want her to live with him until her last breath or she died on old age or a sick that cannot cure. But he says kotaha was really smart and ran away into the forest running for her life and her baby boy inosuke. On the cliff douma didn't really want to kill her but muzan ordered him to kill her.
