No, you do not need to carry the printout of the passport application form when visiting the passport office on the date of appointment. The SMS sent on the registered mobile number is accepted as proof of appointment by the authorities during the visit.

Always print the slip. Dont rely on sms in mobile. My friend went with sms and his phone stopped working. He had to face a lot of problems.

Whenever it is the case that you have different names on 10th marksheet and other documents, the only thing we need to do is ... We have to make an affidavit declaring that your is changed from this to that , then we have to publish this in two newspapers... One should be national newspaper and other should be local newspaper Then take the two newspapers and submit them in the passport office accordingly to your appointment.... Near passport office there are many agents who can do this for some fees like for Rs.1000

Yes it's necessary because in verification they will ask you questions and you have to sign in some paper etc you have to be there and I think if you work in other states then why don't you try at your location police station

@@prathamyadav._ I kept the same name in Passport as in my Marksheet, no issue in Passport Office If you want to put the original name, you can produce your Higher Education Certificate, they will take this!

My Passport was due for expiry on 8th July 2023. (It was issued on 9th July 2013) So I applied for Re-issue in June 2023. As the demand for passport application is high in Mumbai, the appointment date is approximate 1 month after the apply in website. It depends on your city. Tier 2,3,4 cities have less no of days gap. Some have slot available on very next day. In big cities like Mumbai, Delhi etc. it goes upto 30 days or later sometimes. So my slot was on July 7, 2023. Since I live in a rented apartment, I carried my rent agreement along with my Old passport. Aadhar Card. all educational certificates and a letter from my company HR (its called Bonafide certificate, but this document wasnt needed at all). One good thing I did that I have updated my rented address in my Aadhar using agreement. Let me brief you my experience. PSK: Thane Appointment Date & time: Friday, July 7, 2023 at 10.15 AM I showed my appointment SMS at the gate security along with Aadhar card at 10.15 AM. I was allowed to get inside the office. There were 5 counters where Token number was being issued. I stood in the queue for just 10 mins and my turn came. They asked me to show the SMS, Old Passport and Aadhar card (Original and xerox). They staypled my PP and Aadhar xerox and mentioned a Token no and asked me to proceed to "Section A" Section A is for biometry and photo clicking. It took on 3-4 minutes to get displayed my token no. on the TV screen and I was called inside. There I saw many counters with all TCS employees. I was asked to sit at one of the counter. That TCS guys asked my Old passport and Aadhar card. He gave me a Print out of my application and asked me to check if everything is correct. If any changes required, I could have changed coz that was the final time you can change anything, In my case no changes needed. He took my finger prints details and a clicked a photo. He took 4-5 pics and asked which one I like to keep. I choose the best one. Then he asked me to proceed for "Section B" Section B is for verification. Govt employees were sitting there, mostly aged about 45+ years old, It took 15 mins to display my token number on TV screen, The Govt employee asked for my Old Passport and Aadhar card. Checked it and in less then 2 mins, it was verified and asked me for Section C Section C is for Granting. 10 mins wait and I proceeded for Granting zone. The lady officer (aged about 50) just saw my old passport, and Aadhar. She put a stamp on my passport first page "Passport Cancelled, any VISA available will be Valid" and BOOM. I am done. I exited the building at 11 AM. It took less than an hour. All they saw my old passport and Aadhar card. Thats it.
