this is a heavier storyline that while be split into multiple parts.
in this part, once osamu rushes home from suna's house, atsumu expresses that he does not feel prioritized, as he relies on his brother to be there for him to help him get through his nightmare-ridden nights. fortunately, the two sort things out in the evening when osamu apologizes sincerely.
later on, atsumu goes bowling with komori and sakusa. while at the bowling alley, komori asks atsumu if he has feelings for sakusa, forcing atsumu to face his romantic feelings for sakusa. his feelings terrify him due to his trauma from his previous relationship; a part of atsumu fears that sakusa would put him through the same experience.

Ngl Atsumu might have trust issues but fully relying on your sibling to hear out or take care of your problems and trauma also takes a toll on them, I’m not some older sibling who has 13 siblings all with some bad traumatizing experiences but I am the therapist friend and I know from experiences that relying on someone too much takes a toll on them, just stating my opinions and thoughts, (please don’t yell at me!)

that's completely true ! i wouldn't yell at you -- in fact, i agree fully. osamu actually shows in the third part how exhausted and worn out he is due to atsumu's situation because, like you said, a role like his takes a toll on people.
