Traditional treatment approaches often involve a lot of monitoring and patience. As the condition worsens, patients are usually directed towards spinal fusion. On the other hand, conservative approaches provide alternative outcomes and methods. Spinal fusion is a surgical procedure that fuses segments of the spine together. Naturally, the spine is designed for movement and flexibility, which are vital functions of the body. By fusing the spine, we essentially remove this functionality in the treated area to improve alignment. Hence, I refer to this as a non-functional approach. The essence of spinal fusion is to fuse the most tilted vertebra at the top to the most tilted vertebra at the bottom of the curve, eliminating all movement between them. Typically, rods and screws are affixed to the spine using pedicle screws to hold everything in place. The hardware becomes a permanent addition, and bone grafts are placed between the vertebrae. As these grafts heal, the spine solidifies into a single piece of bone.
While spinal fusion is undeniably effective, it's expensive, invasive, and carries risks. The surgery alters the spine's natural movement and function, potentially reducing spinal flexibility and range of motion. Long-term complications post-surgery remain somewhat ambiguous, particularly over spans of 30, 40, or 50 years.
Conservative treatment options are designed to intervene early. The objective is to reduce curves when they are initially identified. This proactive approach seeks to treat curves when they're less severe, to prevent further progression. Surgery is often postponed until curves become more pronounced because of its invasive nature. Scoliosis surgery isn't typically recommended for minor curves since the risks of the procedure might outweigh the benefits. In contrast, conservative treatments without these high risks can commence earlier since they don't detrimentally alter the spine. They focus on strengthening the spine and maintaining its function. We aim to diminish the curve while preserving its function, not immobilizing it through fusion.
In our practice, we offer a variety of treatments to help reduce the curve. Often, patients seeking conservative treatment options must consult various specialists, from orthopedists and orthotists to chiropractors and physical therapists. This fragmented approach lacks coordination. By consolidating these approaches in our practice, we ensure patients receive integrated treatment, which can be both superior in outcome and more efficient.
The treatments we provide are tailored to address the spine's structure. Most therapies, rehab, and chiropractic exercises are usually intended for injury rehabilitation, which progresses slowly. However, scoliosis is a structural issue that has typically developed over years. Addressing it like an injury is insufficient. Thus, we focus on the curve's structural aspects. Our chiropractic techniques, manual therapies, and rehab aim to realign the spine with a structural emphasis.
Therapy plays a pivotal role, but it must be structurally based. Our therapy adopts an asymmetrical approach to restore proper alignment. We also utilize scoliosis-specific exercises to bolster strength essential for maintaining the reduced scoliosis. It's a misconception to think scoliosis arises from a weak core. Many of our adolescent patients are physically active and possess strong cores. The challenge isn't their strength but its application to correct their spine's curvature.
Neuromuscular education also plays a vital role, emphasizing the connection between the brain and body. Re-educating them about maintaining proper posture and alignment is crucial. To complement these efforts, we can also introduce corrective bracing. This bracing acts as a powerful body mold, supporting curve reduction, and enhancing spinal flexibility.
