great as always. just one thing, I think the first test for encephalitis is imaging rather than lumbar puncture. however, correct me if I'm wrong. thanks.

You are right, UpToDate, 2020, Epidemiology, clinical manifestations, and diagnosis of Cryptococcus neoformans meningoencephalitis in patients with HIV: "[...] Prior to an LP, patients suspected of having increased intracranial pressure and/or central nervous system (CNS) mass lesions must have neuroimaging (eg, [CT] or [MRI]). Discussions of when to perform neuroimaging prior to LP and considerations in resource-limited settings are presented separately. [...]"
