Definitions: Tropia and Phoria not quite correct. From medlineplus Main Entry: tro·pia: deviation of an eye from the normal position with respect to the line of vision when the eyes are open Main Entry: pho·ria: any of various tendencies of the lines of vision to deviate from the normal when binocular fusion of the retinal images is prevented Note: Tropias can be intermittent as in intermittent exotropia. They do not always have to be present

Hi there one thing I didn’t understand if problem with left eye and right eye is perfect then why the prism test in front of right eye should be not in front of left eye?? May b I am stupid but please help me I have a squint in my right eye getting double vision right eye turns out so my left eye is perfect no problem in vision and no squint but optician made glasses and also put prism for left eye I told her when I wear glasses the object is turning out in my perfect left eye. Really confused lol she said we have to divide prism for both eye but i am losing object from real position it’s annoying me I watched ur video very informative and easily to understand please guide me really appreciate for you reply

@DrJam0073 The cause of ocular misalignment must be determined 1st, but diplopia is rarely caused by "treatable" neuro problems. Most double vision is from aging decompensation, or untreatable strokes that you have to wait to see if the cranial nerve will repair itself Prism and surgery are not a "fix" but a way to keep our patient from suffering from double vision. In children, you need to fix eye alignment quickly (even surgery) or they will develop permanent vision loss from amblyopia .

The prism should be placed in front of the deviating eye to obtain the full measure of the deviation.

  • @zuzanafreidin3241

+Rooker11111 - of course not. The video shows the left eye deviating. In left exotropia the prism should be rotated so that it will be base in - in front of the left eye ( not the right eye). When the correct amount of prism is placed in front of the left eye then the light deviated by the prism will coincide with the visual axis of the left deviating eye giving the amount of deviation.

Rooker11111 Perhaps I misunderstood you or something, but to my understanding... "Outward exotropia = base-out prism." (your words). Does that mean in your words, that you would correct an Exo with base OUT? Pls just confirm or explain, might by the lack of english skill that makes me simply not understand you.

hi tim my name's paul i'm 39. i had a surgery in 2012 for a exotropia on the left eye. the surgeon left me with a consecutive esotropia due to a overcorrection of the esotropia. i wanted to fix it but surgeon told me my brain doesnt fuse images correctly and the strabismus became so minimal no surgeon wants to operate. Thing is it still shows specially when im nervous or tired and its more noticeable close than far. Medicinal glasses make the strabismus show more. Without glasses is better, but its still there. Is there anything i can do?

i know this video is old but. When you want to find how big the phoria is, by using corecting prisms, you really need the prism bar. The patients fusion has to be removed at all times, while you increase the prisms with the bar by using the alternating covertest at the same time.
