I think the jury needs to see the him dying animation from at least 5 more angles to understand what happened.
This reminds me of a time when working a nitrogen job, we had removed a check valve from service b/c it didnt pass our pressure test, for whatever reason the service supr. mgr. didnt have extra on his truck or make sure there were extras for the job b/4 leaving the yard. Instead of holding the job up and a 4/6 hour delay to pump to go get another one, he decided to put it in line for this job, we had a blow out, injured a few of us but killed a 25 year old kid who had only just started less than 6 months before. From N.M. 4 of us drove to his parents in CA. to express our condolences, I never found out what happened in the lawsuit. A lot can happen and happen quickly out there!

Many think safety shit is a pain in the ass, but it is because someone died over the thing that someone else thought they could get by with. It really hurts because the young and inexperienced don't have the knowledge or clout to say "Hell no!" Sad

Don't know why the stand wasn't latched into the elevators first, unless the derrickhand missed the latch. Also the pipe should have been racked on a gatorback or in a birdbath on the rig floor. Seen lots of accidents when I worked rigs, but fortunately never a fatality.

I have worked on beefy Rigs set up for bigger pipe then tooled down to run Spaghetti String...lots of issues if the crew is not already well versed and you have all your equipment mods done.The finger was bent and they had options to make things safer they ignored doing those things. The Push should not have been on the Floor..he was there because his entire crew was likely green running Spaghetti String...he caused because when he set the stand down hard using the tugger the hand unhooked the line and at the same time the flex of the string whipped back up to the Derrickhand who did not have the elevators on and he was likely cradeling the stand already cause its so small so he lost it immdeiatly when the whip got up to the top of the stand - he was used to 5 inch pipe which does not whip.... ...this is why the Push tried to bring the stand a foot closer it was so when the derrickhand slammed the stand into the elevators the WHIP would not fling the floorhand around...I know how whippy this stuff is because I once slammed a triple stand hard into the rack to send a whip back up at an asshole Derrickhand who almost killed me, it broke his nose I wanted to schwack him and he was so scared he would not come down until I left the Rig in the Crew Truck. You have to work with this pipe to really understand how whippy and flexible it is.Definately should have had a gatorback.

You are sitting here speaking about safety and putting crews in dangerous positions, but finish by saying when you were almost killed by a whip you made the conscious decision to intentionally send a whip back up? I’m all for physical confrontation but that makes zero sense

Really nice work
Did the derrick hand miss the elevators and drop the stand of pipe into the back of the draw works. There should have been gator matting on the rack back, and v-door area to keep the pipe from kicking like that. Plus the floor hand should have been using a catch road and standing off to the side and out of the way of a kick back like that. I know though. Take little shortcuts to speed things up. Time is money, but it also could kill you. Slow, steady, and perfect is always better than hurry, soppy, and dangerous my driller used to always say.
This almost happened to me but with 4 1/2 drill pipe that was dropped by the derrickman into the backside of the Derrick, the driller told us to pull it back with an airhoist, meanwhile the bottom of the stand started to slide and almost slid of the rig floor out the v-door, it would have been catastrophic. Luckily we got th3 stand back in to the board and latched into the elevators to pull it back up and set it backinto place.

Missed the elevator, its why we always use the pipe matts on our rigs
The Push pulled the stand in closer and when he set it down that flex caused it to slide off the bent finger whereupon the Derrickhand who was not ready lost the stand and missed the elevators sending the stand loose an down to wreak havoc.
It's a good thing I watched this.
What caused the stand of pipe to lift if it wasn't attached too the elevators?

The dumbass Push who used the Tugger to lift the stand to move it closer so that when the Derrickhand put it into the Elevators less whip would travel down to rock and roll the Roughnecks on the Floor....the elevators were probably open and waiting...if the Derrickhand had simply leaned over and put the stand into the elevators when the driller lifted the stand the big bend would have whipped down to the roughnecks trying to handle it....the Push wanted to keep the Roughnecks safe so he was trying to bring the stand closer to remove some of the potential whip...the Push failed to tell the Derrickhand that the whip was coming back up at him....when that whip got to him up top he lost the stand....and all hell broke loose.
So what actually killed the guy when he was first hit before being airborne or when hitting the ground, or the pipe hitting him after he was on the ground?
Im a dentist and i see this all the time, it happens a lot

well,that was a nice salto mortale literally..damn so many parts are rotating or under heavy tension..its dangerous in every second..i would not do that job for a million dollars...fuck that oil drilling..
Not clear what kicked the pipe foward at the top. Or what kicked the bottom out.
Ok,so in the animation the driller is not running the rig,they pulled up on the stand before it was latched in the elevators and gravity did its thing. Never seen anything so stupid.

My Grandfather was killed in a similar type rig accident in 1952 in the Hospah field.

If its unsafe to rack the pipe in the derrick ~ lay the pipe down . ya its a lot of work , thats the big bucks !

Ive run Spaghetti Strings on Beefy Rigs normally set up for larger diameter drill pipe. Never had a problem not caused by a stupid human.

Animations like this are used in court for a jury to see. The lawyer presenting this will be describing what is going on as it plays. He will be using a remote to stop, back-up, and replay parts he wants to emphasize. Adding sound would only get in the way.

So they all saw the issues with their own eyes and continued to work like that? Very sad to see people die on the job. Even sadder that workers feel they can't speak up about safety issues out of fear of losing their jobs.

Don't know for sure, with years of working on drilling rigs, I would answer No. That's 30ft from the ground to the floor and this hand was kicked way out to the end of the pipe rack. Probably the kick itself got him. In all my years, nobody would have done that "worm' move, hooking a hoist at the bottom of the heavy-wall pipe (flex pipe).

tool push on a rig floor....Im not gonna be happy as a floor hand! How long has it been since any push worked the floor! He would be the most dangerous guy there and it was his fault! And why didnt the derrick hand latch the elevators or did he miss!? I worked the rigs in northern Canada in crazy cold weather and seen some stuff but never a fatality.
