The clearest sign it was Malcolm's plan: He didn't rant at Nicola. She raved and blasted at him for something that seemed too ludicrous an idea and he had no rant for her back. Just said "What?" told her he was her only option and left before he cracked a smile at his clever work.

Considering that she'd been passively-aggressively sniping at Nicola after Nicola "slighted" her early in the episode, I think Terri knew full-well what it looked like as soon as it happened and was spitefully enjoying every second.

Like so much else, the secret to this is that they play it absolutely seriously. The confrontation between Malcolm and Nicola is written and acted as if it were a drama in which he had backed her into nearly starting a war or something... yet it's all because he shifted the way she stood in front of a sign and affronted her dignity.

If they took the sticks out their rears, the solution would be simple: Go along with the joke. The public likes a person who can have a bit of fun.
