Sir, in q 7 there were even 'blasts' mentioned and presence of PM(Premyelocytes) So the answer shouldn't be AML i.e. (15,17) which was the fourth option

Sir, regarding microcytic hypochromic anemia,though the indices favour thalassemia trait_HBA2 won't be conclusive without serum ferritin. If the pt has thalassemia trait and concomitant IDA,HBA2 will be normal. So,first we should rule out iron deficiency by Sr.ferritin values and then go ahead with HBA2. I marked Sr ferritin as they asked what is the next investigation to be done. So,someone please clarify..

"Iron deficiency is a hypoproliferative state & RBC count is invariably low in IDA" is what I got as an answer from a friend..!!!

@@chakravarthy_n It was definitely not Iron deficiency anemia. It should be 98 to 99% thalassemia trait.Question was on what is next investigation. Even thalassemia trait patients can have mild iron deficiency. Iron deficiency if present will decrease HBA2 levels Thalassemia trait will have elevated HBA2.If both present together HBA2 will be falsely normal. So it is always mandatory to rule out underlying Iron deficiency and Sr.ferritin values should be more than 15ng

@@Bharathiya2023 also let me know your take on question on cobweb coagulum in CSF & MAHA; I bluntly marked pyogenic after finding a PMN in the slide & splenomegaly as it was a case of hemolysis (failed to differentiate btw intravascular & extravascular hemolysis)
