the old guy singing along is a famous egyptian actor whose name is hussein fahmy and the original singer of this song soad hosny was singing this song to him in a an old movie made in the 60's or 70's so that's why they're focusing on him, cuz he's the original person that this song was sung to.

Soad Husni starred in a hit movie called" khalli balak min Zuzu" with a then newcomer handsome Hussein Fahmi (the now old man in the video AND MADE HIM OVER NIGHT heart throb. It's a modern day cindrella story. The song was about him in the movie. That's why he is singing along. The movie took the middle east by storm. Thanks for the memories, Nana.
