Myasthenia Gravis Nursing NCLEX Review Symptoms, Treatment, Pathophysiology Interventions

Myasthenia gravis nursing NCLEX review on symptoms, treatment, pathophysiology, nursing interventions, and pharmacology. This review will also discuss myasthenic crisis vs. cholinergic crisis and the Tensilon test (Edrophonium).
What is myasthenia gravis? It is an autoimmune condition where the body attacks the receptors that allow for voluntary muscle control, which leads to muscle weakness.
In myasthenia gravis, the nicotinic acetylcholine receptors are attacked by antibodies created by the immune system. This limits the amount of acetylcholine that can transmit to the receptor leading to muscle weakness. In addition, the thymus gland plays a role in myasthenia gravis in that it may create the antibodies that attack the nicotinic receptors.
Symptoms of myasthenia gravis include: weakness of the neck, face, arms/legs, eye drooping (ptosis), mask-like appearance that looks sleepy, recurrent gagging or choking while eating (difficulty swallowing), strabismus, slurred speech.
Treatment for myasthenia gravis includes: anticholinesterase medications (Pyridostigmine) which will make more acetylcholine available at the receptor site to increase muscle strength. Also, corticosteroids or immune suppressors can be ordered. In addition, a thymectomy (removal of the thymus gland) can be performed along with plasmapheresis with IV immunoglobulin therapy.
A tenslion test can be performed to diagnosed myasthenia gravis or differentiate between myasthenic crisis vs. cholinergic crisis. During this test signs and symptoms will improve after the administration of Edrophonium if myasthenia gravis or myasthenic gravis is present, but worsen if cholinergic crisis is present.

Nursing interventions for myasthenia gravis include: monitoring airway, assessing neuromuscular status, monitoring for aspiration, education about food and activities, following speech pathologist recommendations etc.

    I don’t know if you all share my feelings but I don’t understand why all nursing programs are so difficult to follow and then I come here and find Sarah and she makes everything so simple to understand. Schools should follow your system. Thank you, you are my savior.

    I am currently 65 years old and was diagnosed in 1984. I have lived a pretty normal, just retired!! Just wanted to tell anyone that with proper care you can get through this. I never told my co-workers or my employer, made that mistake once and was "laid off six weeks latter". I than worked my next job for 25 years!!!

    Hi Sarah, Thank You so much for this video. I am neither a doctor or nurse, but an actual MG patient. Until I watched your video I did not full understand my condition because my doctors either didn't understand or didn't want to take the time to actually explain to me what in the hell was actually going on with me.     I remember when I was in college. Our professors would normally ask us to report. Instead of understanding the lesson well, we tend to get bored and not understand the lesson well. When I enrolled in the review center, the reviewers were able to simplify things. Thanks for simplifying the topics. You are a genius.     I didn't know that I have MG since last year (when the pandemic started). I can't take shower/toilet, can't even comb my hair. I fell on the road few times when I'm on my way to work. Also fell in the toilet several times. I feel so weak, can't eat/talk/swallow & can't do any expression on my face. losing so much weight, from 52KG down to 40KG. It took more than a year before the doctor can confirm that I'm having this disease. I try my level best to be strong & sometimes I feel so down, hating myself & cry. Cuz I didn't expect this will happen to me at the age of 31. But the current meds that the doctor gave me, is helpful (I'm on medication now). It might not last long, but I can walk now, but still can't run.

    I believe I have this, is how I found your video. I do not have the best health insurance down here in Florida. So I can not pick and choose what doctor I want to see. I’ve been complaining about my issues for years. It started with my eyes. I ended up with prisms glasses. Have double vision. I had to have upper eye lid surgery because my eye lids made me unable to open my eyes hardly at all. My arms and legs hurt so bad. I can not hold my arms up. Even the simple task of folding towels has become very painful and exhausting. My voice has deepened. Chewing food hurts my jaws as much as it’s exhausting. I have zero energy. In order to go anywhere at all, I have to take a nap prior. Then when I return home I’m completely exhausted. Peeing has become an issue for me. With all of these issues. Yet no doctor has ever tested me for anything. I have high pressure hydrocephalus and a vp shunt. So every issue must be related to that they say. They put on blinders and refuse to see anything else. I have recently started seeing a new neurologist. She really isn’t listening or asking me any questions. She gave me a brief test and wanted me to close my eyes. I was almost in tears and was afraid I would fall. I can not close my eyes and stand for more then a couple seconds. I waited four months to see her. She is performing a nerve test on me January 4. I am unsure why or what she’s trying to accomplish. I’m not nurse. But your video made me feel I finally may know what’s going on with me. Now how to get a doctor to test me for this? Sorry this was so long. But thank you for allowing me to get it all out.

    I remember watching this video a few years ago when I was in nursing school. I didn't like neurology so I had a hard time to understand this topic. The name "myastenia gravis" sounds really cool to me, and I keep remember that in my mind. Long story short, I recently got diagnosed with general MG. I didn't finished nursing school because I had a hard time during the last year. Looking back I think that's when I started having MG symptoms but never realized it untill 4-5 years later.

    MG is something I tried to learn about so many times (it has never been in my scope of practice tho) and I always failed - because every time I encountered that condition, I thought "ah, MG again, I looked this up so many times, what was it all about again?!".     I really appreciate these videos as a nursing student. For those that wonder 'why can't nursing school be like this', there are several reasons. The first is the accreditation requirements. My understanding is that these are quite strict for nursing schools and, as with all strict regulations on educational programs, it can lead to teaching to requirements, which in turn can hamper a teacher's creativity. There is also a high emphasis on 'pass rates'. Thus, schools are likely to enforce the 'tried and true' curriculum that has maintained their NCLEX pass rate at a certain percentage as opposed to letting teachers improvise. As much as nurse educators say 'we don't teach to the test', the fact is that nursing schools do teach to a test because if they didn't get high NCLEX pass rates, no one would attend the school and nursing schools, like most other schools in the US, are ultimately a business. Lastly, there's the factor on instructors. To be honest, my nursing instructors have mostly been great. However, many of them are asked to double as clinical instructors, which in turn means they need to work clinical hours to maintain their nursing skills and certifications. This can stretch teachers pretty thin when you are teaching 2-3 days per week, working another 2 days in the hospital and then having to balance your remaining time against lesson preparation and grading and having an actual life. As an older student, I've had the benefit of being more candid and straightforward with my professors and I've seen how much work they need to put in just to maintain their certifications and do the basics of teaching, preparing lessons and grading. It's a ton of time and their pay, while not bad, isn't exactly stellar either (I'd make about the same amount of money as a new Crit Care nurse in most hospitals). Just a few thoughts since I see several people complaining about nursing schools. These are the realities that I've seen in my time in school. Personally, I think the best solution would be a shift away from classrooms towards clinicals. In other words, more clinical time, less classroom time. I think this would be the most beneficial. Students would be evaluated on their ability to actually apply skills and knowledge on real patients. This would enhance the training of skilled, safe nurses. It would also be easier (in my opinion) for students. Most of us learn best by doing and seeing and clinicals are just that. For example, I was struggling with sickle-cell and so my Pedi clinical instructor placed me with a patient who was recovering from sickle-cell crisis. It really enhanced my learning and understanding of the disease process, treatments and nursing interventions in a way that no class or video ever could (because I was actually seeing and participating).

    Yay for an MG video from you! Timing is everything, as I just had lecture on this yesterday. I found your videos and resources a couple weeks back when covering cardiovascular, and even though I've just got 11 months left for my LPN-BSN, I will be utilizing your videos until I'm done (wish I'd stumbled across these much sooner).     Thank You! My older brother just got the bad news of having this. The Doctors said his blood is pretty clean. To be honest, I know his life and mine will never be the same. He has all these signs. We have a lot of Dr. Quacks "Doctors" out there in the system. misdiagnosed by hospital one and the doctor number one... Quack... I would never remove thymomas .Doesn't help! Thanks for you expertise! These symptoms aren't hard to notice...
