My approach to solitary pulmonary nodules

Develop an approach to solitary pulmonary nodules by learning the different features that differentiate benign vs malignant disease. We will begin by reviewing the definition of a solitary pulmonary nodule.
The main goal here is to gain an appreciation for what makes a pulmonary nodule likely to be benign and which features increase the probability of malignant lung disease. We will discuss the causes of pulmonary nodules. The underlying aetiology of malignant and benign lung nodules is vast and will not be the primary focus. Instead we need to develop an approach to solitary pulmonary nodules that allows us to risk stratify patients based on the radiological features of the nodule.
This lecture will help use to determine the chances of a nodule being cancerous.
The Brock model for pulmonary nodules will provide us with a cancer prediction probability using the features we have discussed in this video.

    I had a lunes nodules 10 years ago next month fortunately it was caught early it’s was 2 cm lost half of my left lung my surgeon gat a good margin removed limp node and no treatment was needed thank goodness so far I’m good have 3 more nodules in my right lung so far they haven’t grown too much but still I’m being checked every year fingers crossed it will stay stable

    Thank you! Excellent video with actual examples and great visual aids. You provided a wealth of information and I was surprised when you said it was your first video. You are a great lecturer. 👍 I loved learning about the the Cancer zodiak sign and how its claws resemble spiculated margins. I'd like to see a future video with examples from scans on bronchiolitis obliterans. That's my most recent diagnosis. A 5-mm solid nodule in my right upper lobe was incidentally found on that CT, which is how I found this video. Hands down, you provided the best info on this subject. 👏👏👏

    Thank you ! For clarification I just personally experienced a pulmonologist that dismissed my nodules , but you just stated anything over 3 is a mass he stated in my case left lung 1.5 & right 4 so I was correct to seek a second opinion. Asap

    @@Davidh741 (cm ) I was actually thinking of you earlier this week , I’m embarrassed to say I have not found a new Dr. as of yet considering I will be changing Medical Insurance in Oct / access in Jan , but silently it worries me very much ! If you know of Dr. I’m this area I would appreciate your pointing me in the right direction.

    Thank you for this doctor I have been diagnosed with a nodule but it changes in size the last time I saw my doctor he said it was smaller I am concerned he discharged me I’m confused

    Is the overall amount of radiation a patient receives from imaging monitored? I have had 2 PET-CT's, 8 fluoroscopic procedures, countless CT's and x-rays. I am scanned so regularly because I have ankylosing spondylitis and a rare type of vasculitis with cardiac involvement. I also take immumnosuppresives and get frequent chest infections. I cannot have MRI's due to an ICD pacemaker. What are the long term effects of radiation exposure from imaging?

    Curcumin is suppose to protect the good cells from radiation but not effect the cancer sells from getting their radiation treatment. A naturalpathic doctor could tell you the dosage.

    I have a Tirads 4 thyroid nodule on my left thyroid, I have to wait until March to get this thing biopsied. However- I got a chest CT for something totally unrelated and they incidentally found a 3mm module in my right mid lung, classified as “most likely benign”. How likely, if I DO have thyroid cancer, how likely is it that the thyroid nodule and the lung nodule are somehow related? Did this Mets to my lung? I’m 23 y/o female for reference. Also- I understand I shouldn’t be consulting the internet but my anxiety is absolutely killing me over this. I feel like I have cancer and it has spread to my lung. I am worried sick!!

    I have a 1cm Speculated nodule in my right upper lung. The firs two CTs didn't mention "Speculation', the 3rd CT did? Does that mean it grew?

    Speculation is an indicator of malignancy. You should definitely get a Petscan or a biopsy to know what it is.

    Nope. The first nodule has stopped growing. But, in the last six months, a New 1.5 cm "Irregular'' ( Spiculated ) Nodule has appeared. I am scheduled for another CT in six weeks. It is almost comical now. .

    @@Stella20450 I had a Pet scan of the first Nodule. The test was done by two complete morons. Neither of the two were able to find a vein in my arm, so they shot my hand with the radiotracer. And, they tested me 15 minutes after the injection. The test showed a moderate level of input. The thing is, due to my Emphysema, surgery or Biopsy is just not possible. So, the Doctor choose a wait and see. My new nodule, also 'Spiculated' will be the 'Tell'. Due for another CT in six weeks.

    I have psoriatic arthritis and am getting my 1-year followup CT in May for a 5-mm solid nodule found in my right upper lobe. I understood from this lecture that nodules resulting from RA (or similarly, PsA) will most likely be benign because they are caused by an inflammatory process (scar) and not from cancer. As a matter of fact, I have multiple lesions in my brain. Several MRIs ruled out MS because they remained unchanged. It was after that that I was diagnosed with PsA and the MS neurologist said that would explain the brain lesions. Evidently my brain had been inflamed from my PsA at some point, which caused the lesions. Having an inflammatory autoimmune disease can affect every part of our bodies. My pulmonologist, who is following me for another issue, ordered the followup CT because I have recently lost my appetite and am losing weight, and I also have been having night sweats. Fatigue is always present with my PsA. Best wishes to you both. Let's keep each other updated with posts here. It will help this channel as well.

    ​@@radiologytutorials I'm not a doctor but a curious retired medical transcriptionist with multiple medical problems who has Dr. Google on speed dial. 😂 That's how I found this video, because I do have a right upper lobe 5-mm nodule. One-year followup CT next month. 🙏🤞 My research only revealed one theory, that more cancers develop in the upper lobes because of inhaled toxins from the environment. My question to you, doctor: does this mean when we inhale, air first fills the upper lobes before the remainder of the lungs, thereby getting more of the toxic material? No such thing as a stupid question, right? I always thought the lungs expand all at once. 🤷‍♀️ Just trying to make sense of the theory. I'll try to find it again.
