For number 5 of the first MTP how do you know it's osteomyelitis and not chronic gout (tophi)?

Apart from bone, you can see soft tissue aroung that joint, and you see clearly its not swollen, in GOUT it would be swollen and enlarged
For the posterior knee dislocation, it is it posterior as it’s the distal bone the tibia located posteriorly to the femur?

Yes, with a posterior knee dislocation the tibia is displaced posteriorly relative to the distal femur.
