    how long does it take for a cavity to turn into a root canal treatment. i have 5 more days till my dentist will do my filling but the tooth is very painful and now ive got a bump on my gum so i am scared its turned into a serious cavity which requires root canal? or does it take moths?

    Hey Doc, is there a reason why you did the crown prep first followed by the build up or is it your preference? Typically at the dental school clinic with teeth that have lost a lot of tooth structure, they would have us clean out the decay, do the build -up and then crown prep as the last step prior to impressing. And those crowns look really neat!!!

    Great question! Same thing was taught at my dental school. I believe they teach you this to get a better understanding of the foundations of cavity removal and bonding protocol. However, this is an advanced case with extensive decay around every single tooth. It’s not efficient nor reasonable to remove the decay first followed by crown preparation since the cavity removal process would be very difficult to access due to the undermined enamel & most likely you would not be able to properly remove the entirety of the decay if you were to approach it that way.     Any tips for avoid pulp exposures here? Did you have to do partial caries removal and leave some? Also what was your bonding agent & core material? Great case! Thanks

    This a super unusual case where we have receding pulp on all the anterior teeth which is why I was able to avoid pulp exposure. However, in order to avoid pulp exposure I do three things: use a LARGE round carbide bur when removing decay which will allow you to avoid pinpoint exposure, start with the perimeter of the cavity and work your way towards the middle, Air abrasion therapy as you get closer to the pulp horn. Typically I use clearfil se protect as a bonding agent but for core build ups I use photobond with core paste.

    Awesome case, I have a question though I was told to not use Polyvinylsiloxane with metal trays due to possibility of tray getting stuck in an undercut. You would have to cut the tray out with a bur. Have you ever heard this before?

    Yeah great question. Typically I use plastic trays for these impressions but for some reason I was not able to fit the trays we had in stock to the mouth of the patient. The metal tray fit passively so without wasting any more time I decided to just use what we had. Awesome comment!

    This is crowns from upper right canine to upper left premolars. We will be working on all her teeth over the course of the year!
