But the Plastic Surgeon's technique of 'skin tissue expansion,' gradual and most importantly painless, skin stretching, applied to the vaginal opening, with the 4 fingers of both hands pulling sideways, over 7 consecutive days, from 26 weeks, achieving 10cm diameter prior to birth, or even up to the second stage of birth, makes the destructive cruel episiotomy totally avoidable/obsolete. As 10cm is the maximal opening of the pelvic outlet. Wikipedia UK 'Perineal massage' being the traditional way of aiding a natural birth, with no damage to mum or baby. It avoids the possibility of death of mum from the complications of an episiotomy: notably, sepsis following 'necrotising fasciitis', and haemorrhagic shock, as the pelvic floor muscles have a good blood supply and bleed briskly and are not sutured immediately by skin-glue, so exacerbate the blood loss of detachment of the placenta in a prolonger labour, with resulting atonic uterus. I haven't read any special name of this knot. You need to put this suture one centimeter above the apex of episiotomy then continue simple suturing till junction of vaginal mucosa and skin.
