Knee Examination - How to perform a KNEE EXAM for Orthopaedic Clinical Skills OSCE - Dr James Gill

The knee examination is a core skill - in this video, we demonstrate how to perform a KNEE EXAM for an Orthopaedic Clinical Skills OSCE, which should be one of the easier examination stations for medical students.
For a passing grade in your Clinical Skills OSCE, a knee assessment should have the following special tests:
- Anterior Drawer Test or Lachman's test
- valgus stress test
- varus stress test
BUT a McMurray's test is rarely used to test for meniscal damage as it is possible to cause additional trauma with this older aspect of the examination of a painful knee

This knee exam follows the standard "Look, Feel, Move" orthopaedic exam approach, and overall order as set out in MacLeods Clinical Examination
Knee pain and knee complaints are some of the most common reasons that patients will consult a doctor, so being able to correctly examine the knee is about more than just passing your orthopaedic OSCE station, it is about being able to assess a knee injury and understand what the causes of a painful knee are for each patient. This will apply whether during medical student training or the years as a doctor
Please note the examination of the knee joint in orthopaedics should technically also include the hip examination as you should always examine the joint above and the below the joint you are focusing your examination on

Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However, during OSCE assessments, different medical schools, nursing colleges and other health professional courses will have their own preferred approach or order for a clinical evaluation - you should concentrate on THEIR marks schemes for your assessments.

    @@Cindee-Schnitman im not a doctor but there could be an issue with your meniscus(fibrous cartilage) in your knee joint. Alternatively it could be ligament problems. I currently have a tear in my Meniscus so im speaking from just the research that I've done over the past few months. Im too scared to go for surgery thats why im still sitting with the issue just btw. Hope that helped

    Ah yes. Short term memory recall as part of a cognitive assessment. Three unconnected words, which the patient should know and understand However the words are supposed to be objects. So “Honesty” wouldn’t technically be appropriate I normally use “Ball, pencil and car”

    so the first video i watched before this was the hand examination and i'd just like to point out the improvement on the sound. On the last video there was a "bump" in the audoo whenever you cut the video, but with this one the sounds continues smoothly. That's pretty cool!

    May I ask, as a physiotherapy student who just got introduced to all the knee/patella tests, when you pushed down on the patients left patella it seemed more loose/ to give in quite more than the right one. Would that be a positive for this test? Or is it maybe just the different angle that made it seem so?

    Hi Dr Gill. I had reconstruction of my acl in 2015 where a hamstring graft was used. Since then I never been able to achieve full flexion and do have significant tightness when stretching the knee during flexion after warming up. I occasionally during the colder weather get a dull ache deep in the knee and it's rather uncomfortable where the surgical bolt sits in the shin holding the graft in place. Any possible reason why its difficult to improve flexion despite regularly stretching?

    Flexion, ie bending the knee needs a full movement of the ligaments and tendons, if there is shortening it might restrict movements I shattered my elbow a few years ago. Even though I did all the physio, it won’t fully straighten, likely due to a shortened bicep ligament. Any surgery can induce scarring, which simply doesn’t stretch as well. It’s always worth whilst discussing with physio, but they might not be able to make huge changes due to the time frame since the op Hope that helps

    I’ve got a question Dr Gill, my knees crack/make cracking sounds with almost every movement. It’s been like that for years now, I am only 19 though. My knees are slightly turned towards each other and hurt after driving in the car, especially the left knee. My doctor said there is nothing we could do about it, is that true?

    Go for an MRI scan if possible. There may be an issue with your tissue in the knee. Possibly a meniscus tear? Dont leave it unattended please, you have your whole life ahead of you.

    Dr James Gill haven’t been able to get in with the doctor due to covid. It hurts only when bending knee and specially when getting up from sitting. No pain while walking or sitting. Hoping to get it checked out soon.

    @@tetesworld I don't know if you've got it checked out or what the COVID situation is for you now, but I just wanted to suggest communicating over phone calls, video or email if nothing else - it's better than nothing! You could detect problems, or your doctor could see that you might have an urgent problem and push for tests or visits (done as safely as possible).

    Told a doctor my left knee hurts chronically. They did not do all this. Just sent me for an xray. Long story short my knee still hurts like hell.

    Are you able to ask for an ultrasound perhaps? That might show if there's something in the muscles that won't appear in an X-ray. I had monoarticular arthritis in my left knee last year - the ultrasound found excess fluid around my knee where the X-ray showed nothing at all. Unfortunately there was no cure - I just had to wear a knee brace until it healed 😅

    As someone who’s gone hilariously flying across the floor when getting up from squatting because my knee didn’t come with me and for which has popped my knee out of place in so many strange/hilarious ways I cringed just seeing them get on and off that table so quickly, haha, if only it were that easy for me still. Gotta love hereditary joint problems…

    Leg is important for hip and knee pathologies, differences in length can affect gait
