Even if it's not all 999k you can still make perfect gems because of bonus points. For example You only have two 999k gems and the third one is around 600k, the outcome with bonus points will make it perfect gem.

No, you can make a 2 999 997 perfect gem with any gem bigger than 1 000 000 with bonus from the guild, color festival, or the bonua form the respinning of a bug golden gem but it is incredibly hard to get so just focus on the bonus from lvl 40 guilds and color festival wich helps you with about 300k-600k(guild) + color festival

If the gem is too big for the current totem, you need to move to the next dungeon. You can do rituals with smaller gems till you can move to the next dungeon.
