    I'm not a fan of internal release but, everything else you have said in this video is amazing. It's so refreshing to see someone addressing other ideas and not just focusing on "that hurts so I will press on it" technique. The body language reading and actually listening to the patient is a huge benefit. You really seem like you are at the forefront of what you do and are looking to make breakthroughs and advancements in treating people. If only you were in nz so I could see you lol.

    OMG thank you, I've been in pain for the last 7 months (I'm a woman) and last week I cried all evening for the first time from the pain... this gives me hope for the future as I am to see a specialist in vulvodynia in a few weeks. Thx !

    @@BeautifyingBasics Hi in the past year I received trigger point injections (yes in there...) It's painful and uncomfortable but I found I helps. I also started pelvic floor PT, I have muscle spasms I also think it's really helping. I would say on a pain scale from 1-10 I'm at 1 or 2 so good. I am very lucky to live where I have easy access to a pelvic floor clinic. We need to spread the word in the hope that those kind of treatments to become widely available.

    Thank you so much Dr Jilly for this clear explanation. I have been suffering from pelvic pain for years, which has notably reduce my quality of life. I would like to ask you if you know something about the Wise-Anderson Protocol for pelvic pain syndrome and if so, which is your opinion about it? I would greatly appreciate any comment you may have.     Stretchout strap accompanied by book of lower body stretches. Replicate as many of these while butt on wall, legs up wall.

    As a woman dealing with this for a while now, you've opened my eyes. I've had chronic moderate/severe anxiety/panic and I believe 100% this is causing the super tight structures down there..... If i can work on my brain turning off or tuning out, what a great start that would be!

    Hi both. It's great to see you've made the connection between your emotions and pain states. We know emotions (which are chemicals when it comes down to it) can produce different changes in your immune system and change inflammation, percetion and reporting of pain. Emotions have direct effects on pain. So keep up the good work!

    Same here I have sooo much anxiety and I believe it is the root of many of my health issues, plus getting older post menopause and medication which I believe I'm on cause of anxiety. I have had an overactive bladder for 24 years that began with yeast infections but caffeine n anxiety keeps fueling it I feel. Never had a UTI until going on medication and the first 2 were asymptomatic but the last 6 months my anxiety is through the roof. One that after the next and everything is lingering n overlapping..finally a month ago I had wierd scary uncomfortable symptoms waited 6 days till my Drs appointment. She said I had a minor UTI n gave me antibiotics but the symptoms aren't gone and spreading. It's been 2.5 since finishing antibiotics so I think I will see my Dr again. Of course I research and worry which fuels my pain. I am thinking either I have suddenly developed kidney disease..stomach cancers and IC bladder etc or it's my extreme anxiety and medication side effects. I have an appointment with a hypnotherapist later the week n I will see my Dr. I'm on a high dose of a medication n it caused side effects from day 1 now and it's been a few years but it's all culminating so I have a feeling maybe it's the medication n my anxiety. Scared

    Could you do a video relating to pelvic floor spasms and hyper mobility connective tissue disease? My hips and spine partially dislocate throughout the day. The hip that dislocates the most often has chronic spasms and that’s also the side with the worst pelvic floor spasms. I’ve done 10 years of physio but wasn’t able to make much of a dent since standing or walking causes the bones to go out of place. (I also have nerve disease, many infections, and chronic viral encephalitis that has resulted in massive brain damage and organ failure, but I’m trying to take one problem at a time because it’s so overwhelming and I’m not even 30 yet).

    Jilly, How often does sacrum and lower back pain accompany pelvic floor problems? I've had chronic and sever sacrum pain and lack of mobility for 30 years but still not healed completely yet. I wonder if my QL, and SI, and of disc could be effeted by the pelvic floor . I do have groin and floor tension and pain too, for years now. Been working on it by squatting daily with some mild results.

    You are amazing! Thank God for people like you who have such knowledge, passion and COMPASSION for those of us dealing with this pain. I’ve learned a lot in this video. Also, now I know why my physiotherapist is always so positive and upbeat when doing the internal pressure. Instead of nearly having a full blown panic attack initially, now she makes it feel routine and normal. Thank you!

    There is new research proposing that anxiety and IC are on the sane gene. Twice after a prolonged IC episode that cleared my anxiety disosrder has returned, once in full force. I am on my second rround of internal release but it's not as effective this time. This video is very helpful. I know that I need to continue releasing my pelvic floor.

    Correction. After two episodes of IC anxiety arose. This is not an unusaly sequence.

    Hi Jilly. I have Interstitial Cystitis and Hunner's Lesions along with a tight pelvic floor for which I've had physiotherapy but 9 months later I'm experiencing spasms again. I understand what you're saying about pain but would Hunner's Lesions make my condition more painful or not make a difference?

    Sorry I didn't mention that my pudendal nerve (rectal branch) is compressed or inflamed. I have a heartbeat in my rectal area all the time and the feeling of gummy worms in my pelvic area (sometimes). I have pain (electronic shocks) predominately down my right leg and now its radiating to my left leg. Will the wand help?

    Chronic pelvic pain Myofascial trigger points Gluteus minimus and piriformis trigger points can be whats causing pain down your leg.

    I have the same thing, I do use a wand but have to be very gentle. Too much pressure will increase my pain and put me in a flare. I have a great therapist, and says gentle, gentle!

    Unfortunately it is easier said than done to find a GP or physio who is really versed in pelvic floor pain manipulation. Any suggestions please? I had been hoping to visit yourself once lock down was over but I don't think this is an option any longer. The physio I spoke to recommended I do this as she felt she was not as expert as yourself even though she was recommended by the hospital. It is obviously quite a skill.

    Great video! Interesting to see how the views on the much touted 'trigger points' phenomenon is evolving. I have to admit I have found having internal trigger point work done quite limited in effectiveness. I have been to 3 different pelvic health specialist physios and had quite a lot of internal work done, as well as playing around with it myself, and although I have now gotten to a point where I have very few detectable painful internal areas, my symptoms have barely changed (genital pain /numbness / coldness, radiating down to feet, pain upon bladder filling, mild unilateral suprapubic and lower abdominal discomfort (especially with many exercises and after eating certain foods and drinks), sense of incomplete voiding, occasional perineal/ rectal /tailbone pain and extreme tension). The only exception to this was on 2 occasions when I had a DRE and a Meares-Stamey 3 glass test (both of which came back negative), both of which involve palpating around the prostatic region of the pelvic floor. For about 3 days after these medical exams my urinary symptoms significantly improved, but unfortunately eventually returned to their usual troublesome state. Not sure what is going on there! As you say, internal work doesn't do much for some.

    Robert White sorry to hear that. Unfortunately it also comes down to therapist skill and experience. Never let anyone tell you you're not the expert in your own body - sounds like you have part of the answer. Perhaps if you ask your professional to focus around your prostate region? And maybe have a look for someone who can put the whole package together, looking at your lifestyle, nutrition, stressors and provide a more holistic route to recovery. Its definitely not one size fits all! Good luck.

    Hi Jilly thanks for replying. Finding a decent physio is also key as you say, and in my experience is v difficult. I was really enthusiastic about physio when I started it over a year ago and was getting it weekly or even twice a week at times, but as time went on (and so did my cash and loss of holiday leave attending appointments) the dwindling results unfortunately also made my faith in physio helping me dwindle. It got to the point where I was dreading my physio sessions, and about 70% of the time i had a flare up for a day or 2 afterwards. Gradually the anxiety over the thought of attending drove me to quit for quite a few months. I could feel the frustration building in my physio at the time and eventually she realised progress had stagnated for a while and hinted I explore other avenues. This did the opposite to building a relationship and I felt she had given up on me and I gave up on her. The few physio sessions that consistently helped were not internal but based externally around the bulbospongiosus and ischiocavernosus, which reduced my genital pain a lot. However all of the pelvic physios I saw were very reluctant to go there on a man as they were squeemish about going near the penis, which was unfortunate as it is a very important area to work on in male pelvic pain (see pelvic physio Sara K. Sauder's blog). Im not sure if it the fact that almost all pelvic physios are ladies, and that they really don't like touching male parts? Also for men with pelvic pain when doing internal work there is sufficient cause to explore issues in muscles near and around the prostate (which is recommended in the Wise and Anderson protocol and is likely to be the reason why traditionally prostatic massage may have indirectly relieved symptoms) that can definitely be a strong factor in causing urological dysfunction and pain, but this is also an area that many pelvic physios are reluctant to explore. Recently I got a chance to be on the books with supposedly one of the best NHS pelvic physios in London. After my first appointment a few weeks ago she was refreshingly honest that in my case, managing neuropathy and also the mental impact of the condition should be the main treatment branches, with physio playing a secondary role. I follow a painful bladder / prostatitis friendly diet, meditate, do stretches and downtraining with diaphragmatic breathing daily, all with minor success. I'm pretty much resigned to the fact I probably have long-term nerve damage and just got to live with it.

    Robert White I'm sorry you had that experience, Physio shouldn't hurt. Flare ups can happen but shouldn't continually. That's a shame. I'm glad you found someone to work with. You sound really well clued up and in tune with your body - as I said before, you're the expert, we're here to guide you (with evidence and experience) where to go. Sounds like you have a plan now.

    Just because you may have nerve damage doesn't mean you are done. Please, search Pudendal Nerve damage. As I read your symptoms I relate so much to you. After a whole year trying to find an answer to my pain and multiple therapies, I'm looking forward to find a doctor that can give me a pudendal nerve block shot. Maybe you could benefit from this as well. Living in pain shouldn't be an option. My best wishes for you.

    not neccessarily, it's a different plane of tissue we're talking about. A good pelvic health physio can look at internal and external mobilisations, massage and stretches that shoud help.

    Someone please help me . I’m trying to understand and she’s doing a really great job but this is new to me and I am alone with just my son and I and I’m going off the deep end. I’ve had panic attacks but this is something else. I don’t feel like myself. I’m terrifying my 20 year old to break down and I love him too much to have him see me lose my mind.     you are in California home of the Anderson Wise protocol, they do one or two each month. Wise is the Author of a Headache in the Pelvis and no one knows more about this than him, he suffered from this for many many years. Call him! He susally picks his own phone or will call you back. Tim Sawyer is his PT, look him up too, hopefully they still work together, Tim is the Best pelvic Floor PT on the planet. You are lucky to be in Cali. hope this helps.
