Invokana, Farxiga, Jardiance, & Steglatro. What are SGLT-2 inhibitors?

SGLT-s inhibitors reduce blood glucose with the unique action of causing the kidneys to dump some of the excess glucose. In this video we discuss the good and bad about this category of medication.     I was started on Jardiance and Trulicity close to a month ago. My glucose levels went from the upper 200’s fasting to 80-90 fasting. PLUS I’ve dropped almost 20 lbs. For once I’m super excited for my follow-up with my provider!

    ​@@sharon268 My husband started wetting his pants and we were house bound for 3 years. He also had severe uti's that required hospitalizations. He ended up with 2 cardiac arrests and a pacemaker. Strangely enough his arteries were completely clean...he didn't have any heart attacks just cardiac arrests out of the blue. I woke up one morning to the sounds of death (agonal breathing) I revived him and called paramedics. I now have ptsd and he has a pacemaker. Yet, they keep pushing this garbage

    Wow! Your explanation is so practical (of how these meds work) and the overall info in this video is presented in such a positive and encouraging way that it brought a tear to my eye. As a Diabetic patient for 20 years, I have had many Dr./ healthcare pros talk down to me due to this medical condition and poor sugar control, I have been given bad advice and instructions such as "take insulin and eat what you want" to " don't worry about side effects the sugar in your blood is worse" (when I expressed concerns) to the point that I became a bad/noncompliant patient and would stop taking medications. I recently ran out of my Jardiance Rx and was considering not getting a new one because other videos overemphasize the risk and don't explain well enough how it works. Jardince has been good for me, that's why I was on the fence about it, but after watching your video I'll be headed to the pharmacy for my new Rx right this. Thank you for this video, I look forward to seeing more of your content.

    Currently on Invokana for a few years now, and when I first started on it I lost a ton of water weight, weight in general, and my a1c went down. I have a very very sensitive system, and Invokana was one in this group that I could handle. I pee constantly and it smells weird but I'd take that over other side effects! Sadly, alone it isn't working for me much anymore on 300 mg, but I am thankful for using it for so many years with no terrible side effects!

    I used Steglatro and it worked. It got my blood sugar really down and me too. I felt hollow and like throwing up all day. Worse, I was literally camping on the toilet. I normally pee a lot, because I drink a lot of water. I went to the bathroom every 10 minutes. At some point I decided to stop drinking as much, but it didn't stop and I feared dehydration, so I had to keep drinking. After two days, I gave up. It definitely got my blood sugar down, but me too. Needless to say, going to the bathroom every 10 minutes means I couldn't work, couldn't go grocery shopping or anything that required me to go outside. My doctor is now trying to get me on Jardiance. I didn't ask what it was but thank you for telling me that it's just another Steglatro. I'll tell him to save his breath because I won't drink it.

    It's your body and only you know how you're feeling. These people, including your doctor, don't care about anything except pushing this expensive class of drugs. Don't allow yourself to be bullied into taking anything that's making you sick or taking your quality of life away from you. This guy is no doctor, he a drug representative pushing drugs..he couldn't care less about you. He's lining his own pockets at your expense

    This was helpful. I take Jardiance but stopped it with my doctor's approval because I had an episode of dehydration, light headedness, dizziness and nausea. This was because I went from drinking over 72 ounces of water a day to maybe 20 due to a stressful situation. Then I read about the gangrene and panicked and did not think I would get back on. But your explanation helped calm me. I have been on it for along time, and this was the most severe side effects I have experienced.

    Thank you for explaining about FORXIGA. My nephrologist has prescribed this medication for me, mainly to help control the protein that is spilling out into my urine. I am also a type two diabetic, taking metformin so hopefully this medication will help with my diabetes too.

    My doctor had me on Glipizide at one point and immediately I noticed it affecting my vision. I did a little research and found the medicine had been associated with permanent eye damage. At that point I asked to be switched to Jardiance. It works well but it too is affecting my vision to a lesser degree. Blurred vision and light sensitivity still seem a heavy price for the benefits. That said my fasting blood sugars are still unacceptable. I'm back to cultivate other options as I consider being taken off the Jardiance or finding a way to take a smaller dose. Also considering Cinnamon with my evening meal.

    @@NinaNina-pj7ht Hi I’ve been taking it for about 7 months now I’ve lost some weight and my blood sugar levels have improved. But I must say that if you take this medication you need to drink enough water or you will feel sick. Some side effects I’ve experienced are muscle pain, headaches and dizziness but it’s mostly if you don’t drink enough water. I also take it with a cranberry pill. Overall my diabetes has improved and my ac1 is nearly at normal. Best of luck to you.

    Love the information, but would lifestyle changes solve the same problems these meds are solving? Like change in diet, exercise, more attention to sleep habits. I know some people have to be on meds, but i guess generally would people benefit greater from lifestyle changes or these medications or maybe a combination of both with the intent of coming off the meds in the long run? - great video man!

    Diet and lifestyle changes are the first medication for all type 2 diabetics. My people want their cake and eat it too. Sadly if you're diabetic, you cannot. Eat low carb, exercise, and add medications such as metformin as needed. My dad had type 2 diabetes and my mom had type 1. She died. He was diet controlled

    Thank you for this informative video. I used ozempic for about a year. My levels did not go down much remaining mostly around 13, the entire year and weight loss. I started forxiga 5 mg and the very next day my levels dropped to 8.5 fasting. Now it is constantly between 8-9. Looking forward to it dropping further to normal. Ps my eat has been very low carb mostly even whilst on ozempic days. Wishing all diabetics all the best and complete healing to normal levels.🙌🏽🌈💜

    I’m on Metformin, Rybelsis and Forxiga (taken at bedtime). Forxiga gives me unreal heartburn but it works damn good! It keeps my blood sugars from going up overnight while I sleep.

    Does the farxiga keep you running to the bathroom at night? I am on Metformin and just started on farxiga today (replacing Januvia). I took a 5 mg pill this morning and have peed 6 times in the past 8 hours. I have made sure to drink a glass of water or sugar-free drink after every bathroom trip.

    @@beri232 I kept having to pee yesterday until past nightfall. I didn't get any relief until after I went to bed. The weird thing is that my only meal yesterday, about noon, was a Jamaican beef patty and a can of soup. I had nachos and cheesecake the day before, so that may have been the source of all the sugar being flushed out. I already know I will have pasta for lunch today. Oh well . . . You have given me some hope that the frequent urinations will subside. I was seriously wondering if I would have to scale these pills back to every other day. Thanks so much for responding.

    We were growing thyme for many years I used it only occasionally. Now I will dry and preserve. What do you think sage holy basil and oregano. It has helped me with loosening phlegm. I need to be regular on it , as the sinus pressure is chronic. Being diabetic and having Bp I don’t want to treat this with drugs. It is nice to see a doctor validate the potency of these herbs.

    Does taking Farxia mean that it can lead to hypoglycemia? I consume a whole-food diet with little sugar. I am scared that I would my diet will have to include more sugar so that I do not have hypoglycimea. Does this medicine make the pancreas not work anymore on its own because the sugar metabolim is taken over by the medication? In course of time, will the pancreas not work anymore at all?

    Absolutely planning on that. I have a to-do list running that will get into all sorts of different dietary aspects of diabetes. Intermittent fasting is definitely on the list.

    My Doctor put me on Trulicity for my Type 2 Diabetes. However, I started to have a lot of GI side effects (slow digestions, constipation). He then changed the medication to Jardiance. But after reading the black box warning of gangrene development I am scared to take it. How common is this warning on this medication?

    Is there anywhere that lists the glycosuria thresholds for each of the SGLT2 drug brands? Which one has the lowest, and what is the threshold?

    I've not seen any data on differences from one drug to the next as far as at which glucose level they allow glycosuria, or whether there's any significant difference at all. What has been reported more widely is how much glucose per day each drug causes to be expelled. Invokana appears to cause the most glucose expulsion in the urine, but there hasn't been any clear connection between that fact and any greater A1c reduction.

    I took 10 mg Jardiance, for 4 months, i didnt loose any weight, and it didnt lower my Blood Sugar, just a little bit, but still Metformin is much better for lowering Blood Sugar

    I take trulicity and was started on steglatro a few days ago. I constantly have to pee. Like every half hour and if it stays this way I'm not sure if I can do this. It looks bad to be going on a pee break every half hour at your job.

    Hi Doctor, I'm hoping you can help me. I'm on Metformin 3x 850mg, gliclizide 2x60mg, onglyza 5mg, all in a day . Now the specialist on top of all these meds wants to put me on 5mg Farxiga. A1C 6.9 I'm scared of the chance of the flesh eating disease as a side effect with this class of drugs.What are your thoughts? ThankYou.

    I can't give specific medical advice here, but I can tell you that I use similar combinations of medication's in my patients every day. The risk of Fournier's gangrene is incredibly low.

    How would you compare Jardiance to Rybelsus? Which would you prefer and why?

    It really depends on what you're going for. As far as A1c reduction, they did a direct head to head comparison and Rybelsus was superior to Jardiance. The amount of weight loss in each was basically equal. However, Jardiance has other benefits such as reduction in heart failure risk and likely kidney benefit that Rybelsus may not have. So it's really a person-by-person thing and sometimes other diagnoses besides diabetes help me decide which medication would help someone the best.

    This video was nice to watch. I just became a patient today of Farxiga 5mg when my recent A1C was check and my average glucose was reading at 303 and register a 14% the highest I ever been. Now I'm also using intermittent fasting so I looking forward to being taken off all medicine by spring of next year. For now I feel safe using this medication, and increasing my water consumption is something I have begun to do anyway. Looking forward to a normal reading here soon. Happy to say good-bye to empty calories for life. I need to be here for my son.

    One of the actions of the medications is to allow glucose to be excreted into the urine if the blood glucose is above 120 mg/dL. That's why even non-diabetic people with normal glucose can get glucose (and sodium) excretion in the urine when taking these meds. That's one of the things that led to some recent research using one of these meds in non-diabetic people for the purpose of treating heart-failure (a NewsFlash video on that is also coming soon).

    @@sugarhighchannel thats awesome.thank you for the explanation. so, is it possible to feel super hungry on meds like this? I feel like with diabetes, most people probably exceed 120 after every meal, so when the excess of glucose gets flushed out is the person left feeling hungry? I see how weight loss could be a benefit, but all those calories lost could lead to hunger no? also- the higher dose you take, does more glucose get flushed out? thank you for teaching and answering my questions!

    Can I still be prescribed with these SGLT-2 meds if my creatinine in the blood is (1.5 against 1.2 normal) and albumin (+2)? Do these lab results tell that I have kidney impairment already? Please talk about Vildagliptin next time. Many thanks again.

    When they do the blood tests to measure your kidney function, they will usually calculate a number called the eGFR based on your creatinine level, BUN (blood urea nitrogen), and gender. Using these meds is based on that eGFR number, rather than just the creatinine. Some meds (like Invokana, for example) are approved to use with eGFR as low as 30 (90 or higher is normal). My next medication class video will include Vildagliptan. I've had to put that one on pause for a while because I have a fun idea to explain how they work, but it involves having a friend help out so I have to wait for the quarantine to lift. But it's on the way!!

    Yeast infections of foreskin continued after more than a year of use then came a UTI which ended in the hospital with sepsis. I no longer am on Jardiance which for me is not a safe drug.

    With Jardiance, you have to keep your genitals dry, especially under your foreskin. Smegma forms when it’s not cleaned properly. Men don’t usually wipe after urinating, but when your on this type of medication, you should be using a ph balanced wet wipe following urination. Staying dry and clean and NOT holding your urine is key.

    I think you’re asking about taking Steglatro and Jardiance at the same time? If so, they’re in the same family and have the same action, so we wouldn’t generally prescribe them together

    Very similar in terms of glucose control. Jardiance has the FDA indication for reduction of heart risk, Farxiga has the indication for reduction of kidney risk (although Jardiance is likely getting that soon based on some recent clinical trial data). Honestly, they’re more similar than they are different.

    i almost died from taking a SGLT-2 inhibitor. the skin infection they worn about is not something mild the name of it is fourniers gangrene and requires immediate surgery to save your life

    What good are these pills if the side effects for some people are so severe the patient is leary of taking them. Such as a Diabetic prone to UTI's. These pills have a reputation for that Or the employed person that has to continually take a bathroom break. Believe me not all employers are tolerant of these problems. I was transferred to another job because I had to take so many breaks. It does happen. I just think the side effects with these oral drugs is extreme. However these drug manufacturers are very nonchalant with the side effects. All they show is commercials with overweight Diabetics, dancing around as if everything is fine taking these drugs. While the drug people fill their pockets with our money. It's really very sad. I think I will stay with Insulin. None of these side effects with insulin.

    Unfortunately the cost is a limiting factor quite frequently. Sometimes prescribers can get the medication approved through the insurance at a lower price if they can show that a cheaper option is not appropriate or not effective for you.
