City spread may be unrealistic but maybe having slightly changing icons. (Cities get a few more buildings with development, Forts get some houses around it, Temples get a village that grows a bit) Then as the development starts to affect neighboring counties maybe some small traders could walk along dirt roads that show up at higher development levels to show the growing population and commerce.

CK3 will be the standard Crusader Kings game from now on just like Eu4 and Hoi4 for their respective franchises. You can get Ck2 as it is free to play to try it out before CK3 releases, you can get a first impression about the UI (you'll get a better feeling about how CK3's UI is, compared to CK2's) and the politics in the world. If you decide to try CK2 before Ck3 i'd recommend forming the kingdom of Norway as a viking ruler and go raid england :p, or a good peaceful start would be as the king of France in 1066 and try to keep the realm and develop it, in the future preparing it to fight the holy roman empire, england and brittany for modern france's borders.

Barony titles in ck3 are always vassals of the county they are in. However there is a new mechanic where holdings can be rented out, for example to holy orders.
