Interactive PDF in InDesign: How to Show and Hide Form Fields based on User Input

With FormMaker (www.id-extras.com/products/fo...) it is possible to create interactive PDF forms in InDesign with checkboxes, buttons and/or dropdown lists that show or hide entire document layers depending on the options a user chooses.
This can be useful for showing or hiding specific questions in a form, depending on the user's answer to other questions in the form.
In this video, colored circles are made to appear or disappear depending on the state of corresponding checkboxes.
The final PDF can be distributed to anyone who has access to the free Adobe PDF Reader. The end-user does not need FormMaker, and from his or her perspective (and in reality) the final PDF is a completely regular PDF.
Basic Javascript programming is needed, but a sample script is shown that can be easily modified for many needs.
