Incident Investigation Reporting

This video shows a simple incident investigation and demonstrates the Employer Incident Investigation Report (EIIR) documentation and submission process.

    For a basic overview to introduce this subject to H&S Reps, I found this video to the point, without being too long and boring. Ideal for refresher training. Note, I am from South Africa, but my experience has shown it does not matter where in the world you stay, health and safety rules and regulations pretty much remain similar. I found this video very useful.

    Nice informative video with some great production value too (which isn't usually the case in these types of educational videos). Goes over things quickly and uses an example that could happen on quite a few worksites.

    At 3:34 in the video it says the joint health and safety committee must participate in any investigation and receive copies of all four reports. I think this is a 1st of April fool day prank as in the regulation it says where available and at the committee I am a worker representative in a hospital the only report is the white report without the name of the injured worker and in the majority of cases no corrective actions taken.

    +engao What regulations are you referring to? The video appears to be correct, referencing "Workers Compensation Act", RSBC 1996, c 492, ss 175(2)(d)(i), 175(4)(b)(i), 176(2)(c)(i), 176(5)(b)(i). Those four sections require the joint health and safety committee be provided with four reports: a report of the preliminary investigation, a report of immediately identified necessary corrective actions taken (if any), a report of the full investigation, and a report of necessary corrective actions taken (if any). An investigation is required to "identify any unsafe conditions, acts or procedures that significantly contributed to the incident." Identification of any of these requires corrective action(s) to be identified and implemented. Having no corrective actions taken should only occur if the investigation was unable to find any unsafe conditions, acts or procedures that significantly contributed to the incident. As to the first part ("the joint health and safety committee must participate in any investigation"), this comes directly from the same Act, ss 130: "A joint committee has the following duties and functions in relation to its workplace: [...] (h) to ensure that accident investigations and regular inspections are carried out as required by this Part and the regulations; (i) to participate in inspections, investigations and inquiries as provided in this Part and the regulations" Finally, I'm not quite sure exactly what you're referring to when you say "as in the regulation it says where available". The regulations *do* spell out various rules determining whether a worker representative is "reasonably available." But generally speaking, a worker representative is **entitled** to assist in inspections and investigations; this cannot be denied unless it's for a reason explicitly outlined in the regulations.

    +engao looks like you have something to take back to your committee. The names are a privacy issue so it does not need to be provided. the committee needs to be involved in the investigations (act 130(i)) also in 175 it states (2) The employer must ensure that a report of the preliminary investigation is (i) provided to the joint committee or worker health and safety representative, as applicable, or and (4) If the employer takes corrective action under subsection (3), the employer, as soon as practicable, must (a) prepare a report of the action taken, and (i) provide the report to the joint committee or worker health and safety representative, as applicable,

    +engao Thank you for your comment. Employers are required to include a worker representative in an incident investigation where reasonably available. There may be a number of situations where a worker may not be available to participate in an incident investigation, however these should be the exception, not the rule. If there are concerns that workers are being inappropriately excluded in incident investigations, the issue should be raised at a joint OHS committee meeting to resolve. If unsuccessful, a WorkSafeBC Prevention Officer can be requested to attend a joint committee meeting and determine the employer’s compliance with this obligation. Similarly, if the incident investigation report appears incomplete or no corrective action appears to have been taken, those issues should be raised with the joint committee, and if unresolved a WorkSafeBC Prevention Officer can be requested.

    +WorkSafeBC Not the OP, but this is really great practical info. The regulations and rules are there for a reason, so if they aren't being applied properly, it's good to know what steps to take to make things right. Thanks, WorkSafeBC! 👷👍

    Where is your predictive and preventive maintenance programme? Why was the place not cordoned off before had oil spill been an imminent occupational hazard of the work being done? Where was the work permit? The video could have been a bit more detailed considering WorksafeBC is producing it!
