I would love to see you debate more traditional doctors that manage pain rather than cure issues

This fits what I feel has been happening to me for years. I have not been able to turn it off with the strategies I’ve tried but I am actively using your suggestions. I have had migraine that evolved to chronic that has been so painful and yet when I broke my wrist, I felt little pain - waited for others to finish skating, drove them home and then drove myself to the hospital to get it cast. Why all the brain induced pain yet but such a high threshold at other times?

In my experience, the problem with CBT and similar therapies is the “Cognitive” part. The problem is happening from the “unconscious”, so doing the work at that level makes most sense. Working from the cognitive (conscious) side of things seems like a long and drawn out way that may not even get across the finish line.

Hello Doctor, When you mention neurological findings excluded from your tms coverage, would this include small fibers neuropathie ? Or diminution of the number of small fibers, could be considered as a normal abnomaly ?..knowing that symptoms comes and go, move from à part of the body toanother , can be very strong or mille or even from time to time no symptoms at all..?. Thanks for your answer..

depends if it was actually totally structural or not. If it was, the fusions should help. If it wasnt, there is still a path to healing but through brain's neuroplasticity and unlearning the neuro circuits at the root of the problem

Maybe we should treat each individual as an individual instead of deciding that patients need to be treated as problematic if they take medications for pain

Not all injuries heal Doctor. That is a very generalized statement. Although what you are saying has some truth you make great leaps to string this together. 2 out of ten people have physical disabilities which will always cause pain. I have fibrous dysplasia in my femur and I can guarantee you it will be with me for life.

I haven’t listened to all of this yet but I’ve listened to Dr.Schubiner before as well As others and I don’t think he thinks everything always heals right. However sometimes we will continue to have pain even when the injury has healed right. THAT type of pain is the neuroplastic kind (mind/body-various terms for same thing). I have also heard that even in a situation like yours there can be some reduction of pain using some of the techniques of mind-body medicine. The pain is generated in the brain. So if the condition is chronic I imagine that your brain is creating lots of pain because of it’s view that you are in danger. But, if you can teach the brain that this situation is NOT dangerous, you may be able to reduce your pain. Do I encourage you to listen to lots of people and try not to be offended. There’s lots of ways to be impacted by the mind/body connection and many strategies that can be employed to use it to your advantage.👍

I think the mistake and confusion by doctors and patients and there is not a proper explanation or segregation of chronic pain due to structural pain - a cause for the pain continues to be there, or from original trauma, there maybe bone spurs that have formed and are pressing on nerves or our posture has changed due to the original trauma, causing stress on muscles tendons ligaments etc or idiopathic pain. However any pain is still real to that person, the pain still hurts but the difference is, with idiopathic pain (no longer a structural, disease or illness causing pain) but the pain receptors and or brain are still firing in these situations, for example your emotional state can drive the pain up or down, it usually possible to tone down, re wire the pain signals which are still firing, therefore it can be treatable with the right treatment, right therapist and the right mindset and understanding.

So this is all a placebo? Its not real?

It's very real. It's possible that the pain is neurological and the nervous system is causing the problems.

It's different than placebo, in placebo you're doing/taking something and expect a reduction in pain that's it but here you change your neurological circuitry and you work on the underlying issues.
