"Exposure Therapy, in a phrase, is the systematic and intentional triggering of fear while minimizing- and ideally eliminating- all pathological responses. Imaginal Exposure accomplishes this with thoughts and ideas. It is done in the context of addressing unhelpful/pathological responses to fear."
The first thing we do besides offering good education is gaining a strong assessment data- it must underlie good exposure. Know what you're working with. Make a list of your obsessions or bothersome thoughts. See a specialist. Get educated. Get to know your symptoms, your motivations for getting better, and start a running log (monitoring) daily. Once you have a tally of key examples, placing them on a hierarchy really helps to get a road map and be realistic and also to monitor progress (like the one above for arachnophobia).
Once you've got your hierarchy, we consider conceptualizing the core fear behind an uncomfortable thought or action. Start simple; don't overthink it. Once a person can tolerate facing their fear at a lower level with scripting (or exposure in general), they can then move up the hierarchy and face higher and higher ones.
Don't worry if you run into issues- that's what specialists are for! But also don't hold out from facing fears that you have a good reason to face.

I had a hard time with the goal of this exercise. Is there a scale of anxiety? Should it be reducing while I do the script. So you basically say a thing that is untrue or causes anxiety until it doesn’t cause anxiety?

Me too - I found out within 2 months. Currently doing ERP and scripts and let me tell you, THERE IS HOPE. I’m a qualified psychologist and have OCD, let me tell you, OCD is a jigsaw puzzle and is full of irony. I have been doing audio and scripts for 2 days now. You have to bring the thoughts to the forefront and allow yourself to be desensitised. It is a process but know that it DOES work.

Say to the rumination, "you belong --- I hear you, I hear the fear that is making you want to ruminate" and then do nothing. It's miraculous what simply feeling and hearing your emotions can do.

what if your intrusive thoughts aren't exactly fears or even "what ifs" because you don't believe on any level they might happen but they're deeply disturbing images & you can't stop them from looping? i don't really have compulsions, & if it's during a busy day i can usually just keep doing what i gotta do but when i'm alone or at night i can get stuck in really bad loops. & the uncertainty script doesn't feel helpful to me because i...wasn't worried about the thing, i was just uncontrolably picturing the thing. like how do i tell my imagination, "go home, you're drunk."

Hey- please can someone help me? I struggle with ROCD and have been for about 2 years now. However- a new thoughts come up that is scaring me. Whilst being intimate with my partner, I became uncomfortable and wanted to change. I didn’t vocalise this and whilst in the moment my partner didn’t really notice. Normally- I wouldn’t question this but my brain is now screaming at me- he shouldn’t have done that, he crossed a line and it’s even going as far to say it was non-consensual. (It was, I never said no or became visibly uncomfortable- I know he’d have stopped if that was the case). However my brain won’t let up. I don’t know what to think or believe. Does anyone have similar experience or advice?
