HOW TO USE AN INDIRECT OPHTHALMOSCOPE! | Explained by an Ophthalmology Resident

In this video I'll show all the medical students, residents, and newcomers to the field of ophthalmology how to use the indirect ophthalmoscope. If you are in med school or about to do an ophthalmology away rotation and want to know the basics of how the indirect works this video may be helpful to help you learn how to use the indirect ophthalmoscope. This video will be most helpful to med students who are new to ophthalmology or those getting ready to start their ophthalmology residency and want some guidance on using the indirect ophthalmoscope. Or if you're an ED doctor an have access to one and some lenses maybe you'll find this video useful. The skill of indirect ophthalmoscopy makes you very valuable for diagnosing eye diseases in ophthalmology- not many doctors can use it!

    Zak, great video as usual! Thank you for taking the time to make it. I have a question about the lenses. Have you tried the digital series of lenses from Volk? If so, what do you think of them as an alternate option to the standard 20, 90, 78, 28D set?

    Hi, I've tried the digital wide field lens. It was a little awkward for me at first after using the 20 for so long. I'm sure it's a great lens but I personally preferred my 20.

    @@doctoreyeballmd7493 Hi Zak, please correct me if I'm wrong. I think the DWF is mainly for use with a slit lamp (instead of a 90D). For BIO, Volk has Digital Clear Field and Digital Clear Mag.

    Hello bro, I have an army fundus exam after two days with an indirect opthalmoscope, is there anything I can do to decive the doctor when he looking at the fundus of my eyes? i mean is there any eye drop or anything that can cause a temporary problem? thank you.

    Just a quick question sir. Is a diopter a diopter? I guess I mean this. Are all diopter created equally when it comes to magnification, and clarity? I will get to the point. I have a couple of achromatic aspherical diopters, in 58mm and 72mm, both 10x that I use for photography. They make eyes look stunning! What is the difference between your diopters and mine? I would love to take photos of the inside of eyes! Thank You For Your Time Sir!!

    Without the BIO and slit lamp can the 20D lens be used to view the fundus by providing your own illumination? I've seen a couple of videos suggesting that. Could you clarify?

    Hi, yes. You can use your phone, turn on the video on the away facing camera. Set the flash to on continuously (that is your light source). It takes some practice but you can use your phone and a 20 diopter lens this way to view and even record video of the fundus. They even make some special lens holders that keep the 20 at the right distance from your phones camera. There is also an app that is supposed to help but you can just use the camera app on the phone. It’s not easy though, good luck!

    I’ve always struggled with the Indirect BIO headset. I’m definitely going to stick with it though, got to learn this damn thing.

    I have a doubt whether to place the light beam in centre of field or in upper part of field because when i keep in centre of filed, the lower half of image which i observe is black.

    make sure the light circle is centered in your field of view before you even use your lens

    Do you always use a scleral depressor when using the indirect ophthalmoscope?

    Often not. The depressor helps get those very far peripheral views out to the ora if you are looking for tears or breaks.

    Quick question, is a BIO a replacement for using a slit lamp to view the Retina? Maybe a dumb question...Just trying to learn!

    Not a dumb question. It's not really a replacement. There is some overlap in what they can do but there are also parts of the exam that each cannot do. A complete eye exam is going to require the use of both.

    I’ve never tried but optically speaking I don’t think it will work. No harm in trying though.

    @mohammed Hilmi the cost depend on the company ( manufacture ) of indirect ophthalmoscope/(20D)lens. The indirect ophthalmoscope has many advantages, it has wider field of view , and the use of binocular vision for better depth perception. On the other hand, the direct ophthalmoscope has high magnification ( X15 vs X3.5 for the indirect ) and it is available in almost every clinic. Hope that was helpful.

    If you have all Volk lenses they have the mulitlens cases available separately. You can check out their website or ask your local dealer too from whom you generally buy the lenses. I cannot say about any other company though.

    My lenses are volk which came with the case. Last I checked you can get an empty case for your lenses in the neighborhood of $30-50. Volk will custom make lens cases for your specific set of lenses as well. I'd just message or call them and they can likely make one to house all your lenses, likely up to 5 or so.

    Hmm. If you wat to be an ophthalmologist it looks like you'll have to be able to wink with both eyes. I wonder what percentage of med students that eliminates. Or do you have to do that in order to get into med school?
