On Center Icon make sure you also turn on "Death Warning", besides that you'll want to manually add "Infest" as a Debuff. I knew i forgot something :P

auras > Create new Debuff > type the name > add the debuff in the layout section on "center icon"
recommended percentage on hp and mana if low......didnt see it in video cuz im lazy and skip to the important parts (sorry)
What addon is for the cooldown bars you have to your abbilities?
