Guys, you need to do thorough research on your topic, nobody can tell you which questions to make and what your questionnaire should have other than you and your supervisor. Everybody can guide you overall but exactly what should be the content of your questions, this is next to impossible.

Good idea Sir x but I think most important thing is doing literature review in good way. If literature review done well researcher will learn everything thing. Making questionnaire, methodology etc.. Just read best papers in your field and identify best researchers in your field, in their work you will find questionnaire
