Can the heart murmur be heard by the patient without a stethoscope?

No. Though Some of the murmurs can be heard directly on the chest, by keeping your ear on patient's chest wall, close to the patient's heart, this no doubt unreliable without a stethoscope but you also need quite trained to pick these up.
