I've never worked for an employer that had employee medical insurance. What do I do with filling out the L564? I've had VA care for the last 7 years.

Thank you for your excellent videos. I just want to confirm that when entering Medicare Part B after leaving a qualified employer plan after 65 using the process described in this video, I can still add a medigap policy at that time without fear of denial or underwriting.

Yes, you are correct. You will be able to do that.

What about question 6, as an employer we offer both long-term and short-term disability coverage. The former employee I am filling this out for was on the short-term at the time they retired. Would I fill that section out with the time he was on short-term disability?

So basically I have to contact my employers HR dept. ???

I submit Medicare application forms for my wife and I the same day , the same envelope . Today my wife receive Medicare card but not me .Could you answer me why ? I. really appreciate it .

Thank you for your excellent instruction. I am still unclear on one thing. When you turn 65 and sign up for part A, is there a form or any action that you need to take to inform Medicare that you are delaying enrollment in part B? Or is the paperwork and proof at SEP all there is to it? Thank you!

They can be faxed to your local SSA office. Or you can simply apply at www.ssa.gov

Hi, thanks for the informative video. If I am retiring on Sept 1st of this year but I want to start paying for Plan B until Oct 1st . On the CMS-40b form, under remarks, do I write down please begin my Medicare affected Oct 1st.

Yes you can!

It was never mentioned what goes under employer in Section B line 1 and 2. If my wife is retired and I'm working would that be my employer I put there?

Hi, thanks for the informative video. I'm preparing to fill out these forms and was wondering whether for the employer address, I should use the corporate office address (in Maryland -- where the main human resources department resides) or the office address that I work out of in Michigan? Or, doesn't it really matter? Thanks.

what if the company headquarters HR does not sign but type signature of the Business name. will that be acceptable since HR claim they have never signed these forms from past employees. please help

I can't say for sure, but generally speaking, yes that should work.

Hello Sir, Nice and very informative presentation. If one is over 65 and have a group plan under FEHB, can someone apply for and collect Social Security say in January and want their Medicare to start in July since you have 8 months to apply during SEP? I am a bit confused when you said something that you can tell Medicare when you want coverage to start in the REMARKS section of CMS-40b but you adviced to keep it to like 90 days or less. So what about the 8 months window?

You have 3 months to enroll before you turn 65 and the month you turn 65 and then 3 months after the month you turn 65 to enroll. If you have any other questions or need help getting your Medicare coverage set up correctly, give us a call to schedule an appointment: 800-864-8890.

If I am retiring June 1, can I have my employer fill out the form now, indicating that I still have coverage? I would want my part b to start when I lose my group coverage, which will be the month after I retire. Or is it best to have my employer fill out the form when my coverage has already ended?

We advise having them fill out the form asap, if they have a hard end date for your coverage then have them do that sooner than later, as it could delay your processing for Part B and when you wish for it to start.

Thanks! I had them fill it out but they indicated that I still have coverage. Is it okay to submit in this way and indicate on form CMS 40b what month year I want Medicare Part B to start?

I desperately need advise on who would complete Section B on a CMS L564 for a union machinery operator that worked for a company for over 15 years. The group insurance for the union operators was paid by that company to the the union's health insurance plan and they also reported banked hours. This was only done with their union employees. The company had a group insurance plan for their non union employees. I can't find anyone knowledgeable with this scenario, including social security. Are we eligible to enroll under the SEP? We have Part A already.

Is there someone in an HR department at the company that could fill out his dates of employment / remaining info. If that doesn’t work you should also call the union and see if they have any ideas, or if there is a benefits person you can get it to.

what is the best way to send your CMS-40B form and your CMS-L564 form; via regular mail, Fax, or drop it off at your local SSA office? Thanks, Timothy

You can do all of the above, we usually recommend sending via fax, and follow up with a phone call a few days later to make sure it was received.
