If your police station accepts online FIR then you will be able to do otherwise you need to visit the Police station and lodge sanha not fir. No, only any identification proof like aadhar voter card etc sometimes some police stations need affidavit to file sanha.

No offline appointment is not possible, you will have to take appointment online, if you want, you can book an appointment in tatkal and as far as passport is concerned, passport reissue for damaged passport is possible only from your regional passport office

Hi my pasport has expired , my name....father name....mother name....has different spelling from Aadhar Card.... most important my date of birth is different from Aadhar Card.... My passport has made in year 2010 without aadhar card if I renew passport names can be corrected (adver. in newspaper and affidavit) but date of birth cannot be corrected. As my passport is old when aadhar card is not necessary......if i apply new fresh passport with aadhar card all problems can be fixed.. is there any problem ? Mujhe lagta hai ki agar apka passport pahle ka bana hua hai bina aadhar card ke to apko renew nahi kara ke naya passport banwa lena chahiye jisse apke sabhi problem fix ho jayenge lekin agar kahi passport office ko apke pahle ke passport ke bare me jankari ho gayintab apko dikkat ho sakti hai. Jyadatar chances hai ki apko naya passport issue ho jaye kiuki apke personal details ki spelling mismatched hai.

Dekho date of ka antar within 3 years hai to change ho sakta otherwise wahi rahne dijiye. Aur agar apka passport aj se kafi pahle bana tha tab aap ek naya passport banwa lijiye agar hal hi me apka passport bana tha tab reissue ke liye online apply kar sakte gai

Do tarike hai ya to regional passport office visit kijiye aur passport ka details niklwaiye ya fir agar apka apka aadhar link nahi hai to naya je liye apply kijiyega agar apka details passport office me match kiya to chalan ja paisa jayega aur reissue ke liye apply karna hoga

No there is no need for affidavit you poduce your original passport at the time of psk verification.
