If you already have Part A, and need to add Part B...
Option 1: Hand-deliver Forms 40B and L564 to your local SSA office as a walk-in.
Option 2: Mail or fax the forms to your local SSA office.
Option 3: Go to the Medicare Part B SEP Enrollment Link: ssa.gov/medicare-partb-sep
Enter your data and upload your Form 40B and Form L564.
If you need both Part A & Part B...

Then send your Form 40B and Form L564 to the Social Security rep who contacts you by phone or mail. Or just take the forms to your local SSA office.

    As of today 05/10/24 I see online portal gives the option to just show proof of Medical Insurance instead of submitting CMS L564. Can you confirm or do I still need to submit CMS L564? Thanks Larry

    Form L564 is what they are referring to, but the upload spot is being generic when it says "Click to Attach Employment Verification". Technically, in the absence of Form L564, you could provide proof in other ways, but only if your employer won't fill out the form for you. The truly want Form L564.

    On the CMS-L564 form, when requesting while still employed, is the Still Employed date the current date or the future date on which you will terminate the employment?

    The "Still Employed" date is the month/year that we're currently in when the form is filled out by HR.

    Could you also show how to apply online for both A and B when you delayed both due to HSA contributions and you had a HDHP? Thanks!

    Thanks for your question. You would fill out the application the same way, but in the Remarks section at the end, you'll want to write in when you want Part B to start. And then you'll need to turn in Form 40B and Form L564 to SSA to finalize the application.     Is the information in this video still valid as of today, 7/26/23? If I go to a local social Security office can I have the paperwork completed at the SSA office? If I am only applying for Part B(I have Part A and still working, age 70, receiving monthly benefits since 9/2022)can I apply for a drug and vision supplement at the same time, and at the SSA office?

    You can submit the paperwork at your local SSA office. See the video description section below the video for details on all ways to submit your paperwork. You can also go to our Special Enrollment Period page at www.medicaremindset.com/sep for specifics to your situation (Scenario #1...I have Part A and just need to add Part B). You do not choose or enroll in supplemental medical, drug, or vision plans at SSA. Once you get confirmation that Part B is active, you can enroll in a Medigap plan and Part D plan...or...Medicare Advantage plan directly through an insurance carrier or an insurance agent. I recommend you get in touch with an independent Medicare insurance agent who can guide you through these steps without making any mistakes or missing any deadlines.

    I submitted my application for Part B online with only the L564 form uploaded as proof of insurance. Will that be enough or should I have uploaded more?

    You're good. Form 40B doesn't have to be uploaded...but Form L564 definitely does.

    Good presentation but the SS/Medicare system is needlessly complex. This is because they have no competition so they don't have to try. They remind me of a closet where you just throw stuff in for years and years. What you want is in there, you just have to dig and dig and dig.
